           New Forms Revised Again
           In January 2007, Canada Life updated their Title Change Form (584 CAN) and Dividend Option
           & Premium Offset Form (740 CAN). Based on your feedback, additional changes to these forms
           have been made effective February 2007. Click on the above link to view the announcement
           and a list of changes made.

           Please discard all versions of these forms that are dated prior to 2-07. Canada Life will
           continue to accept old versions until May 1, 2007. Copies of the new forms may be obtained
           from VirtGate, Supplies and Forms section.

 New Life and Critical Illness Application
 The new Life and Critical Illness Application 07002E March 2007, can be used in all provinces
 and is now available for ordering from your Resource Centre or MSPS Team. You may also
 download a copy of the new application from VirtGate, Supplies and Forms section.

 Older copies of this application, 99235E, will continue to be accepted until September 4, 2007.

 February 2007 Fusion Illustration Software & Patch Download Now Available
 Desjardins new February 2007 Fusion software CD is now available for ordering from your
 Resource Centre or MSPS Team. Once you have downloaded the February 2007 CD visit Dundee
 Mast – Insurance – Software to download the March 2007 Patch download to update your Fusion
 software with the latest changes.

 Revised Retirement Annuity Application & Contract
 The Retirement Annuity Application Form 1433A (06-12) was revised to incorporate CLHIA
 guidelines as of January 1, 2007. The application is now available for ordering from your
 Resource Centre or MSPS Team. Note, the old form will not be accepted after March 30, 2007.

 What Does Proper Policy Delivery Mean?
 This Desjardins notice explains why it is of paramount importance that policies be delivered at
 the earliest possible date upon receipt from the New Business Department.

 The Fight Against Cancer Is Not Over!
 One woman in three and two men out of five will suffer from cancer during their lifetime. It is
 estimated that the number of cases of cancer will increase by 70% by the year 2015.

 The new Interface Suite, 5.2.0 illustration software CD is now available for ordering from your

 Also, please note that illustration applications for products no longer available for sale (such as
 Meridia, Maxirance, Genesis 1.0, Axis 1.0, etc) will no longer be available in Interface Suite.

 Updated Home Protection Plan
 Pick-A-Term credit insurance is offered as a complement to Home Protection Plan mortgage
 insurance. The two types of insurance contain several notable differences that are worthy of
 special attention.

 The Home Protection Plan mortgage insurance only covers mortgage loans, whereas Pick-A-
 Term credit insurance covers a multitude of loans or lines of credit other than mortgage loans

           New Illustration Software Update
           Unity Life’s new February 2007 illustration Patch update is now available for downloading from
           Unity’s website or from Mast – Insurance – Software section.

           Medications & Liver Function Test Results
           Blood Profile is one of the more common age and amount requirements submitted to support
           an application for insurance. A routine part of the Blood Profile is a Liver Function Test. The
           results of this Test provide a common reason for an application for life insurance to be
           declined or postponed, i.e. elevated Liver Function Test result(s) with the applicant having no
           apparent medical history to explain why the results would be abnormal.
