                                                      2007 Northern Nevada Healthcare Survey

1.    Please report pay rates and employees at your establishment as of January 1, 2007.


2. Please review and refer to the job descriptions when deciding where to list your personnel. The descriptions and departments may be quite broad so that a
maximum number of different organizations can participate in this survey. It is not necessary for the descriptions to precisely match your organization. They are
meant to serve as guidelines to help you decide where to list your personnel.


3.    In completing this questionnaire:

     a)   Provide the TOTAL NUMBER OF INCUMBENTS (employees) within the job classification.
     b)   Provide the AVERAGE BASE WAGE FOR INCUMBENTS (employees) within the job classification.
     c)   Specify the PAY FREQUENCY of the average base wage for incumbents being reported (i.e., monthly, bi-weekly, weekly, hourly, annually).
     d)   Provide the MINIMUM and MAXIMUM wage range rates for each job classification.
