                                Submission Details & Checklist

Please submit in the following order (DO NOT submit the checklist with your application):
    An Application for Admission and an Intended Academic & Co-op Work Plan using the attached form.

    A typed, standard application cover letter (maximum 1 page), explaining why you are interested in the
     program, why you should be accepted (discuss the skills, attributes, and experience you would bring to this
     program), your career goals, they type of work that interests you, and how the Co-op Program would enhance
     your academic studies.

    A résumé (maximum 2-pages), which includes the following information: previous work and volunteer
     experiences; research, communication, and computer skills; awards, scholarships, and achievements; extra-
     curricular activities.

    Two to three references, including name, job title, company/organization name, affiliation, phone number, and
     email required. Do not include letters of reference.


Evaluation Criteria:

Any students who submit a complete application package by the due date and meet the above criteria will be
considered for SLAIS co-op admission. Preference for admission will be given to students who are able to relocate,
are open to completing more than one co-op work term, and whose work interests are flexible. . In some cases, an
interview with co-op staff may be required for clarification.

Once accepted, students are required to pay a $212.24 Administration and Career Advising fee, which gives them
access to all co-op training workshops, unlimited access to co-op staff for one-on-one advising, access to the
SLAIS co-op online learning resources for their co-op job search and after graduation, and access to all SLAIS co-
op job postings. Students will also be required to sign a Letter of Understanding agreeing to the terms and
conditions of the Arts Co-op Program.

Once students are accepted into the program, the SLAIS co-op coordinator will provide each admitted student with
a feedback form outlining suggestions for ways to develop and improve the student’s cover letter and resume for
the co-op job search. The form will include a short suggested learning plan to guide students during the co-op job
search process. Also, attendance at the 1-hour SLAIS co-op orientation (Tuesday, October 12, 2010, from 12-1 pm
in IBLC 260 for students starting their SLAIS program in September 2010 or Tuesday, January 11, 2011, from 12-1
pm [Room TBA] for students starting their SLAIS program in January 2011) is mandatory for students who wish to
participate in the program. All other workshops are optional.

Students will also be required to attend a work term planning session with the SLAIS co-op coordinator after their
admittance. Details will be provided once students are admitted to the program.

                         Intended Academic & Co-op Work Plan
In order to assist you with your planning, and also to give the Arts Co-op Office an indication of when you will be
available for your work term(s), it is important that you develop an academic/work plan. While course changes or
conflicts may arise, if accepted into co-op we expect you to be able to follow this schedule as closely as possible.

Directions:
 For each term, check the appropriate box to indicate if you will be taking SLAIS courses or working in the Co-op
    Program.
 Remember, this is a plan - use the best possible course information available at this time to make it.
    Adjustments can be made with new information at a later date, if necessary.
