                            Complete the following 3 forms and file them with the Probate/Juvenile Office:

FORMS:
   • A packet of forms is available from the Register in Probate for $5.00.
   • You may access all juvenile forms on the internet at: http://www.wicourts.gov/forms1/circuit.htm
   • You can print “.pdf” forms and complete them by hand, OR
   • You can fill in “.doc” forms on the internet and then print them.


Form JC-1690.DOC or JC-1690.PDF – Petition in Juvenile Court for Temporary Restraining Order and/or
Injunction (Child Abuse)

•     Insert the county name at the top of the form – “Washington” County.
•     Under the words “IN THE INTEREST OF”, type or print the name of the child victim and insert his/her date of birth.
      1. Type or print the name of the petitioner (the person bringing this action) and place a check in the appropriate box
          which identifies the petitioner’s relationship to the victim.
      2. Place a check in the appropriate box.
      3. Place a check in the appropriate box.
      4. Insert information regarding the Respondent.
      5. Place a check in all the appropriate boxes.
      6. Type or print a statement about what happened, where, when and who did what to whom. If you do not have
          enough room, attach an extra sheet.
•     On Page 2, after the words “I Request”, place a check in any of the boxes that apply.
•     You MUST sign and date this form in front of a Notary Public. That can be done at the Juvenile Office.



Form JC-1691.DOC or JC-1691.PDF – Notice of Hearing – Temporary Restraining Order (Child Abuse)

•     Insert the county name at the top of the form – “Washington” County.
•     At the top of the form under the words “IN THE INTEREST OF”, type or print the name of the child victim and insert
      his/her date of birth.
•     Type or print the name and address of the Respondent.
•     In the next paragraph, insert the name of the child victim and his/her date of birth.
•     Juvenile Court Staff will fill in the information regarding the date and time for the hearing and the Judge or Family
      Court Commissioner will complete the rest of the form.

•     Insert the county name at the top of the form – “Washington” County
•     At the top of the form under the words “IN THE INTEREST OF”, type or print the name of the child victim and insert
      his/her date of birth.
•     In Question No. 2, type or print the information requested regarding the Respondent.
•     The Judge will complete the remainder of the form.



Other Information

•     There are no filing fees involved in this procedure.
•     After you file these documents, the Family Court Commissioner or Judge will review the contents to determine if a
      “temporary” order can be issued. If the temporary restraining order is signed, staff will give you authenticated copies
      of the Order that you must take to the Sheriff’s Department to have served on the Respondent. You are responsible
      for any cost of serving the Respondent.
                               Respondent MUST be served at least 24 hours before the hearing.
•     YOU must appear at the court hearing. Please bring any witnesses you want to testify, bring any proofs or evidence
      and be prepared to tell the Judge the details of what happened.
•     If the Judge issues the Injunction, either the Respondent will be served in court, or you must have the Respondent
      served with the Injunction through the Sheriff’s Office.
