 Are our performance management reports
 satisfactory?
Are local IT and organisational development support
 systems effective?
Choose and Book is much more than an IT project. It
involves substantial changes in the way staff work in
order to deliver the potential benefits for patients.
Boards should check that this is being adequately
 managed and whether specialist organisational
development advice is needed. The SHA Director of
Development or equivalent can advise if necessary.
 Are local commissioning rules agreed?
By now all PCTs should have agreed the
commissioning rules (i.e. choice menus) for all types
of consultant outpatient referral by GPs and GDPs
and have ensured they have commissioned services
to allow choice of care provider. This information
should have been loaded onto the Directory of
 Services (see below).
Is our Directory of Services working well?
Trust Boards should check that their Directory of
Services (DoS) is adequate. It will have a major
bearing on how well the trust's services will be able
to develop under patient choice and payment by
 results.
 Have we considered benefit realisation?
Substantial benefits are to be gained for patients and
 staff from Choose and Book.
Board members should satisfy themselves that a
formal scheme of benefits realisation is established
 and they should monitor progress.

     Implementation checklist:
     Practice managers
The Choose and Book team have developed an implementation toolkit to help NHS
organisations establish the necessary IT and managerial systems to make Choose and Book a
reality. This includes tailored guidance to suit you, whether you are a GP or practice manager,
consultant or medical secretary, or involved in management, IT or support. This checklist is
designed for practice managers.

GP practice managers will play a central role in co-ordinating the implementation of Choose and
Book and managing change in the practice. Not only are they the fulcrum around which much of
the implementation work in each practice will proceed, much of the change management within
the practice will be co-ordinated by the practice manager.

Own the process and inform the practice of the aims,
goals and benefits of Choose and Book so everyone
understands the service and how it will benefit them
in their job roles. Ensure that all staff are involved in
the Choose and Book implementation process.
Liaise with the local Choose and Book project
management team regarding initiation of planning for
the implementation process in your practice.
Co-ordinate software and hardware upgrades and
Choose and Book installation with the local Choose
and Book project management team and relevant IT
  suppliers as required.
Engage in the referral process redesign by mapping
and evaluating existing processes in the practice,
and then design new processes to take full
advantage of the benefits possible through Choose
  and Book.
Organise familiarisation and training for all users
within the practice based on their new
responsibilities, which have been identified in the
  process redesign.
Having discussed all aspects of the implementation
with other practice staff, make the decision on when
to go live and ensure this, and any impact it might
have, is communicated to all stakeholders at the
  practice.
Ensure ongoing evaluation of the practice systems in
place to ensure maximum ongoing benefits.
