                      - No - Not developed or implemented at all
                      - Partial - Partially developed and/or implemented

Make the selection that best reflects your knowledge related to the statement.

5.1 The following questions relate to Energy Management Planning.

                                                                                                   Yes No Partial
The facility has an up-to-date energy management action plan including energy
performance targets, key energy use performance indicators, staff responsibilities/

          annual facility planning process.
Energy performance benchmarking is carried out at least once per year.

An energy audit of appropriate effort and scope is carried out at least every 5
          years.

5.2 The following questions relate to Organization and Accountability.

                                                                                                   Yes No Partial

The facility management organizational structure clearly delegates responsibility
and accountability for energy management budgets, actions, and targets.

          Utility data management

The following approaches/tools are used to identify energy management
          opportunities:
Suggestions/proposals by energy leader, energy committee, employees, etc.

          Energy performance benchmarking

          Energy audits or feasibility studies

          Maintenance activities

Life-cycle replacement, asset renewal, or renovations

          automation system (as applicable).

5.4 The following questions relate to Project Management and Implementation

                                                                                                    Yes No Partial
The facility uses standardized procedures to manage the implementation of energy
          management projects.
The facility uses a designated project manager to oversee the implementation of
          energy management projects.
Project measurement and verification plans (of appropriate scale) are normally
carried out as part of the project design for energy performance improvement
          projects.
The measurement and verification plan references standards such as the
International Performance Measurement & Verification Protocol (IPMVP); or
ASHRAE Guideline 14-2002, Measurement of Energy and Demand Savings; or
          equivalent standards.

The facility has metering or sub-metering equipment for all energy utilities.

Facility utility information is gathered and recorded on a monthly basis and key
            performance metrics are generated.
Regular reporting on energy performance relative to targets and budgets is
provided to those with accountability at the corporate and facility levels (e.g.,
energy leader, facility managers, energy committee).
Reports on energy use performance are actively used to support decision-making at
the corporate level, and action tasks and training and awareness at the facility
            level.
A variety of tools are used to display performance and targets as part of an ongoing
            facility energy awareness campaign.
