This is an unofficial compilation of the radio services and requests for FCC actions that are subject to fees. The public should consult the
Commission's Rules as set out in Title 47 of the Code of Federal Regulations (CFR) for application filing requirements. Further information on
fees may be obtained in Part 1, Subpart G of the CFR or in the Commission's official decision implementing the Congressional Schedule of
Charges. This decision is published in the FCC Record or may be purchased from the Commission's current copy contractor.

The fee amounts contained in this guide are subject to review annually and may result in changes to these amounts. The FCC will issue a notice
to reflect any changes.
                                             PART A

IMPORTANT NOTICE OF CHANGE:

The Remittance Advice 159 has been revised to accept payer and applicant FCC Registration
Numbers (FRN). Effective December 3, 2001, the use of the FCC Registration Number (FRN) is
now mandatory. Failure to register or include an FRN on your FCC Form 159 will result in your
application being returned as unprocessable. If you do not yet have an FRN, you can obtain one
through the FCC website listed below, or by filling out the registration form (Form 160) included
in this fee filing guide and submitting the registration form along with your payment and
Remittance Form 159. Because the use of the FRN is now mandatory, the July 1997 Form 159
will no longer be accepted as a remittance form. Unless the payor and the applicant are the same
person or entity, a separate FRN number must be used for the payor FRN and the applicant FRN.
If you are acting as an agent for an entity, and the remittance (i.e., check or credit card) carries
your name as an official designated to pay on behalf of the entity, you must include the entity’s
name both as payor and applicant in order to use the same FRN. Failure to follow these
instructions will result in your application being returned as unprocessable. Please note that
transactions with the FCC require the use of the FCC FRN. Therefore, please have the
FRN available when contacting or submitting documents to the FCC.

To obtain a revised Remittance Advice 159:
        • A revised Remittance Advice 159 is included in this package
        • Go to http://www.fcc.gov/formpage.html
        • Call the FCC’s Form Distribution Center at 1-800-418-FORM [3676]
        • Pick up the form at the Commission in Room TW-B200
        • Call CORES Helpdesk at 1-977-480-3201
        • Dial Right Fax (202) 418-0177

If you were registered in the Wireless Telecommunications Bureau’s Universal Licensing System (ULS)
and your registration included all the information we needed to issue a Registration Number, we pre-
registered you and mailed you your FRN in a letter dated July of 2000. You may want to check CORES
to determine if you are still registered, or if you have forgotten your registration number.

To obtain a Registration Number:
        • Register at www.fcc.gov, click on E-Filing at the top of the page, and scroll down to
            the CORES Registration link.
        • Check your pre-assigned number at www.fcc.gov, click on E-Filing at the top of the
            page, and scroll down to the CORES Registration link.
        • File FCC Form 160, (CORES Registration) which is included in this package. You
            may obtain the form from the FCC’s web site at http://www.fcc.gov/formpage.html,
            by calling the FCC’s Forms Distribution Center 800-418-FORM (3676), from Right
            Fax by dialing (202) 418-0177, or by calling the FCC’s CORES Helpdesk at 1-877-
            480-3201. You may also pick up the form in Commission Room TW-B200. Mailing
            instructions are on the form.

If you are unable to register electronically, you can still submit your application for a Registration
Number (FCC Form 160) directly to Mellon Bank (along with your filing documents and payment) or fax
the FRN registration form to the CORES Helpdesk at (202) 418-7869. Mellon will register you and you
will receive a confirmation letter from the FCC through the U.S. Postal Service mail.

FCC FORM 159 - REMITTANCE ADVICE FORM

The FCC Form 159, Remittance Advice, must accompany payment to the Federal Communications
Commission for Regulatory Fees, Application Processing Fees, Fines, Forfeitures, Freedom of

Information Act (FOIA) billings or any other debt due to the FCC. The information on this form is
collected and stored in a database to ensure credit of full payment of monies due, to expedite any refunds
due, and to service public inquiries. The form must be used when paying for application fees and
regulatory fees. Please refer to Part B of this guide for specific form requirements. A copy of this form,
with specific instructions, is included and may be used to submit fees. Reproduced forms are acceptable.
FCC Form 159C is a remittance advice continuation sheet that must be used when paying for more than
two call signs in a single filing or when paying for multiple applicants in a single filing. Each call sign
and/or applicant must be listed separately on the FCC Form 159/FCC Form 159C.

METHOD OF PAYMENT (DO NOT SEND CASH)

Payment of fees may be made by check, bank draft, money order, credit card or wire transfer. If paying
by check, bank draft, money order or wire transfer, your remittance must be denominated in U.S. dollars,
drawn on a U.S. financial institution and made payable to "FCC." No postdated, altered or third party
checks will be accepted. No checks will be accepted for processing if older than six months. The
Commission will accept Visa, MasterCard, American Express, and Discover credit cards. Online credit
card payments are also acceptable. Online credit card payment is an available option when filing
electronic application submissions. If you choose this method of combined filing/payment, do not
send an FCC Form 159 to Mellon.

If paying by wire, applicants located in foreign countries should contact their local bank to determine
what U.S. financial institution their bank is affiliated with that will allow a transfer of funds. Please keep
in mind that there may be an additional transfer fee added from your bank for this service. If an
additional cost is required by your bank, and you have not included additional funds to cover this cost, it
will be taken out of the funds that will be transferred. This will result in your remaining transfer amount
being insufficient to cover the fee for the filing. Please check with your bank prior to completing this
transaction to determine their policy. For further instructions in making payment by wire see our website

Payment of fees, fines, and other debts may also be made by electronic payment. There are two types of
electronic payments available. The first type is designed specifically for larger corporations capable of
CPU/CPU communication. Under this method, the payer bank wires funds directly to the Commission's
lockbox bank. As with cash payments, the funds must be wired from an U.S. financial institution. The
second type is designed for smaller businesses and individuals. Under this method, the lockbox bank is
given authority to withdraw funds from the customer's bank account. Authorization can be transmitted by
a personal computer or by calling a toll-free number. To obtain more information about electronic

NONFEEABLE APPLICATIONS

All nonfeeable applications filed in Washington, DC must be filed directly with the Secretary's Office,
Room TWB204, 445 12th Street, SW, Washington, DC 20554 and should be captioned Attention:
Enforcement Bureau.

FEE EXEMPT APPLICATIONS

Please check the appropriate block on your application and provide the proper documentation, as
required, to certify that your application is fee exempt. 47 CFR, Part 1, Section 1.1114 of the
Commission's rules explains who qualifies as fee exempt.

MANAGING DIRECTOR DECISIONS

Requests for waivers, fee determinations, reconsiderations, applications for review, deferments, and
specific refund requests are referred directly to the Office of the Managing Director. Each request is

forwarded to the Office of General Counsel for review and legal determination. 47 CFR, Part 1, Sections
1.1113, 1.1114 & 1.1117 govern the Commission's policy in these instances. The requester will receive
written notification of the Managing Director's decision. These decisions are published monthly and are
placed in FCC Docket 86-285. All such requests must be submitted in writing to:

WAIVERS, FEE DETERMINATIONS, AND DEFERRALS PROCESS

The required filing fee must be paid for the service requested using the normal process. All requests for
waivers and fee determinations are filed directly with the Managing Director in Washington, DC. If the
Commission grants the waiver request or the fee determination results in a lower fee, a refund will be
issued. Deferrals of fees are also filed in Washington, DC, and have a limit of up to six months with good
cause. Deferrals of fees are considered when the ability to pay the fee by financial hardship (i.e.,
bankruptcy), and must be accompanied by supporting documentation.

REFUND PROCESS

The appropriate Bureau/Office handles routine refund requests. When a Bureau/Office determines that a
refund is warranted, it is forwarded to the Office of the Managing Director, Revenue and Receivables
Operations Group for processing. The Revenue and Receivables Operations Group (RROG) handles all
approved refund requests. The refund process usually takes approximately four (4) weeks depending
upon the complexity of the request. Once a request has been reviewed and processed by RROG, it is
forwarded to the U.S. Treasury in San Francisco, California where a check is issued to the payer of the
remittance. For further information on return or refund of charges, refer to 47 CFR, Part 1, Section
1.1113 of the Commission's Rules.

MAILING INSTRUCTIONS

Parties hand-delivering applications or filings may receive dated receipt copies of the application or filing
from the acceptance clerk at the time of delivery. Receipts will be provided for mail-in applications or
filings if an extra copy of the application or filing is provided along with a self-addressed, stamped
envelope. Only one piece of paper per application will be stamped for receipt purposes. A "stamp and
receipt" copy must be placed on top of the original package and clearly identified as a return copy.

When delivering feeable applications by hand or by courier, use the following address: Federal
Communications Commission, c/o Mellon Client Service Center, 500 Ross Street, Room 670, Pittsburgh,
PA 15262-0001 (Attention: FCC Module Supervisor). This address is for hand or courier delivery
only. DO NOT use it for mailing applications. When using the address, enclose the application
package in an inner envelope marked with the correct Post Office Box number. See the relevant
Bureau/Office Fee Filing Guide for the correct Post Office Box number.

                                  GENERAL FEE EXEMPTIONS

Further fee exemptions may be identified with a specific radio service in this guide. See 47 CFR,
Part 1, Section 1.1114 of the Commission’s Rules for further explanation of who qualifies as a fee
exemptions.

The following are exempt from ALL fee requirements:

   o   Governmental Entities - Check the block for Governmental Entity on your application.

   o   Non-commercial Educational Broadcast stations

   o   Requests for Special Temporary Authority in the Ship and Aircraft Radio Services

   o   Amateur (except Vanity)

   o   Pro-Forma Transfers of Control and Assignments of Authorization if filed under the forbearance
       rules.

The following are exempt from either application and/or regulatory fee requirements:

   o   Nonprofit applicants in the Special Emergency and Public Safety Radio

   o   Services are exempt from the application fee, provided a current Determination Letter
       accompanies the application from the Internal Revenue Service documenting their non-profit
       status under IRS Code Section 501(c)(3).

   o   All Special Emergency and Public Safety Radio Service applicants are exempt from the
       regulatory fee.

   o   Nonprofit applicants in radio services other than Special Emergency and Public Safety are
       required to pay the application fee. However, they are exempt from paying the regulatory fee,
       provided a current Determination Letter accompanies the application from the Internal Revenue
       Service documenting their non-profit status under IRS Code Section 501 or current certification
       as a nonprofit corporation or other nonprofit entity by state or other governmental authority.

   o   No regulatory fee is required for Commercial Radio Operators Licenses.

   o   No application fee is required for Amateur Vanity licenses.

                                                           AUCTIONED LICENSES
Initial License Applications. Auction winners file with the Bureau a "long form" application, FCC Form 601 "FCC WIRELESS
TELECOMMUNICATIONS BUREAU APPLICATION FOR RADIO SERVICE AUTHORIZATION" shortly after an auction closes. There is no
application fee associated with the initial long form application, or amendments to same.

Post-grant License Applications. Once an initial auctioned license has been granted to a winning bidder in an auction, all subsequent post-grant
license applications may require a fee as described in this fee guide. Specifically, if there are fees applicable to incumbent licensees in a given radio
service, the same fees will apply to auction winners. For specific fee amounts, see the individual sections of this guide addressing the specific radio
services.

Note that there are no application fees for any type of application or request regarding the Personal Communications Service (PCS), the Wireless
Communications Service (WCS), Local Multipoint Distribution Service (LMDS), General Wireless Communications Service (GWCS), Location
and Monitoring Service, Multilateration (LMS), or 700 MHz (Lower Band, Upper Band, including Guard Bands). Regulatory fees, which are
assessed annually, may apply to these services. Refer to Part C of this guide for information on regulatory fees for these services.




                                            UNIVERSAL LICENSING SYSTEM
The Wireless Telecommunications Bureau has been implemented into a Universal Licensing System (ULS) for all Wireless services except for the
Multipoint Distribution Services. ULS is an integrated database and automated processing system which will support electronic filing of applications
and other filings associated with applications (i.e. amendments, waiver requests, transfers and assignments of licenses, licensee ownership
information, etc.), collection of licensing information and public access to such information. The Commission has replaced eleven separate WTB
licensing databases with this new integrated licensing system. The radio services were converted to ULS and public notices were issued with specific
instructions as each radio service migrated to the new system.

Upon submitting applications electronically via ULS, the applicant receives a confirmation of the fee(s) due (if any) for the application and is
instructed to either pay on-line credit card or print FCC Form 159 directly from ULS. The applicant must remit payment as described in this guide
within 10 calendar days of submitting the application, or the application will be dismissed. Payment received more than ten (10) days after
electronic filing of an application on a Bureau/Office electronic filing system (e.g., ULS) will be forfeited (see §§1.934 and 1.1109).

In order to avoid processing delays and dismissal due to insufficient fees, the Bureau recommends that applicants filing feeable applications via ULS
observe the following:

          (1)   Use the ULS-generated FCC Form 159;
          (2)   Pay the amount specified by ULS;
          (3)   Submit fee payment in a timely manner;
          (4)   Submit only one instrument of payment (e.g., a check) for each FCC Form 159;
          (5)   If paying by credit card, be sure to complete Section E of FCC Form 159; and,
          (6)   Mail or deliver FCC Form 159 and payment to the proper Pittsburgh address.

Marine Coast Radio Service was converted to the Universal Licensing System (ULS). For information on ULS electronic filing, visit the Internet @

licensees will also be assessed an annual regulatory fee. A public notice will be sent out indicating the regulatory fee payment

                               INFORMATION ABOUT REGULATORY FEES
The Federal Communications Commission, by Congressional mandate, implemented a regulatory fee schedule to recover costs incurred
by the Commission relative to policy and rulemaking actions, as well as its enforcement, user information and international activities.

The regulatory fees do not currently apply to Amateur Radio Services (except Vanity call signs), governmental entities, Commercial
Radio Operator Permits and Licenses, Non-commercial Educational Broadcast stations, Special Emergency & Public Safety Radio
Services, modifications of existing authorizations unless that modification results in a renewal or reinstatement for which a regulatory fee
is required, and non-profit entities. Non-profit entities must provide a current Determination Letter from the Internal Revenue Service
when filing documentation for non-profit status under IRS Code Section 501 or current certification as a nonprofit corporation or other
nonprofit entity by state or other governmental authority.

These regulatory fees will be assessed annually and will apply to those licensees and entities, which benefit from the Commission's
regulatory activities, other than licensing or application processing functions. Among the services affected by these fees are the Media
Services and Commercial Mobile Radio Services (CMRS). The regulatory fees for Media and Commercial Mobile Radio Services
(CMRS) are paid on a predetermined date established by Public Notice (See Public Notices issued July, 30 2003 regarding FY 2003
payments). The regulatory fees for other Wireless Telecommunications Services are paid in advance to cover the term of the license
and are submitted along with the application fee at the time the application is filed except for Local Multipoint Distribution. See Part B for
additional information on the other Wireless Telecommunications Services regulatory fees.

Regulatory fees may be adjusted annually by the Commission pursuant to Section 9 of the Communications Act 47 U.S.C. 159.


                          MULTIPOINT DISTRIBUTION REGULATORY FEES
When paying a regulatory fee for the Wireless Telecommunications Bureau Multipoint Distribution Services (LMDS, MDS, MMDS), the
payor must submit FCC FORM 159 (FCC Remittance Advice), along with the appropriate fee. FCC Form 159C (FCC Remittance Advice
Continuation Sheet), may be used for any additional requests within the same lockbox. You may use a single FCC Form 159 to pay
multiple regulatory fees and to pay for more than one service in the Wireless Service. Instructions for completing both forms are
included in this guide. PLEASE NOTE: The first two numbers in the payment type codes that follow represent the year the user
is paying regulatory fees for. These numbers will change for subsequent years.

Local Multipoint Distribution - Payment Type Code: 0314 $265.00 per call sign

The FCC Form 159, "Remittance Advice,” and FCC Form 159-C, (Continuation Sheet) is a multi-purpose
form that must accompany any payment to the Federal Communications Commission (e.g., Regulatory
Fees, Processing Fees, Auctions, Fines, Forfeitures, Freedom of Information Act (FOIA) Billings, or any
other debt due to the FCC). The information on this form is collected to ensure credit for full payment, to
ensure you receive any refunds due, to service public inquiries, and to comply with the Debt Collection
Improvement Act of 1996.

                                Instructions for Completing FCC Form 159 & 159-C

NOTE: All required blocks must be completed or it may result in a delay in processing or
the return of your application.
(1) Lockbox No. # - Enter the appropriate six-digit P.O. Box Number as found in either the FCC Fee Filing Guide
for the service requested, or as specified in the Public Notice.

(2) Payer Name - Enter the name of the person or company (i.e., maker of the check) making the payment. If using
an individual name, enter the last name, first name, and middle initial. If a company, enter the name used
commercially. If paying by credit card, enter the name exactly as it appears on your card.

(4)    Street Address Line 1 - The street address or post office box number to which correspondence should be sent.

is foreign, enter the appropriate ZIP (postal) code.

(9) Daytime Telephone Number - Enter the payer's ten -digit daytime telephone number, including area code. For
foreign telephone numbers include the appropriate country dialing access code, as if you were calling from the
United States. This daytime number should be the number where you can be reached during norma l business hours.

(10) Country Code - This section is for payers who have an address outside the United States of America. Enter
the appropriate code here. To obtain country code information, contact the Mailing Requirements Dept. of the U.S.
Postal Servi ce.

(11) Payer (FRN) - Enter the payer's ten-digit FCC Registration Number (FRN) assigned by the Commission
Registration System (CORES). The FRN is a unique entity identifier for everyone doing business with the

processing.) You are allowed to file multiple actions on one FCC Form 159. There are three ways "multiple actions"
are defined. The following examples provide instructions on how multiple actions should be filed when using FCC
Forms 159 & 159-C:

(i) If a single service allows for a quantity of more than one of the same action, as defined in the appropriate
FCC Fee Filing Guide or Public Notice, complete Section C (e.g., if you are filing an ownership report in the
mass media services you may pay for both your AM & FM stations using the same payment type code and a quanti ty of
two as long as it can be filed in the same lockbox). Blocks 28 & 29 are only to be completed when required by the
Bureau/Office or by Public Notice.

(ii) If you are filing concurrent actions (not the same actions) in the same lockbox, on the same ap plication,
refer to the appropriate FCC Fee Filing Guide or Public Notice for specific instructions as to the number of
quantities allowed. Complete Section C (e.g., you may file a regulatory fee for a CARS license and Broadcast
Auxiliary license or you ma y file a regulatory fee for a mass media service and a common carrier service on the
same FCC Form 159 by using the designated payment type codes, and quantities as defined by the Public Notice).

(iii) If a single Remittance Advice is used to pay for more than one applicant, licensee, regulatee or debtor,
for permitted action(s) in the same lockbox, then a Conti nuation Sheet (159 -C) must be completed for each
applicant, licensee, regulatee or debtor (e.g., if you are paying for different applicants submitting separate
Domestic 214 Applications in the common carrier services, they can all be filed on one FCC Form 159 as long as
they are filed in the same lockbox). A separate FCC Form 159 must be completed and submitted for each applicant.
Blocks 28 & 29 are only to be completed when required by the Bureau/Office or by Public Notice.

Remember, if any of these additional applications fall into category (i) or (ii) above, you can follow those
instructions. Make sure to check the appropriate FCC Fee Filing Guide or Public Notice for any special filing
stipulations that may apply.

(25) Quantity - Enter the total number of actions required with this submission. Refer to the FCC Fee Filing
Guide or Public Notice for information concerning multiple requests.

Use this form for any additional services pertaining to this filing or if you are paying for multiple applicants

                          HOW TO USE FCC FORM 159-REMITTANCE ADVICE

The FCC Form 159, "Remittance Advice,” and FCC Form 159-C, (Continuation Sheet) is a multi-purpose
form that must accompany any payment to the Federal Communications Commission (e.g., Regulatory
Fees, Processing Fees, Auctions, Fines, Forfeitures, Freedom of Information Act (FOIA) Billings, or any
other debt due to the FCC). The information on this form is collected to ensure credit for full payment, to
ensure you receive any refunds due, to service public inquiries, and to comply with the Debt Collection
Improvement Act of 1996.

Note: Fee Filing Guides can be obtained by calling Forms Distribution -- (202) 418-3676 or 1-800-418-3676, or by calling
FCC's fax-on-demand -- (202) 418-0177 from the handset of a fax machine.

                                Instructions for Completing FCC Form 159 & 159-C

NOTE: All required blocks must be completed or it may result in a delay in processing or
the return of your application.
(1) Lockbox No. # - Enter the appropriate six-digit P.O. Box Number as found in either the FCC Fee Filing Guide
for the service requested, or as specified in the Public Notice.

                                                            SECTION A

(2) Payer Name - Enter the name of the person or company (i.e., maker of the check) making the payment. If using
an individual name, enter the last name, first name, and middle initial. If a company, enter the name used
commercially. If paying by credit card, enter the name exactly as it appears on your card.

(3)   Total Amount Paid - Enter the total amount of your remittance.

(4)    Street Address Line 1 - The street address or post office box number to which correspondence should be sent.

(5)    Street Address Line 2 - This line may be used if further identification of the address is required.

(6)   City - The name of the city associated with the street address given in (4).

(7) State - If the payer has a United States mailing address enter the appropriate two -digit state abbreviation
as prescribed by the U.S. Post Office. If the payer has a mailing address outside the United States, leave this
section blank.

(8) ZIP Code - Enter the appropriate five or nine -digit ZIP code prescribed by the U.S. Post Office. If address
is foreign, enter the appropriate ZIP (postal) code.

(9) Daytime Telephone Number - Enter the payer's ten -digit daytime telephone number, including area code. For
foreign telephone numbers include the appropriate country dialing access code, as if you were calling from the
United States. This daytime number should be the number where you can be reached during norma l business hours.

(10) Country Code - This section is for payers who have an address outside the United States of America. Enter
the appropriate code here. To obtain country code information, contact the Mailing Requirements Dept. of the U.S.
Postal Servi ce.

(11) Payer (FRN) - Enter the payer's ten-digit FCC Registration Number (FRN) assigned by the Commission
Registration System (CORES). The FRN is a unique entity identifier for everyone doing business with the
Commission. The FRN can be obtained electronically through the FCC webpage (www.fcc.gov.com) or by requesting FCC
Form 160 through the FCC forms webpage (www.fcc.gov/formpage.html).

(19) Daytime Telephone Number - Enter the applicant's ten -digit daytime telephone number, including area code.
For foreign telephone numbers include the appropriate country dialing access code, as if you were calling from
the United States. This daytime number should be the number where you can b e reached during normal business
hours.

(20) Country Code - This section is for applicants who have an address outside the United States of America.
Enter the appropriate code here. To obtain country code information, contact the Mailing Requirements Dept. of
the U.S. Postal Service.

(21) Applicant (FRN). Enter the applicant's ten-digit number FRN assigned by the Commission Registration System

                                                      SECTION C

(23) Call Sign/Other ID - Enter an applicable call sign or unique FCC identifier, if any, as prescribed by the
appropriate FCC Fee Filing Guide or Public Notice.

(24) Payment Type Code - Enter the appropriate payment type code for the service you are requesting as found in
the appropriate FCC Fee Filing Guide or Public Notice.
(Incorrect or omitted payment type codes may result in your application or filing being returned to you without further
processing.) You are allowed to file multiple actions on one FCC Form 159. There are three ways "multiple actions"
are defined. The following examples provide instructions on how multiple actions should be filed when using FCC
Forms 159 & 159-C:

(i) If a single service allows for a quantity of more than one of the same action, as defined in the appropriate
FCC Fee Filing Guide or Public Notice, complete Section C (e.g., if you are filing an ownership report in the
mass media services you may pay for both your AM & FM stations using the same payment type code and a quanti ty of
two as long as it can be filed in the same lockbox). Blocks 28 & 29 are only to be completed when required by the
Bureau/Office or by Public Notice.

(ii) If you are filing concurrent actions (not the same actions) in the same lockbox, on the same ap plication,
refer to the appropriate FCC Fee Filing Guide or Public Notice for specific instructions as to the number of
quantities allowed. Complete Section C (e.g., you may file a regulatory fee for a CARS license and Broadcast
Auxiliary license or you ma y file a regulatory fee for a mass media service and a common carrier service on the
same FCC Form 159 by using the designated payment type codes, and quantities as defined by the Public Notice).
Complete a separate item for each action required. Blocks 28 & 29 are only to be completed when required by the
Bureau/Office or by Public Notice.

(iii) If a single Remittance Advice is used to pay for more than one applicant, licensee, regulatee or debtor,
for permitted action(s) in the same lockbox, then a Conti nuation Sheet (159 -C) must be completed for each
applicant, licensee, regulatee or debtor (e.g., if you are paying for different applicants submitting separate
Domestic 214 Applications in the common carrier services, they can all be filed on one FCC Form 159 as long as
they are filed in the same lockbox). A separate FCC Form 159 must be completed and submitted for each applicant.
Blocks 28 & 29 are only to be completed when required by the Bureau/Office or by Public Notice.

Remember, if any of these additional applications fall into category (i) or (ii) above, you can follow those
instructions. Make sure to check the appropriate FCC Fee Filing Guide or Public Notice for any special filing
stipulations that may apply.

(25) Quantity - Enter the total number of actions required with this submission. Refer to the FCC Fee Filing
Guide or Public Notice for information concerning multiple requests.

(26) FEE Due for (PTC) - Enter the fee due for the PTC listed in item 24.


(27) Total Fee - Enter the total fee due by multiplying Block 25 (Quantity) times Block 26
(Fee Due for PTC).

(28) FCC Code 1 - This section is used for special filing codes as required by the Bureau/Office or Public
Notice. Do not complete this block unless instructed to do so.

(29) FCC Code 2 - This section is used for special filing codes as required by the
Bureau/Office or Public Notice. Do not complete this block unless instructed to do so.

(30) Certification Statement - This section must be completed and signed. Failure to do so may delay the processing
of your application/filing.

(31) Credit Card Payment Information - If remitting payment by credit card, place an "x" in the appropriate block
for the credit card bein g used – MasterCard, Visa, AMEX, or Discover. Enter your credit card number and
expiration date (For example: (mm/yyyy)). Sign and date the FCC Form 159 to authorize your credit card payment.
(If any area required for credit card approval is incomplete, the application will be returned unprocessed.)


Form 159-C FCC Remittance Advice Continuation Sheet
Use this form for any additional services pertaining to this filing or if you are paying for multiple applicants
with a single payment. (See Sections B and C of the instructions to assist you in completing this form). For each
additional applicant listed in Section BB of the FCC Form 159-C, you must complete Section BB - Block 21: FCC
Registration Number. Each additional applicant must use a separate Form 159 -C.

Note: Checks must be denominated in U.S. Currency and deposited in an U.S. financial institution. No checks
drawn on a foreign bank will be accepted.

The FCC Form 160, ("CORES Registration Form") is a form that must be completed to obtain a FCC
Registration Number (FRN).         The FCC Registration Number will be assigned by the Commission
Registration System (CORES) and is required for anyone doing business with the Commission (feeable and
non-feeable). The information on this form is collected to ensure you receive any refunds due, to service
public inquiries, and to comply with the Debt Collection Improvement Act of 1996.
