                                                                                          HOW TO FILL IN THIS FORM
         l      Enter your Community Housing Organisations's name and the month the payment is to be reported against.
         l      Enter the property location.
         l      Enter "Y" in the NMT column if the Tenant is not a member of the Community Housing Organisation. (Applicable only to Housing Co-operatives).
         l      Does the property have rent arrears/rent in advance?
                If "No",
                      Indicate Nil or "0".
                      If ALL properties have NO rent arrears, a notation similar to "No Rent Arrears" can be added to the bottom of the report.
                If "Yes",
                      Enter the Cumulative $ Amount of the arrears or advance (ie total year to date amount, not just for the current month).
                      Enter either "ARR" for Arrears or "ADV" for Advance in the Arrears/Advance column for each property.
                      Enter the Cumulative Number of Weeks the *property has been in arrears or advance (ie total year to date, not just for the current month).
                      Enter the Action Taken to Recover Arrears.

                                             * NOTE: The rental arrears or advance reported against a property should only relate to the current tenant.

         l      Enter the amount of the Capital component (Column A) for each property for the applicable month (including vacant properties).
         l      Enter the amount of the Vacancy Rate Due (Column B) for ANY property that is or has been vacant for any part of the month reporting.
         l      For LEASED PROPERTIES Only (SAHT & SACHA leases), enter the $ value of the Major Maintenance (Column C) due to SACHA.
         l      Enter amounts for all column totals (even if Nil/"0").
         l      Sum the total figures from Columns A, B and C, which should be the cheque amount to be made payable to SACHA.
         l      All cheque details are to be included on the form ie cheque number and cheque amount.
         l      DON'T FORGET TO DATE AND SIGN THE FORM BEFORE FORWARDING TO SACHA, TOGETHER WITH PAYMENT.
         l      INCLUDE A CONTACT NUMBER IF SACHA STAFF HAVE ANY QUERIES, THEY CAN CONTACT YOU IMMEDIATELY.

                Vacant Properties:
                 If a property has been vacant for a period of 6 weeks or more, you are required to report this on your Monthly Rent Arrears, Capital Component
                   and Major Maintenance report, by supplying the property address. This is to be included on every report until the property has been re-
                                                                                    tenanted.
