application form, please include the information on a separate sheet of A4 paper.
Please include these sheets with your application form, where appropriate,
together with any supporting documents, marking clearly on all enclosures the
name of your firm and the number of the questions to which they refer.

                           GUIDANCE FOR CONTRACTORS



The purpose of the questionnaire is to establish the contractor’s scope of
business and to enable the Council to satisfy itself that the firm has the resources
and the ability to perform satisfactorily.

If information supplied on this form is found to be incomplete or inaccurate it could
result in disqualification by the Council.

Where any question refers to relevant UK legislation, non UK businesses should
answer on the basis of applicable laws in the business’s own jurisdiction. This
form must be completed in the English language or if not, a bona-fide
independent translation must be appended.

GENERAL INFORMATION - Questions 1.1 - 1.24

Please note the term "firm" within this questionnaire refers to sole proprietor,
partnership, incorporated company, co-operative, or other trading organisation as
appropriate.

The questions in this section are designed to ensure that the Council knows
exactly with whom it may be entering into contracts. If there are any significant
changes to this information after you have submitted your application the Council
must be informed.

(1.1/1.2)         This is the name and address that will be used in our records and
                  where, if your firm is invited to tender, tender documents, enquiries
                  etc. will be sent.

(1.5)             In this question "other" would include co-operatives registered as
                  Industrial and Provident Societies, other Local Authorities,

                  Nationalised Industries, (which are incorporated under specific Acts
                  of Parliament), or other public sector firms.

(1.17)            Please ensure that if your firm operates more than one company
                  within a group you are giving the registration number of the firm that
                  you wish to be considered for inclusion. This is the firm that will be
                  eligible for tendering. (Other firms in the group may make separate
                  applications to be considered for tendering but each firm must
                  complete a separate questionnaire). You must notify us if any major
                  changes take place to the structure of the company during the
                  period of this application.


TECHNICAL RESOURCES AND REFERENCES - Questions 2.1 - 2.14

The Council will use the information you supply here to assess whether your firm
is suitable for carrying out the types of work for which you wish to be considered.

(2.8)             The Council reserves the right to approach any, or all, of the clients
                  listed for the purposes of obtaining information on the performance
                  of your firm. If you have never carried out work for a public authority
                  before, the Council may at its discretion consider private clients.


FINANCIAL INFORMATION - Questions 3.1 - 3.11

The Council will use the information given to assess the financial position of your
firm. We also wish to ensure that you are properly registered for VAT and can
meet your responsibilities in respect of VAT and Insurance.

(3.1)             This is the person who will be contacted for further financial
                  information if required.

(3.3)             When submitting accounts, please ensure that the accounts contain
                  information on turnover, the balance sheet and any notes to the
                  accounts, and that they are not draft accounts. Where you are
                  submitting accounts for a group, please ensure you submit accounts
                  for the individual company that is applying for inclusion. If these are
                  not available, a statement of turnover for the applicant company
                  must accompany the group accounts.


EQUAL OPPORTUNITIES - Question 4.1 - 4.26

In order to comply with the requirements of the Sex Discrimination Act (1975), the
Race Relations Act (1976), the Race Relations (Amendment) Act (2000) and the
Disability Discrimination Act (1995), a firm should be able to demonstrate that
they have taken reasonably practical steps to ensure:-

                  i)     The widest possible response from all sectors of the
                         community to employment opportunities within the firm and

                         that all potential job applicants receive equal encouragement
                         to apply;

                  ii)    That no job applicant/employee receives less favourable

                         made to enable applications from people with disabilities.

                  iii)   That all employees are treated fairly and equally with respect
                         to terms and conditions of service, allocation of work,
                         promotion, transfer and training opportunities within the firm,
                         with reasonable adjustments being made for people with
                         disabilities as required.

                  iv)    That there is progress towards a fair representation of
                         women, ethnic minorities and people with disabilities at all
                         levels throughout the workforce;

                  v)     That appropriate arrangements are made to regularly review
                         the firm's progress in achieving equal opportunities.

These criteria are based on the Equal Opportunities Commission and the
Commission for Racial Equality and the Disability Rights Commission Codes of
Practice.

HEALTH AND SAFETY - Questions 5.1 - 5.5

The Council will use the information you supply here to assess your firm's
approach to the management of health and safety. Companies not currently
operating in the UK should attempt to answer each of the questions, substituting
where relevant the appropriate legislation, codes of practice etc. which are
applicable within their domestic jurisdiction.

Section 2(3) of the Health and Safety at Work Act 1974 requires that all
companies/firms employing 5 or more persons shall have a written statement on
health and safety. While the contents of the policy will vary according to the type
and size of the firm, the Council will look to ensure key pieces of information are
contained. The contents should include your firm’s:

           general policy, or statement of intent
           firm - how duties and responsibilities for health and safety are
            allocated; and
           arrangements - the systems and procedures for ensuring the health
            and safety of employees and others who may be affected by your firm.
