This Trading Party Record is between Pantellos Group Limited Partnership, a Delaware limited partnership
(Pantellos), and the Trading Party named below. Execution by both parties indicates agreement to the Uniform
General Agreement posted in the Pantellos Website.

2. Affiliates
      2.1. Attachment 1. The Affiliates of the Trading Party that may participate directly in using services
           subscribed to by the Trading Party or named Affiliate are as set forth on Attachment 1.
      2.2. Update. The list of Affiliates may be updated by an Authorized Representative at any time by Notice and
           the submission of a replacement Attachment 1.
3. Authorized Representatives
      3.1. Attachment 2. The Authorized Representative(s) of the Trading Party and Affiliates, including names,
           employer and email address, are as set forth on Attachment 2.
      3.2. Update. The Authorized Representatives of the Trading Party and their respective email addresses may
           be updated by an existing Authorized Representative at any time by Notice and the submission of a
           replacement Attachment 2.
4. Notices
      4.1. If to Trading Party. The Trading Party will designate one individual as its primary administrator of the
           relationship with Pantellos on Attachment 2 to whom notices should be addressed at the address above.
           The Trading Party may also designate additional Authorized Representatives to receive email Notice of
           proposed Amendments.
      4.2. If to Pantellos. Notices to Pantellos should be addressed to:
