Attention: Complaint Department

Dear Sir or Madam:

The following accounts do not belong on my credit report. I have no
recollection of ever having credit obligations with any of these creditors and I
believe they, are listed on my report in error. Upon investigation I'm sure you will
find that none of these companies can verify these claims. Please delete these
entries immediately pursuant to [15 U.S.C. § 1681i] of the Fair Credit Reporting
Act.

*Furthermore, since I am sure that I have no liabilities with these companies, I
am requesting a description of the investigation process your agency performs. If
I receive my new reports and any of these companies remain I demand to have
the name, number, address etc. of the persons contacted so that I may pursue
my rights under federal law to ensure that this information is deleted accordingly.
Thank you.
Since 30 days from the receipt of this letter is your allotted time under the law to
reverify these entries, it should be understood that failure to do so within that 30
day period constitutes reason to promptly delete the information from my file [15
U.S.C. § 1681i] section 5.

Also, I demand that you notify me when these items are deleted/updated by
sending me a copy of my updated credit report to the below address. There shall
be no charge for this report pursuant to USC 15 section 1681j of the Fair Credit
Reporting Act. Your immediate compliance is greatly appreciated. Thank you.

Sincerely,
