         ACCEPTABLE DOCUMENTATION TO VERIFY
                   U.S. CITIZENSHIP

       YOU MAY SUBMIT A COPY OF ANY ONE OF THE FOLLOWING TYPES OF
       DOCUMENTATION TO VERIFY YOUR U. S. CITIZENSHIP STATUS:

       A   Birth Certificate
       A   valid U. S. Passport Book (current or expired)
       A   valid U. S. Passport Card (current or expired)
       A   Certificate of Naturalization from the INS
       A   Certificate of Citizenship from the INS
       A   Report of Birth Abroad of a Citizen of the United States
           (Must bear an embossed seal including the words “United States of America” and “State Department”.
           Acceptable Form numbers: FS-240, FS-545, or DS-1350)

IF YOU DO NOT HAVE ACCESS TO ANY OF THESE DOCUMENTS AND YOU ARE A U.S.
CITIZEN, CONTACT THE APPROPRIATE GOVERNMENT AGENCY AND MAKE ARRANGEMENTS
WITH THEM TO SEND A COPY OF THE DOCUMENT TO YOU. HERE ARE SOME TELEPHONE

If you are now a U.S. citizen and became a citizen after you received your Social
Security card, go to the Social Security Administration, provide them your updated U.S.
citizenship status, and request a duplicate Social Security card.
