I am issuing you this formal letter of reprimand as the minimum disciplinary action,
which I feel, is needed to correct your misconduct and encourage the appropriate
responsibility and standard of conduct.

a. Cite the reason for the reprimand.
b. Provide a detailed description of the incidents. (dates, people involved, how service
   was adversely affected by the employees conduct)
c. Summarize any previous offenses.

I hope this reprimand will impress upon you the need to (correct action/behavior), so that
further disciplinary actions will not be necessary. You should be aware, however, that
future conduct offenses may result in more severe disciplinary action up to and including
termination.
