Entrance into the Diablo Valley College dental hygiene program is highly competitive, with enrollment limited to 20 students.

                                            PRIMARY ADMISSION CRITERIA
PREREQUISITE COURSES must be:
   • College-level courses (not high school courses), with grades of “C” or better
   • Complete by December 2010—in-progress courses will not be considered
 Prerequisites include:
             ENGL-122 Freshman English: Composition and Reading [CAN ENGL2]; or an equivalent course*; or
             an AP English exam score of 3, 4, or 5 [provide official transcript from the College Board]
 Also, the following science curriculum prerequisites, or equivalent courses*
   • which include hands-on laboratory experience
   • must be completed within the last five years (2006 or later);
   • with a grade of “C” or better in each course; and
   • achieve a minimum 2.75 GPA overall in the following group of science courses

   *Upper Division Courses: may be substituted for lower division classes to satisfy a prerequisite.

APPLICATIONS
  • Apply to DVC online at www.dvc.edu [not necessary if you are currently enrolled at DVC, Contra Costa, or Los
      Medanos Colleges].
   • Submit a complete Dental Hygiene application packet postmarked between 1/1/11– 2/15/11. (You may purchase a
      Certificate of Mailing from the US Post Office if desired.) Applications can also be hand-delivered by 2/15/11.

            not apply to this program. EXCEPTIONS: transcripts from Diablo Valley College, Los Medanos College, and
            Contra Costa College are not required.

    •   Letters of recommendation are not required and will not be considered in the evaluation process.
    •   Applications are not carried over from one year to the next. If you have applied previously, to be considered for the
        Fall 2011 dental hygiene program, you must submit a new, complete application packet. Application requirements

TIMELINE and REQUIREMENTS
   • All applicants will be notified by email of tentative acceptance, or non-acceptance, into the dental hygiene program at
       the end of April or early May 2011.
   • When requested by the dental hygiene program, applicants who received a letter of tentative acceptance will be
       required to submit the results of a recent physical examination that provides evidence of good health, tuberculosis
       clearance, and immunizations.
   • Students accepted into the Program will be required to:
           • Enroll in and attend DENHY-101 Dental Hygiene Orientation in summer 2011. Students attending the
               Orientation course will be required to pay a $62.00 fee for materials and the required textbook.
           • Attend a counseling session with the dental hygiene counselor at the start of the Fall 2011 term.
           • If you do not attend these meetings, your position in the class will be awarded to an alternate.

ADDITIONAL INFORMATION
   • No guarantee can be made that an applicant will be accepted to the dental hygiene program.
   • Making a false statement on the application is cause for entrance denial or dismissal from the program.
   • This is a rigorous program. If you have not already completed an AA/AS degree or higher, it is strongly
      recommended that you complete all of the general education course requirements prior to enrollment. ( Sociology
      120, Speech 121 and Psychology 101 or 122; or equivalent courses, are required by CODA) For course information:

      Profession.
   • Graduation from a dental assisting program is helpful in acquiring skills for the Program.
   • If you are not admitted into the dental hygiene program, DVC offers other programs that may be of interest, e.g.,
      dental assisting, dental technology, sports medicine, and respiratory therapy.

COMMUNICABLE DISEASES
  • The Colleges of the Contra Costa Community College District shall cooperate with local health officers in measures
     necessary for the prevention and control of communicable diseases in students and comply with any immunization
     programs required by State Department of Health Services regulations.
  • It is the intent of the Contra Costa Community College District to provide students with a safe learning
     environment. Students who may “reasonably anticipate” coming in contact with blood and other potentially
     infectious materials through their studies will be provided with training materials, personal protective devices,
     medical treatment and any necessary follow-up. Additional information can be found at:

FELONY CONVICTION AND BACKGROUND CHECKS
   • Dental hygiene applicants who have committed a felony may not be eligible for licensure.
   • Applicants who have committed a felony should contact the California State Board of Dental Examiners to determine
     their eligibility to take the National Board and State Board examinations.
   • Making a false statement on an application regarding a past conviction is cause for denial of licensure, regardless of
     the nature of the crime or how long ago it occurred.
   • Fingerprinting is required and checked through the Department of Justice and/or the Federal Bureau of Investigation
     prior to state licensure.
   • Certified background checks and drug testing are required each year of the program. Information will be
     disseminated during DENHY 101 Dental Hygiene Orientation.

ADDITIONAL REQUIREMENTS
     •    Students enrolled in the program must achieve a grade of “C” (75%) or better in each of the Dental Hygiene courses in order to
          continue in the program.
     •    An Associate or higher degree from an accredited institution must be completed prior to, or concurrent with, the Dental Hygiene
          requirements. Eligibility for the California licensure examination is contingent upon achievement of an Associate degree or higher
          and a Certificate of Achievement in Dental Hygiene from an accredited dental hygiene program. Students who earn an Associate
          in Science degree in Dental Hygiene at DVC are not required to also complete a Certificate of Achievement in Dental Hygiene.
     •    An Associate in Science (AS) degree in Dental Hygiene or a Certificate of Achievement in Dental Hygiene is awarded following
          completion of the program curriculum and achievement of Dental Hygiene competencies. For information on Certificate or

     •    Students are required by the State Board of Dental Examiners to complete a course which certifies them for 2 years in Basic Life
          Support for Health Care Providers from either the American Heart Association or the American Red Cross. (The course must
          include CPR for infants and children, Automatic External Defibrillator, and first aid.) A copy of current certification along with a
          negative TB test must be on file in the Dental Hygiene office prior to the first term of the first year or you will not be accepted
          to start the program.

The Diablo Valley College dental hygiene program asks our dental hygiene applicants to observe the following dental-
related procedures in Part 1 and Part 2 in order to gain an understanding of dental and dental hygiene practices and to be
better informed regarding their prospective career path. We appreciate your time and efforts in allowing students to
observe you in your workplace.

Each observation must equal a minimum of three (3) hours. Students must observe a Registered Dental Hygienist (RDH)
performing the procedures listed below. Or, in Part 2, students may observe a Periodontist in lieu of the RDH.

Please sign in the spaces indicated below. Also, attach to Parts 1 and 2 of this form the business cards of the Dentist in
whose office the RDH practices. If, in Part 2, a Periodontist is observed, then attach the business card of the Periodontist.
