This Instructions Tab explains how a Respondent should respond to the qualitative and quantitative requirements
of this RFP using this Excel workbook and the other Attachments.

Response Requirements
   Respondents must be able to provide new qualifying machines for a minimum of FOUR (4) Black-and-White
   Copier equipment categories or TWO (2) Color Copier equipment categories, for consideration for award of
   the portion of the RFP pertaining to those types of equipment respectively.

Format Requirements

    Excel Workbook Information:
    This Excel workbook will be the only format you may use for your proposal submission.

    Non-Excel Workbook Information:
    Your response may also include non-Excel workbook information as part of your proposal, but only in
    addition to all required information outlined in the Excel workbook. If Respondents choose to include non-
    Excel workbook information, please attach it electronically with the Excel workbook submission.

    Response Cells Highlighted in Yellow:
    As you fill out each tab, fields shaded in yellow highlight the only cells each Respondent should be
    completing in their response.

    Do Not Change Format of Any Tab:
    Respondents should not try to change either the formatting or structure of this Excel workbook including
    pricing proposal grids, as this could result in an incorrect analysis of the proposal and disqualification of the
    Respondent.

    Entering Numerical Information:
    Respondents should be precise in the use of number terms and use them according to the following
    definitions - Enter “0” if there is no charge for an item. Leave cells blank if something is not applicable to the
    proposal. Do NOT enter text into cells that request numerical responses. Doing so could result in an
    incorrect analysis of the proposal and disqualification of the Respondent.
    Number formats: Any cell in this Excel workbook requesting a numerical response has been pre-formatted to
    show information in dollars, rounded to the nearest cent, percentages, or other relevant numerical format.
    Enter numbers on each grid as a “number” (e.g. four-place decimal), and not a “currency” or other format
    unless otherwise stated. That is, omit dollar signs, commas, and any other non-essential symbols. For
    example, $7.9000 should be entered as 7.90, and 10% should be entered as 10. All decimal places will be
    captured by the response grid. However, the cell will only show the number entered out to four (4) decimal
    places.

 This section must include a statement indicating that the CEO and/or CFO has taken personal responsibility for
the thoroughness and correctness of any/all financial information supplied with this proposal. The particular areas

assures board integrity, the separation of audit functions, and consulting services. The State will consider the

A sample contract that the state expects to execute with the successful Respondent(s) is provided in Attachment

Selected out-of-state Respondents providing the products and/or services required by this RFP must be registered
to do business within the State by the Indiana Secretary of State and the Indiana Department of Administration,

 The Respondent is responsible for the performance of any obligations that may result from this RFP, and shall not
be relieved by the non-performance of any subcontractor. Any Respondent’s proposal must identify all
subcontractors and describe the contractual relationship between the Respondent and each subcontractor. Either
a copy of the executed subcontract or a letter of agreement over the official signature of the firms involved must
accompany each proposal.



Any subcontracts entered into by the Respondent must be in compliance with all State statutes, and will be subject
to the provisions thereof. For each portion of the proposed products and services to be provided by a
subcontractor, the technical proposal must include the identification of the functions to be provided by the
subcontractor and the subcontractor’s related qualifications and experience. The combined qualifications and
experience of the Respondent and any or all subcontractors will be considered in the State’s evaluation. The
Respondent must furnish information to the State as to the amount of the subcontract, the qualifications of the
subcontractor for guaranteeing performance, and any other data that may be required by the State. All
subcontracts held by the Respondent must be made available upon request for inspection and examination by
appropriate State officials, and such relationships must meet with the approval of the State.

The Respondent must list any subcontractor’s name, address, and the state in which formed that are proposed to
be used in providing the required products and/or services. The subcontractor’s responsibilities under the proposal,
anticipated dollar amount for subcontract, form of organization, and an indication from the subcontractor of a
willingness to carry out these responsibilities are to be included for each subcontractor. This assurance in no way
relieves the Respondent of any responsibilities in responding to this RFP or in completing the commitments
documented in the proposal. The Respondent must indicate which, if any, subcontractors qualify as a Minority

    The State requires that you be able to meet the requirements listed below, at a minimum, as a prerequisite for responding to this RFP. Please read the requirements below and indicate your

    The State of Indiana requires that each Respondent must be able to meet award criteria, and certify that all devices that are offered in this RFP are newly-designed digital imaging systems,
    not redesigned analog machines. No discontinued, refurbished, rebuilt or remanufactured equipment will be accepted, unless otherwise granted by the State. Respondents must be able to
    support state agencies, institutions of higher education, qualified political subdivisions and non-profit corporations throughout the State of Indiana for the duration of the contract.

2 The State of Indiana requires that each Respondent be licensed to do business in the State of Indiana, as required by State Law.
3 The State of Indiana requires that each Respondent must be a manufacturer or designated/certified manufacturer's representative capable of providing statewide sales and service through
  dealer and/or manufacturer representation.
4 The State of Indiana requires that machines offered by each Respondent must be in production and available for sale at the time of Proposal opening.

  The State of Indiana requires that all equipment offered by each Respondent must have published capabilities. Responsive Vendors must submit a copy of the published page e.g. Buyers
  Lab Inc (BLI) or other industry standards for each machine proposed. For equipment not yet published in BLI, or other industry standards, responsive Vendors will provide published
  descriptive literature, which includes capability in documents (copies/prints) per minute, and stipulate the list price to be published (in BLI or other industry standards).

  certification letter dated and signed by the manufacturer that Respondent is currently authorized distributor and/or service representative, authorized to submit proposals for the
  manufacturer's products. Authorization is to include a statement providing assurance that, should the manufacturer withdraw the authorization, a replacement representative will be provided
  within a time frame acceptable to the State, and that representative will continue all agreements and fulfill all contract requirements in place. Failure to comply with this requirement will
  result in proposal rejection.

Per section 1.3 in this RFP, the State has entered into an Equipment Maintenance Management
Program. Although not mandatory, it is highly preferred that Respondents adhere to this program.
By completing this technical proposal that is formatted for those Respondents that are willing to
comply with the State's EMMP, you are committing to utilize the State's EMMP if awarded this
contract. Please confirm this by stating yes in the yellow shaded box to the right, and complete

2.4. ePROCUREMENT

2.4.2 SERVICE FACTORS

The winning Respondent is required to respond to service calls within a guaranteed time set forth in the contract.
Availability of representatives to service all equipment is a requirement of this RFP. Respondent must respond on-site
within four (4) hours in urban areas or eight (8) hours in rural or remote areas to all service calls placed by state
agencies. Working hours are defined as 7:00 a.m. – 5:00 P.M. Eastern Time, Monday through Friday. The winning
Respondent must be willing to provide Time and Material service and respond within four (4) hours in urban areas or
eight (8) hours in rural or remote areas to all service calls placed by State agencies. The winning Respondent must
also be willing to invoice Third Party with a field service report. .Please indicate your ability to comply with this
requirement.

2.4.2.2 DELIVERY, INSTALLATION AND REMOVAL                                                                                         Yes / No          If No, Proposal / Explanation


Winning Respondents shall be expected to deliver, set up and install machines within three (3) calendar weeks after
receipt of a Purchase Order. Failure to provide deliveries in a timely manner may result in cancellation of the order, at
the User’s sole discretion. The Vendor is required to fully install all hardware and software for all networked digital
machines at no additional cost. Machines must be installed within 24 hours of delivery, unless otherwise agreed to by
the Vendor and the User. Vendors will also be required to remove all leased machines no later than two (2) calendar
weeks after the lease agreement is completed. Emergency/rush delivery requiring special shipping and handling will
be at User's expense (with prior written approval only). Rush Delivery is defined as a situation that would require
urgent intervention to prevent a worsening of the situation. Please state your company's ability to comply with this
requirement.

Any equipment delivered in poor condition, items delivered in excess of the amount authorized by the requisition form,
or items not included on the requisition form or purchase order may, at the discretion of the Purchasers, be returned to
the Vendor’s warehouse at the Vendor’s expense within 30 days. Credit for returned goods shall be made
immediately after Vendor receives returned goods.

If any product is returned to a Vendor for Failure to Perform, the Vendor will, at the State’s discretion, refund all
amounts paid to the Vendor for such product or replace the product, and the following shall apply:

• Within twenty (20) days of written notification by the Purchaser, the Vendor will make arrangements for the return of
the product.
• The Vendor will bear all shipping and insurance costs.
• Vendor will be liable for damages to the product, unless caused by fault or negligence of the Purchaser that occur

2.4.4 EQUIPMENT

When the Purchaser determines that additional features are required or the volume of a leased machine exceeds the
manufacturer’s suggested maximum volume for that machine, for a minimum of two consecutive months, the leased
machine may be returned to your company under the following conditions:

• If machine performance is unacceptable or if the machine is not capable of handling the higher volume, a machine
from a higher volume level may then be leased. The State is the sole judge of what is unacceptable performance.

Agreement for the new machine would start as a new Lease Agreement from the time the new machine is ordered
and the previous lease would be null and void. Purchasers shall not upgrade more than 10% of each Vendor’s

• Unless otherwise specified, full parts and labor warranty period shall be for a minimum period of ninety (90) days,
commencing on the equipment acceptance date
• Vendor warrants that the Equipment shall be in good operating condition and shall conform to the Specifications for

• During the warranty period, Vendor shall adjust, repair, or replace all equipment that is defective or not performing in
conformance with the specifications. All costs for such adjustments, repairs, or replacements, including all costs for

expense. Any defective equipment shall be repaired or replaced for Purchaser so that it conforms to the specifications.
• Vendor agrees that all service provided hereunder shall be performed by manufacturer-trained, certified, and
authorized technicians. Vendor further agrees to act as the sole point of contact for service. Vendor shall provide help
desk services for reporting equipment issues and for troubleshooting problems. Vendor's
help desk services shall be accessible via e-mail or via one or more toll-free telephone lines.
• Vendor shall provide escalation procedures to ensure that the proper level of attention and resources are
directed towards resolution of equipment and service problems in a timely manner. The escalation
procedures shall indicate the steps to be taken in response to a problem report, the contact information and
title of your company's employee(s) responding at each level, and the elapsed time before the next level of
response is invoked. In addition, Vendor shall provide a copy of its disaster recovery plan for managing
crisis situations in order to help avoid or reduce the potential adverse affects of catastrophic events (e.g.,
floods, fires, earthquakes, fuel/gas leaks rendering a building unusable, etc.)
• Purchaser agrees that your company will not be liable for any damages caused by the Purchaser’s actions
or failure of Purchaser to fulfill any of its responsibilities for site installation.

In the event of conflict between contract terms and conditions and warranty submitted, to afford the State

copier and then connected to the network at a later date. Any upgrades that increase the functionality of a machine

2.4.4.12 ELECTRICAL CONNECTIONS, SURGE PROTECTORS AND TELEPHONE LINES                                                            Explain

network and electronic components; this is in addition to any internal surge protector. Surge protectors must come
with a minimum five year manufacturer’s warranty stating the surge protector will be replaced free of charge if it fails

surge protector.

2.4.3.4 EQUIPMENT RELOCATION/TRANSFER                                                                                        Explain



The State of Indiana shall be allowed to move or transfer equipment from one agency location to another (within the
same agency or transfer to another agency) as deemed necessary with no change to lease or maintenance plans.
Will you be able to provide all necessary support for such relocation/transfer both within the same office/facility and
between facilities? No installation or removal charges will be billed at either location.

Will you be able to provide training to the Purchaser’s staff in the operation of all equipment and systems? An initial
training session shall be conducted within three (3) business days of equipment installation. Subsequent training

conducted at Purchaser’s request. There shall not be any charge to the State for conducting training sessions.

You must also provide a copy of general specification sheets or literature and one operating manual upon delivery of

The State of Indiana Office of Technology is implementing a server based solution to manage and monitor printing.
This system will track printing by users and departments, and will be device-independent. In addition, the State would
like to manage print devices with a system that provides a dash board for Help Desk tasks that has broad support for
mixed fleet environments and minimizes manual processing of meter reads. The Winning Respondent will be
expected to comply with this initiative and ensure all equipment is compatible with Equitrac Software. Please explain if

2.4.4 EQUIPMENT

Any new equipment, supplies and/or services accepted by the state may be added to this contract and/or substituted
for discontinued models, items and services. Evidence of discontinuation must be provided from the manufacturer,
acceptance of which shall be at the discretion of the State. New items shall meet or exceed all the specifications of
the original award. Replacement items will only be accepted if provided at the same pricing as, or better pricing than,
the replaced model. If the list pricing for the new equipment is lower than the list price for the old equipment, it is the
State’s expectation that at least the same level of discount off of the list price will be applied to the new equipment to
determine the final contract price. Requests must originate from your designated contact, and be presented on

As new products and capabilities develop, additions to the contract may be made in Categories other than originally
sought. Such additions will be considered for equipment introduced three months or more after award. These
additions must meet all contract requirements, and will be considered if presented with a
pricing structure equal to other equipment in place in the requested Category, or similar to that originally bid
and awarded elsewhere on the contract. Thirty days prior notification to IDOA is required before adding or
removing any equipment from the contract. Please confirm your willingness to comply with this requirement.


2.4.4.2 LEASED MACHINE UPGRADES                                                                                                 Yes / No         If No, Proposal / Explanation


When the Purchaser determines that additional features are required or the volume of a leased machine exceeds the
manufacturer’s suggested maximum volume for that machine, for a minimum of two consecutive months, the leased
machine may be returned to your company under the following conditions:

• If machine performance is unacceptable or if the machine is not capable of handling the higher volume, a machine
from a higher volume level may then be leased. The State is the sole judge of what is unacceptable performance.
                                                                                                             The Lease
Agreement for the new machine would start as a new Lease Agreement from the time the new machine is ordered
and the previous lease would be null and void. Purchasers shall not upgrade more than 10% of each Vendor’s
installed base for that agency (no less than one machine) within a given facility, in a given 12-month period. Data on
available new upgrades should be communicated with the State's Vendor Manager on a quarterly basis. Please
confirm your company's ability to meet this requirement.

When the volume of a leased machine is more than 25% below the minimum volume commitment for that Equipment
Category for two consecutive months:

• The leased machine may be returned to your company (the Lease Contract for the new machine would start as a
new Lease Contract from the time the new machine is ordered)

Purchasers shall not downgrade more than 10% of each Vendor’s installed base for that agency (no less than one

The Vendor will agree that there will be no cancellation of products used, without an equal and acceptable
replacement approved by the Vendor Manager during the term of the agreement. Vendors will communicate
manufacturer’s discontinuation of any products to the Contract Manager in writing within five (5) business days. In
such instances, Vendors will work with the Contract Manager to identify and implement alternative options that will
maintain or reduce costs associated with the replacements. Vendors will be prepared to offer detailed quarterly
reports if requested by the State, of suggested replacements. Vendors will offer suggested replacements of
discontinued products at least 30 days prior to substitution, including replacement part number, description, and final

The Vendor shall be required to furnish all materials, equipment and/or services necessary to perform contractual
requirements. Materials and workmanship in the construction of equipment for this contract shall conform to all
codes, regulations and requirements for such equipment, specifications contained herein, and the normal uses for
which intended. Materials shall be manufactured in accordance with the best commercial practices and standards for
this type of equipment.

2.4.9.6 WARRANTY FOR LEASED AND PURCHASED MACHINES                                                                              Yes / No         If No, Proposal / Explanation



• Unless otherwise specified, full parts and labor warranty period shall be for a minimum period of ninety (90) days,
commencing on the equipment acceptance date
• Vendor warrants that the Equipment shall be in good operating condition and shall conform to the Specifications for
the entire period of the warranty, commencing upon the first day after the acceptance date.
• During the warranty period, Vendor shall adjust, repair, or replace all equipment that is defective or not performing in
conformance with the specifications. All costs for such adjustments, repairs, or replacements, including all costs for
replacing parts or units and their installation and any transportation and delivery fees, shall be at your company's
expense. Any defective equipment shall be repaired or replaced for Purchaser so that it conforms to the specifications.
• Vendor agrees that all service provided hereunder shall be performed by manufacturer-trained, certified, and
authorized technicians. Vendor further agrees to act as the sole point of contact for service. Vendor shall provide help
desk services for reporting equipment issues and for troubleshooting problems. Vendor's
help desk services shall be accessible via e-mail or via one or more toll-free telephone lines.
• Vendor shall provide escalation procedures to ensure that the proper level of attention and resources are
directed towards resolution of equipment and service problems in a timely manner. The escalation
procedures shall indicate the steps to be taken in response to a problem report, the contact information and
title of your company's employee(s) responding at each level, and the elapsed time before the next level of
response is invoked. In addition, Vendor shall provide a copy of its disaster recovery plan for managing
crisis situations in order to help avoid or reduce the potential adverse affects of catastrophic events (e.g.,
floods, fires, earthquakes, fuel/gas leaks rendering a building unusable, etc.)
• Purchaser agrees that your company will not be liable for any damages caused by the Purchaser’s actions
or failure of Purchaser to fulfill any of its responsibilities for site installation.

In the event of conflict between contract terms and conditions and warranty submitted, to afford the State
maximum benefits, the contract terms and conditions shall prevail. Please confirm your company's ability to meet this

All leased equipment and purchased equipment under a maintenance contract that develops a trend of requiring an
excessive number of service calls shall be reported by the Purchaser to your company or IDOA for review and
resolution. Your company is to provide service log reports upon request from IDOA and/or Purchasers. Such logs are
to describe service and repairs for specific Purchaser's machine(s) and are to be provided within five working days.

If you feel that excessive service calls are the result of operator or Purchaser problems, your position must be
documented and supplied to the State Procurement Officer administering this contract for review, evaluation and
correction if required. Your company shall provide additional training to Purchasers as necessary to help alleviate

In the case of leased equipment, the Purchaser and your company, with IDOA’s assistance, will resolve the issue on
unacceptable performance whenever possible. In cases where this is not possible, IDOA will be the determining party
on issue resolution.

substituted replacement models as provided for in Section 2.4.4.1. (Definition: “Like-for-like” replacement is a machine
in perfect working condition that offers the same approximate features, functionality and speed at no extra cost.)

2.4.4.10 RECYCLED PAPER CAPABILITY                                                                                               Explain


All machines bid will produce consistent, high quality documents on paper containing virgin and/or recycled content.
The State of Indiana is currently required by executive order to solicit for and purchase, to the greatest extent possible,
paper containing recycled content. Please confirm you company's ability to meet this requirement.

All network-connected machines are required to come fully equipped with all software necessary to successfully
perform all functions the machine was acquired to perform. All future software upgrades for fixes and enhancements
must be furnished to the State and installed by Vendor’s qualified technicians free of charge. Further, all future
software upgrades must be furnished and installed free of charge on any copier that is acquired as a stand alone
copier and then connected to the network at a later date. Any upgrades that increase the functionality of a machine
may be outside the scope of the lease agreement; however the State reserves the right to decline if the upgrade if the
functionality is not required. Please confirm your ability to meet this requirement.

2.4.4.12 ELECTRICAL CONNECTIONS, SURGE PROTECTORS AND TELEPHONE LINES                                                            Explain

All purchased and leased machines come equipped with an External surge protector to protect their telephone,
network and electronic components; this is in addition to any internal surge protector. Surge protectors must come
with a minimum five year manufacturer’s warranty stating the surge protector will be replaced free of charge if it fails
and the manufacturer will repair or replace any machine damaged by power surges when properly connected to the
surge protector.

All awards must include external surge suppression/power filter equipment at no additional charge and must meet the
following minimum specifications.

• Fax/Modem Protection –
o In/Out RJ11 modular jacks.
o FCC registration numbers must be marked on each unit.
o Must contain automatic resettable fuse in protection circuit.
• Network Protection –

2.4.4.14 SUPPLIES                                                                                                                Explain


Supplies for copiers/color copiers/MFDs/Faxes, for which your company provides maintenance and supplies, must be
furnished to the location designated by the Purchaser on a periodic basis and will include all shipping and handling as
part of the maintenance cost. Agencies will take reasonable steps to safeguard supplies from loss or misapplication.
Your company must deliver supplies within two (2) working days after they are requested. Also, a minimum of four
weeks worth of supplies will be delivered at one time. Failure to promptly deliver supplies may be grounds for
cancellation of the order and/or subsequent cancellation of the contract. Wire spools for finishers must be included in
