                         How to fill out this Questionnaire

1. Verify that you have opened the correct questionnaire for your component and then save it
to your computer based on the instructions in tab 2. File Naming Instructions. (Example:
OIA-Pillarbase-Ap-05012008.xls, OIA-Pillarbase-SeU-05012008.xls)
2. Click on the System Attributes tab and complete the questionnaire, answering the
questions and supplying the information for your component. If a question is not applicable,
enter N/A.
3. Collect the supporting documents listed in the DR Plan Support Documents Checklist
(last block in System Attributes). Recovery Scripts and Verification Scripts are required, all
other supporting documents are optional.
4. Save supporting documents based on the instructions in tab 2. File Naming Instructions.
(Example: Pillarbase-Ap-Recovery Script-05012008.doc, Pillarbase-SeU-Server Configration-
05012008.xls). Add a footer to each supporting document that contains the file name and

6. Supporting documents can be in Word, Excel or Visio formats. Supporting documents can

7. After completing the System Attributes tab and assembling the supporting documents
please put copies in the appropriate sub-folder in S:\cco\staff\CCOSHARE\DISASTER
RECOVERY\01 Incoming Documents folder and send email notification to OIT-
8. You have the due date discussed at the Swim Lane meeting, this will give you approximately
12 business days to complete the questionnaire.
