               Please register by noon of February 13, 2004 to qualify for the $10 discount.
         Meeting fee:   $35.00 Vibration Institute Members
                        $40.00 Non-Members
               A $10.00 discount - on meeting fee only - is available for those who pre-register by February 13 .

         Vendor fee:     $50.00 Vendor Charge per Table
                         (Vendor Tables must be pre-paid and fee does not include individuals meeting fee)
                         Remember to reserve early, as tables may be limited!
Each person is responsible for his/her own meeting fee which is payable by Registration on Friday, February 27 .
Receipts will be given at that time. Prepayments must arrive by above Registration deadline date. Be sure to attach a list
of attendee’s names. List your name(s), Company, mailing address, and telephone number. Make checks payable to
Piedmont Chapter #14 – Vibration Institute and mail to Tom Dixon – P.O. Box 152, Lavonia, GA 30553. Refunds are
available only if cancellations are received before Registration deadline. There are no refunds for participants who pre-pay
and fail to attend.

                  Vendor exhibits will be set up during breaks and lunch in Salon B.

SOMETHING TO CONSIDER - Do you know that attendance at a Chapter meeting will earn you Professional
Development Hours (PDH)? Attendance is one PDH per hour of technical session. A presentation is 2 PDH's per
presentation hour. Also serving as an officer (board member) earns 2 PDH's per year.

               We need participation and input from everyone involved to keep this chapter strong!

                                                 Presentations Needed!
Members and vendors are needed to present case histories, new technologies, analysis techniques, and other related
topics during chapter meetings. All should focus on improving maintenance practices or on enhancing maintenance
related programs. All presentations must be void of product promotion. If you or someone you know could make a
presentation at one of our future meetings, contact one of the Program Chairmen today! Also, please let us know of any
ideas or suggestions for presentations.
