We have limited rooms that can be reserved at The Legacy Hotel, Marsh Lane, Preston at £56 B&B Per night.

payment take using the method indicated below. Once these have been allocated we can advise you of other
accommodation in the area that you can book direct yourself, but these rates may be higher.

Important Notes about Completing this Form
Section 1
    1. Please ensure that if completing by hand you use BLOCK letters.
    2. Your name and organisation/place of study as stated on this form will be used for your badge and the delegate list.
    3. Please provide your full postal address including the name of your organisation/workplace.

    1. Please ensure you tick the appropriate fee box for the registration category you are applying for.

      1. Please ensure that you complete this section for any dietary, access, hearing or mobility requirements you may have.
Special Diets
All catering is organised with options to fulfil the requirements of vegetarians. However, we can accommodate most dietary requirements
if these are indicated to us on booking.
Adapted Facilities
All buildings have wheelchair access and various other special facilities. We are happy to clarify these prior to booking on request.
Section 4
    1. Please note that we cannot accept payments by Diners Club and American Express.
    2. If you require an invoice please complete the details in full and always include a purchase order number. We can not issue
          an invoice without this.
Payment Terms
Returning this form constitutes a firm booking. On completion of the booking form, payment for the conference may be made by cheque
(made payable to UCLan Business Services Ltd) or credit card. Receipts will be issued within twenty eight days after the conference. On
production of a purchase order by the delegate’s organisation, an invoice may also be raised.
Violence in Public Places and Institutions Conference – IMPORTANT INFORMATION
Standard Conference Fee
The cost of the standard conference fee includes refreshments and lunch on 26th and 27th June, conference drinks reception 25th June
with full conference documentation. Please note Conference Dinner is not included and must be booked separately.
Accommodation is not included in the standard conference fee and must be booked by the delegate separately. We have a
limited number of rooms reserved at the Legacy Hotel, Marsh Lane, Preston that can be booked direct with ourselves. Once these rooms
have been allocated further details of accommodation that can be booked by yourself, can be supplied by the Conference Office, please
e-mail eakelly1@uclan.ac.uk
The cost of the standard conference daily rate includes refreshments, lunch and full conference documentation but does not include
accommodation, breakfast or the conference dinner/reception.
Student rates are available for students in full time education and there are a limited number of discounted places for DFP Members who
are attending the FULL DFP Conference on the 23rd – 25th June 2009.
 (A request must be sent in along details of University/Course and BPS numbers respectively to qualify for one of the reduced places and
a booking must have been made for the FULL DFP Conference on the 23rd – 25th June 2009)

TERMS AND CONDITIONS
1. All forms must be received by 8th June 2009. Bookings will be accepted and confirmed only when full payment or invoicing instructions
have been received. Bookings will not be accepted unless they are made using this official form. Payment sent independently of this
booking form must state details of the delegate and the conference. If you do not receive a confirmation e-mail of your booking within two
weeks of the conference, please contact the Conference Office.
2. Cancellation Policy – Any cancellations received up to and including 23rd May 2009 will be given a 50% refund. Any cancellations
received after this date, a refund will not be given. Cancellations must be made in writing to eakelly1@uclan.ac.uk Verbal
cancellations will not be accepted.
3. Substitutions Policy – Name substitutions are accepted up to 8th June 2009 and should be made in writing to eakelly1@uclan.ac.uk
Verbal substitutions will not be accepted. Packages cannot be shared under any circumstances.
4. All fees include VAT at 15%. Our VAT registration number is 677 3793 76.
5. The information you provide on the booking form will be held on a database to process the confirmation of your place and so that we
can keep you up to date with other relevant products and services provided by the University of Central Lancashire. We do not pass your
details on to any companies outside the University group for marketing purposes. Please write to Conference and Events Management if
you do not wish to receive any mailings.
6. Booking Acknowledgment and Information - Please note that your booking acknowledgement and information will be sent to you via
email approximately two weeks before the start of the conference.
7. Quality Assured - At the end of the conference, you will be asked to complete an evaluation form. To ensure we maintain a high quality
of service, we welcome your comments and suggestions and would ask that you take the time to complete these details and return them
to us.
