5. Construction: a. Bid abstract with engineer’s estimate b. Narrative describing construction sequence, problems, conditions c. List of prime contractors and subcontractors d. Change Orders/Supplemental Agreements e. Schedule information including dates for: award, NTP, contract and actual completion f. Statement of compliance with labor clauses g. Completion: 1) Final quantities (explain variations over 10%) 2) Liquidated damages (describe if assessed) 3) As-builts (AutoCad or pdf file and optional 11x17 hard copy) 4) Final inspection report 5) Contractor statement that no further payment is due/subs paid in full 6) Updated Pavement Strength Survey (5320-1) 6. Summary of Test Results: a. Construction Management Plan (only needed for paving over $250,000) b. Acceptance Test Summary Sheets c. Mix Design Summary d. Consultant Certification for Project Testing (E-1) e. Sponsor Certification of Testing Laboratories (F-1) 7. Summary of Project costs (Final Payment Summary Worksheet)
8. DBE Program: (only if over $250,000 federal) a. Approval Date by Civil Rights b. DBE Participation Summary form submitted 9. Required Determination for Project Close-out
10 ALP Revision date (accepted by ADO) (copy of approval letter to state planner)
