INSTRUCTIONS FOR THE CANDIDATES


1.         All entries in the form must be made in BLOCK LETTERS or TYPED;

2.         Strike out what is not applicable, but DO NOT LEAVE ANY ENTRY UNFILLED

3.         In case an entry does not apply to you, the words NOT APPLICABLE should be clearly
           written In all other columns , appropriate replies, such as YES .NO.NIL ,N.A. etc .must
           be given;

4.         Candidate who has graduated 18 months before the last date of submission of application
           must explain what he /she had been doing during this period.

5.         Three (3) attested copies of each of the following documents should accompany the form

     (a)      Matric/O Level Certificate
     (b)      Intermediate/A Level Certification and detailed Marks Certificate.
     (c)      Equivalence certificate in case of “O” level and “A” level qualifications
     (d)      Bachelor’s degree and or Detailed Marks Certificate.
     (e)      B.Sc (Hons.)/MSc. Degree.
     (f)      M.Phil Degree and detailed marks certificates in case of PhD applicants
     (g)      Domicile Certificate (Candidate’s).
     (h)      No. Objection certificate (in case of In-Service candidate).
     (i)      Hifzi-Quran certificate (if applicable)
     (j)      I.D. Card (Candidate’s)
     (k)      I.D. Card (Father/Parent’s signed)



Note: (1) Originals of the above documents will be produced at the time of interview for
          admission.

           (2) Candidates finally selected for admission will be required to produce a Migration
               certificate (in original) on joining the University.

           (3) Candidates finally selected for admission will be required to submit an Affidavit on a
               stamp paper to the effect that his/her certificates are genuine and in case these are
               found fake/bogus/tampered, the University will have the right to cancel his/her
               admission and initiate legal action against him/her. The candidate will also be

           required to produce and affidavit in accordance with the decision of the Supreme
           Court.

    (4) Candidates having obtained certificates/degree from foreign Universities / Institutions
        should provide equivalence certificates and conversion of grades into marks for
        Bachelor’s degree from Higher Education Commission, Islamabad/Inter Board
        Committee of Chairmen for Matric and Intermediate level certificates. In the absence
        of equivalence certificates, foreign degrees/certificates will not be accepted.
        Certificates/Degrees obtained from local University/Institution with GPA also require
        conversion of grades into marks from the concerned University/Institution/Higher
        Education Commission.

    (5) Foreign students must route their applications through the Ministry of Education,
        Government of Pakistan.

    (6) In case incorrect/misleading information or concealment of facts, the University will
        have the right to refuse/cancel the admission and expel the student form the
        University.

DECLARATION BY THE CANDIDATE

    I hereby solemnly declare that:

    (a) The information given in this admission Form is correct to the best of my knowledge
        and belief and if any thing is found incorrect, the University will have the right to
        cancel my admission and expel me from the University.

    (b) I promise to:-

            (l)       Abide by the statutes, Regulations and Rules etc. framed by the
                      University. from time to time;

            (ii)      Maintain good behavior;

            (iii)     Work diligently and maintain the dignity and prestige of the Univ. both on
                      and off the Campus

     (c)      I undertake to be a full-time and regular student of the Univ. and shall not join
              any other programme or accept any employment for the duration of the
              programme registered. (The in-service teachers/persons are required to produce
              NOC/Study Leave duly sanctioned by the employers).


     (d)      I further undertake that I will not claim hostel accommodation as a matter of right
             if admitted in the University.

         (e)      I promise to pay all the dues regularly.

   (f)   I hereby declare as binding on me, that as long as I am a student, I accept all the statutes,
         Regulations, and Rules in force at the time of joining the University. and framed
         subsequently. I shall submit to the discipline of the Univ. authorities as exercised through
         its various committee and officers.

    (g) I accept as a condition of my Admission the authority of the Univ. to the effect that if in
        the opinion of the Vice Chancellor my stay is not conducive to the normal academic and
        community life on the campus, I will not hesitate to withdraw my name form the
        University after being called upon me to do so., and my admission will be treated as
        cancelled.
