The purpose of this appendix is to summarize of the MYOM Shopping System forms and
how they are used.

Included here are the following:

    1.   The MYOM Rate Sheet Quote form

    4.   The MYOM Quote Comparison form

By logging onto www.mindyourownmortgage.com and becoming a member, you can
point and click you way through the shopping process – it’s completely automated.

The system facilitates the exchanges of information with your lender, including your
quote requests – which gives each lender a unique web page where he can complete the
required forms. Your quotes will automatically be sent to your personal, secure account
and you will be notified by e-mail as they arrive. Once you’ve collected your quotes, you
can easily compare quotes at various interest rates with just a few clicks of your mouse.

The MYOM Rate Sheet Quote includes a handy reconciliation to the Good Faith Estimate, in
addition to all of the information you’ll need to help you compare multiple quotes. The MYOM
Quote Detail form allows you to review important details for each quote and supplies the totals that
are transferred to the Rate Sheet Quote.

Your lender will receive the following instructions with your quote request (refer to the forms on the
following pages):

Your prospective borrower wishes to obtain and review ALL interest rate options for the loan
program you and your prospective borrower have discussed. Please note the following instructions
for completing the forms:

MYOM Rate Quote Sheet:
  • Enter the quote date and all other information at the top of the form.
  • Include ALL interest rate options for the loan program, starting at with the lowest at the top
    and ending with the highest at the bottom.
  • Include the related points or rebate for each interest rate option expressed as a percentage of
    the loan amount and the related dollar amount.
  • Lender fees are to be quoted as one amount, in the same manner as your Good Faith
    Estimate under HUD’s final rule issued in January 2010 (the amount is carried from the
    quote detail form)
  • Total third party fees are carried from the MYOM Detail Worksheet provided herewith.

Quote Detail Worksheets – Parts 1 and 2:
  • Enter the quote date and all other information at the top of the forms.
  • Include all settlement charges as listed on the form (HUD1/HUD1-A reference numbers are
      included for your convenience).

About your Good Faith Estimate (GFE):

The MYOM forms will be automatically delivered to your prospective borrower. Please also supply
your GFE Worksheet, your written rate lock policy, and all other disclosures required by law.

Please complete the GFE for your zero point rate, the rate closest to your zero point rate if you do
not have a zero point rate on the day of the quote, or any rate within the range of rates available as
requested by the prospective borrower.

Your total estimated settlement charges per the GFE must equal the sum of total points and
lender/third party fees on the MYOM Rate Quote Sheet (points for the rate chosen to complete the
GFE) and total government, prepaid, and recurring closing costs as presented in the MYOM Quote
Detail Worksheet (which will not differ, regardless of the interest rate used to complete the GFE).
Please complete the reconciliation of the MYOM form to the GFE as presented on the MYOM Rate
Quote Sheet form.

                Quote Comparison Worksheet

The MYOM Quote Comparison is generated from the quotes you collect. The quote comparison will
allow you to select any rate from the quotes you have received and allow you to easily determine the best
price.


Note:
This form must include the SAME interest rate for each lender. You should pay close attention to points
and lender fees – these can’t change unless there are significant differences between the mortgage
application and what is verified when your loan is processed. If the best price is associated with the
lowest third party fees, keep in mind that these fees (unless your lender guarantees them) can change by
plus/minus 10%.

The new Good Faith Estimate form was passed into law by way of HUD regulations in January 2010.
Included in the example form below are cross-references to the four elements of price (explained in
chapter 9). These cross-references are presented for your convenience as a tool to compare your GFE to
the MYOM Shopping System forms. They are referenced by way of the numbers in the table below.

The MYOM Shopping System excludes costs that have no bearing on your selection of a mortgage
provider (items such as impounds and other recurring closings costs) for the purposes of comparing
quotes. As you study the GFE and the compare the numbered references to the MYOM Shopping
System Forms, you will discover the GFE combines price elements with elements that have no bearing
on the selection of a mortgage provider (prepaids/recurring closing costs) in the total settlement
charges. This makes the GFE a poor tool for comparing quotes.

               HUD-1 and HUD-1A Settlement Statements

HUD-1A and HUD-1 settlement statements are for mortgage refinance and home purchase
transactions, respectively. These forms are used to detail the settlement charges at closing (when your
loan funds).

HUD-1A and HUD-1 reference numbers for detailed settlement charges appear on the MYOM Quote
Detail Worksheets, which are provided for the convenience of lenders completing your quotes.

The HUD-1A and HUD-1 forms also include a comparison of final costs to the GFE. Because the HUD
reference numbers are included on the MYOM Quote Detail Worksheet, you can compare your final
HUD settlement statement to the MYOM forms as well.

In addition, cross-references are provided using the numbers in the following table and are placed on the
forms presented below. This will help you better understand how the items on the HUD forms related to
