ITEMS REQUIRED TO OBTAIN A GDO OPERATING PERMIT
DERM Grease Discharge Permit is required upon purchase or the operation of any non-residential facility/business where grease waste is
generated and discharged to the public sanitary sewer system of Miami-Dade County.
I. DERM - GDO Application
A new business owner applying for a Grease Discharge Operating Permit (GDO) must apply in person. If your business is located in a
Municipality, you can visit any of our three Plan Review offices listed below:
Overtown Transit Village Downtown Office 701 NW 1 Court 2nd Floor, Miami
City of Hialeah 601 Palm Avenue, 2nd Floor, Hialeah
West Dade Office 11805 SW 26 Street, Miami
If your business is located in Unincorporated Miami Dade County, you may only apply at our West Dade Office (see details in item V below).
You may complete the top portion of the application prior to visiting the DERM office. The “Payment Affidavit” and “Payment Amount”
sections will be completed by DERM Personnel. No payment for GDO will be accepted until all required documents listed below are
submitted and approval by the DERM Office of Plan Review.
II. Miami-Dade County Occupational License/Local Business Tax Receipt
All Businesses that operate in Miami-Dade County are required to obtain an Occupational License from the Miami Dade County. The
information found on this license must match the document information found on items I, III and IV or V on this form in order to obtain
approval from DERM. You may find out more information regarding the County's Occupational License at http://www.miamidade.gov/
III. State of Florida Hotels and Restaurants License/Department of Agricultural License
Hotels and Restaurants operating in Miami-Dade County must have a license from Florida Department of Business & Professional Regulation.
The information found on this license must match the document information found on items I, II, and IV or V on this form in order to obtain
approval from DERM. For more information on the Hotels and Restaurant license, you may go to http://www.myflorida.com/dbpr/hr. Some
businesses may be required to obtain a license from the Florida Department of Agriculture & Consumer Services called the Annual Food
Permit. For more information on this requirement, please visit http://www.doacs.state.fl.us/fs/permitrq.html and http://www.doacs.state.
IV. Municipal (City or Town) Occupational License/Business Tax License
Each Municipality requires a Business License to operate within their jurisdiction. This license is different for each Municipality and may be
titled “Business Tax License”, “Occupational License” or “Certificate of Use”. The information found on this license must match the
document information found on items I, II, and III on this form in order to obtain approval from DERM.
V. Certificate of use (only required for Business located in unincorporated Miami-Dade County
For the business is located in Unincorporated Miami-Dade County, you must obtain the Certificate of Use for your business location from
Department of Planning and Zoning. This Department is located at 11805 SW 26 Street, Miami, Fla. With this Certificate and items I through
III above, you can visit the DERM Office of Plan Review located at this same site in order to process your GDO Permit Application request.
The Certificate of Use must contain the same information found in items I, II, and III on this form, in order to obtain approval from the DERM.
VI. Possible additional information needed
You may need to obtain Sewer capacity verification information from Water and Sewer Utility serving your area prior to DERM approval.
This will be determined by the Office of Plan Review upon application submittal.
