   11.1   Housing Need
          Provide evidence of housing need as demonstrated by a local survey, Consolidated Plan or market study. Attach and label as
          "Exhibit 11.1"

   11.2   Administrative Capacity
          Provide a description of relevant experiece and qualifications for non-profit corporation to develop, sponsor, or manage the housing
          program. Attach and label as "Exhibit 11.2"

   11.3   Proposal Narrative
          Provide a proposal narrative addressing, at a minimum:
          11.3.a. The need that the project will fill in the community and why the need is desirable;
          11.3.b. How and to whom you intend to market the proposed project;
          11.3.c. Statistical data used to support both need and marketability;
          11.3.d. If you intend to transfer, sell, or lease the improvements on the land to another eligible entity, describe the parties, terms,
          and conditions of the lease or sale;
          11.3.e. If the site is claiming historical significance, include a letter from the city/town

       interim or bridge financing, if any. If proposal includes LIHTC, a syndication letter setting forth the discounted value of the credit must
       be provided. Include copies of all funding commitments. Attach and label as "Exhibit 11.7"

       If your organization has received Tax Credits in previous years under this program, detail the current status of the project. At a
       minimum, include date of credit reservation, expenditures made, conformity to application and approved changes (if any), anticipated
       or actual date of completion and number of occupied units. Attach and label as "Exhibit 13.2"

13.3   Documentation of location within an RC, QCT or NRZ
       For Workforce Housing and Revolving Loan Fund applicants:
       Certification of percentage of units that will be targeted to families in Regional Centers or Qualified Census Tracts, with the specific
       targeted locations stated.

       For Housing Development applicants:
       Evidence from local government indicating project is located in a Regional Center as defined in the Connecticut Conservation and
       Development Plan 2005-2010, Qualified Census Tract or Neighborhood Revitalization Zone as determined by the municipality. The
       impact on neighborhood revitalization is evidenced by a letter from the municipality.
