Instructions: Please complete all sections of this Proposal Form. Return completed
form in a sealed envelope and mark on the outside “RFP Disposition of Historic
Houses”. The envelope should be addressed to:

In accordance with Florida Statutes Section 163.380, all Proposals shall be submitted
within 30 days after the date of the public notice. All Proposals must be received no
later than Friday, September 10, 2010, 4:00p.m. The Proposal Form is used to
establish financial ability to proceed, to disclose the intended use of the real estate, and
to establish the overall parameters of the project. In the event you need more space to
answer, submit additional pages.

A signed Land Sales Agreement must accompany the Project Proposal Form at the
time the Proposal is submitted. Failure to meet this requirement will eliminate the
Proposal.

4.       Development Intent:

Describe in detail the proposed use of the location that you are interested in acquiring.
Include the future intended use, anticipated project and the source(s) of financing.
Attach any drawings or plans you have to explain the proposed use and include an
explanation of the timetable for redeveloping the property.

5.       Property Value:

The properties have not been independently appraised. According to the Hillsborough
County Property Appraiser’s website, the assessed land value is as follows:

                                            2502 East 10th Avenue - $16,894.00
                                            2504 East 10th Avenue - $16,681.00



6.       Proposed Purchase Price and Purchase Agreement:

Please indicate purchase price that you are willing to pay for the Project and indicate
your choice of the following two options regarding the purchase and redevelopment of
the Project:

     A. Pay cash for the purchase price of the Property together with all closing costs.
        Proposer shall be required to construct a sidewalk and apply a fire retardant
        material to the eaves and exterior walls that abut the adjacent property. The
        rehabilitation of the exterior of the building must be completed in accordance with
        the Secretary of the Interior Standards for Rehabilitation and Guidelines for

         Rehabilitating Historic Buildings within 2 years from the closing date on the sale
         of the Property.

     B. The City agrees to take a Purchase Money Deferred Payment Mortgage and
        Promissory Note (“DPL”) for the principal amount of the purchase price plus
        closing costs. Proposer shall be required to construct a sidewalk, apply a fire
        retardant material to the eaves and exterior walls that abut the adjacent property
        and complete the exterior rehabilitation in accordance with the Secretary of the
        Interior Standards for Rehabilitation and Guidelines for Rehabilitating Historic
        Buildings (the “Standards”) within 2 years from the date of the closing on the sale
        of the Property. Proposer also agrees to rehabilitate the interior of the building
        contemporaneous with the interior rehabilitation or within 3 years after the
        exterior rehabilitation is complete. All rehabilitation is required to be completed
        and the DPL satisfied no later than a total of 5 (five) years from the closing date
        on the sale of the Property.

In the space provided below, identify your purchase price, your choice of either paying
cash or securing a DPL from the City. Also include a timetable for the Redevelopment
and Rehabilitation, amortization of build out and any special conditions. Please note
the City will not approve a DPL for the purchase of the Property if the Proposer
currently has an outstanding DPL with the City in the amount of $100,000.00 or
more from either the Interstate Historic Preservation Trust Fund or due to the
purchase of a historic home under FDOT Phase II.

7.       Financial Profile:

Provide a summary of the most recent personal and/or corporate financial statement. A
copy of the most recent audited financial statement may be required on all proposed
ownership interests. Should the ownership be in a closely held corporation, the
financial statement of the stockholder may be required. Include three sources for future
financial references such as, banks, corporations, etc.

8.       Proposer’s Profile:

Please indicate addresses of other properties that you currently own within the City.
Please indicate whether they are located in the Ybor City Local or National Landmark
Historic Districts, or any other local or national Historic District. Please also list any
property previously owned (within the last 10 years) and the approximate date of sale.

9.       Consistency with the Ybor City Community Redevelopment Plan

Florida Statute, 163.380(1) states that, “Any county, municipality, or community
redevelopment agency may sell, lease, dispose of, or otherwise transfer real property or
any interest therein acquired by it for community redevelopment in a community
redevelopment area to any private person, or may retain such property for public use,
and may enter into contracts with respect thereto for residential, recreational,
commercial, industrial, educational, or other uses, in accordance with the community
redevelopment plan, subject to such covenants, conditions, and restrictions, including
covenants running with the land . . .”

Provide details regarding how your proposal furthers the goals and objectives of the
Ybor City Community Redevelopment Area 2 Plan. The plan is available on-line on the
City of Tampa Dept. of Economic and Urban Development Department website.

10.      Proposed Terms or Conditions of Purchase or additional needs:

Please state any conditions of purchase other than those stated in this Proposal Form
or in the Land Sales Agreement.

11.      Closing/ Occupancy Schedule:

The City desires to close within 45 days of approval of the sale of the historic house by
the Tampa City Council and the execution of the Land Sales Agreement and Quit-Claim
Deed by the Mayor of the City. Indicate any conditions or special circumstances that
would prohibit the Proposer from closing within this timeframe.

      In selecting a proposal, the City of Tampa (“City”) is not bound to
      make the award on the basis of the highest monetary offer. It is
      the City’s goal to strengthen the mission and stability of the
      residential community. The City reserves the right to accept or
      reject any and all proposals, either in whole or in part, with or
      without cause, waive any informality of any proposals, cancel this
      request for proposals, and to make the award in the best interest
      of the City, subject to City Council approval.
