					                                          2010-2011 ECEAP Self-Assessment – Due June 15
         The ECEAP Self-Assessment is part of your on-going process of continuous improvement. It is your annual review of compliance with
         ECEAP requirements, including parts of the ECEAP Contract and all sections of the ECEAP Performance Standards.
         Directions:
              1. Plan your self-assessment process, starting as early in the year as possible so that it is meaningful and useful for you.
              2. Engage parents and staff, including subcontractors if you have them.
              3. For each item on this form:
                       a. Work side-by-side with a copy of the current ECEAP Contract and Performance Standards side as you assess. Read the
                          entire text of each contract provision or standard before deciding your response.
                       b. Fully respond to any questions in the Description column.
                       c. If the Description column is blank, write what your program did this year to meet the standard. Give specific examples.
                          Explain why the item is fully met or not.
                       d. Rate each item as “Fully Met” or “Action Required” for this year. Action is required if any part of the standard is unmet
                          at any of your locations.
                       e. Note that some standards are grayed-out on this form, if DEL gathers the information elsewhere including your 2011
                          Funding Renewal Application due March 2011.
              4. Answer the three questions at the end of each section of the standards, even if all items in that section are fully met. This is
                 your opportunity to write:
                       a. Changes you plan based on lessons learned from this process.
                       b. Your plans for ongoing monitoring of that section of standards.
                       c. Technical assistance requests.
              5. Compile your assessment information from all sites and subcontractors onto one copy of this form.
              6. Email your self-assessment to eceap@del.wa.gov by June 15th.
