In addition to the facts of the case and any specific language set forth by the Judge, the following items should be included
in the Order Appointing Conservator:

    •   The amount of the Surety Bond and approval to pay the bond premium (submit bond with Order.)

    •   That the Conservator has reviewed his/her statutory duties as set forth in S.C. Probate Code §§62-5-424 and 62-5-
        425, and has been given a copy of same by his/her attorney.

    •   That the Conservator shall sign the Conservator’s Oath and file it with the Court within 30 days of the
        appointment.

    •   That the Conservator shall provide the Court with an inventory of the Protected Person’s assets within thirty days
        of the date of this Order.

    •   That all funds shall be placed in a federally insured, restricted account with the designation
        “_____________________, Conservator for _________________ (Protected Person)” within 30 days of the date
        of the Order.

    •   That the Conservator shall provide the Court with an Annual Accounting and a Petition for Approval of the
        Annual Budget; to be reviewed by the Judge, and approved on an annual basis (budget not required for minors).

    •   That the Conservator shall not make disbursements, transfers, or any other transactions of the Protected Person’s
        assets without prior written approval from the Probate Judge.

    •   The amount of Attorney’s fees to be paid from conservatorship assets (fee affidavit must be attached).

    •   A certified true copy of the Circuit Court Order approving the settlement shall be submitted to this Court within
        30 days (if approved by Circuit Court).
