Welcome to Division 1 All-Stars Basketball. Now that you have been selected to be a
member of one of our teams, you will need to do each of the following:
1. Submit your personal information. Go to the On-Line Registration link on our
website and select Player Registration.
2. You need to submit the following required documents:
a. Parent Agreement Form – Go to the Participation Forms link and the
select parent aggrement to print, read and sign document.
b. Birth Certificate
c. Current AAU Card - If you need to purchase an AAU membership, please
go to the Purchase / Renew AAU Membership link on our website.
d. Report Card – submit most recent
e. Proof of Physical - Your child is required to have been cleared for
extensive physical activity (running, exercise, etc.) within the past 12
months. The clearance must be renewed annually. Please submit a
doctor signed document that shows proof of a physical.
Please email all five (5) documents in pdf format to Dena Falls @
dfalls123@att.net.
3. Submit Quarterly Payment. Fees to participate are as follows:
Elite (travel) Team - $500 per quarter
Maroon (non-travel) Team - $400 per quarter
Quarterly Fees are due according to the schedule below:
Session Start Date Fees Due
Fall October 1 September 15th
Winter January 1 December 15th
Spring April 1 March 15th
Summer July 1 June 15th
If the player joins our organization mid-session, the fees will be prorated
accordingly.
4. Purchase Uniforms. Games and practice uniforms are mandatory and must be
purchased on-line from our website. Team shoe is optional. Go to the
purchase team gear link to order and pay for uniforms.
