Should library material that has previously been paid for be recovered, the student/parent is entitled to a refund.
The library media teacher will fill out the Reimbursement Request (Media Account) form and send it to
theschool office. A second copy wil be sent to the Library Technician at the ESC.
The staff member that handles the school's revolving fund will write a check for the amount of the refund and
mail it to the student's home. This same staff member will issue a Revolving Fund: Request for Reimbursement
form, attach the Reimbursement Request (Media Account) form and send it to the ESC Business Office

Lost Library Materials Reimbursement Procedure Checklist
NOTE : All refunds should be done immediately upon return of materials. Under no circumstances should the
Library Media Teacher hold reimbursements for any length of time.
[ ] Student returns lost paid item
Library Media Teacher checks in item in Via (Circulation Desk>Check In tab>scan barcode, "Item status is LOST AND PAID.
Clear it?" window opens, click "Yes")
[ ] Library Media Teacher fills out Reimbursement Request (Media Account) form and routes to site Revolving Fund administrator
[ ] Revolving Fund administrator issues a check from the site Revolving Fund and mails it to the student's home
[ ] Revolving Fund administrator fills out Revolving Fund: Request for Reimbursement form (nhusd.k12.ca.us>Business>Business
Forms) using media lost books account code: 01-9234-0-0000-2422-8699-(site code)-0000, attaches the Reimbursement Request
(Media Account) form and sends them to the ESC Business Office
