Thank you for your interest in a redistribution program with Dot Foods, Inc. Redistribution is a supply
chain strategy that manufacturers use to outsource their existing Less Than Truckload (LTL) distribution
and a sales strategy that they use to expand their sales and marketing reach.. In order to determine if
there is an opportunity for a partnership between our organizations, we need to understand some basic
information relative to your business. The information that we are for on this Potential Supplier
Questionnaire is the minimum required to gain that understanding and will also be necessary to develop
the program should we both determine we want to move forward. With that in mind, please fill out the
