Policy:
Definition of series presentation application: An application for Category 1 CME for regularly
scheduled events that are linked together by a need and common objectives designed to meet the
need. It can include Grand Rounds, case presentations, and tumor boards or morbidity/mortality
conferences. A series application may be submitted for a calendar year.
Series presentation applications may be approved for Category I credit if they meet the following
conditions.
1. All series presentation applications must include an identified need for the series,
objectives, specified target audience, series budget plan, evaluation method, planning
committee disclosures, and publicity sample (see below).
a. Needs assessment
b. Global objectives must address the overall need for the series
c. An evaluation must be conducted at the completion of the series and must
evaluate how the global objectives were met.
d. Disclosures for all planning committee members are submitted as part of the
application. All other components of the Disclosure Policy apply.
e. Publicity must include the required CME Accreditation Statement, behavioral
objectives, and acknowledge any commercial support.
2. Should a potential conflict of interest exist, the Activity Planning Committee must meet
or discuss with the presenter prior to the activity, review the presentation, ensure that a
balanced perspective will be presented, and document the resolution. Documentation
related to conflict resolution must be submitted to the CME Coordinator prior to the
activity.
3. Post-Series Follow-up Documents: Submit to the CME Coordinator within 30 days after
the series: CME 1 Activity Summary, including a list of presenters and topics, combined
attendance statistics, and final budget; series evaluation summary based on global
objectives.
Additional Requirements
Grand Rounds
1. Objectives: Specific objectives for individual topics must be submitted prior to
publicizing and at least two weeks before a Grand Rounds conference.
2. Publicity: A sample of publicity should be submitted at least two weeks prior to
distribution and must include the behavioral objectives specific to the topic, the CME 1
required accreditation statement, and acknowledge any commercial support.

3. Evaluation: Each activity must also be evaluated using the standard evaluation form to
assess whether content objectives were met, measure speaker effectiveness and bias, and
capture comments relevant to the effectiveness of the presentation in promoting physician
change in practice, suggestions for future topics, etc.
4. Disclosures: Grand Rounds speaker disclosures and credentials must be submitted to the
CME Coordinator at least two weeks in advance to screen for potential conflicts of
interest.
5. Post-Activity Follow-up Documents: The following items must be submitted to the CME
Coordinator within 30 days after each Grand Rounds: Series Session Attendance and
Budget form, evaluation summary, attendance roster, verbal disclosure verification form,
syllabi or handouts distributed.
Case Presentations
1. Confidentiality:
Patient identifiers should be blocked on slides, charts, handouts, etc., to maintain
confidentiality.
Each attendance sign in sheet must state: By signing this attendance record, I
agree to protect the confidentiality of all patient information discussed at this
CME Category 1 activity.
2. Immediate Post-Activity Follow-up: Submit to the CME Coordinator as soon as possible
following each activity session:
a. Disclosures and credentials for primary case presenters or moderators if not
included in the application [Note: Individual disclosure forms are valid for one
calendar year, so only one is needed per presenter.]
b. Signed verbal disclosure verification form
c. Attendance roster
d. Series Session Attendance and Budget form
Tumor Boards and Morbidity/Mortality Series
1. Confidentiality:
Patient identifiers should be blocked on slides, charts, handouts, etc., to maintain
confidentiality.
Each attendance sign in sheet must state: By signing this attendance record, I
agree to protect the confidentiality of all patient information discussed at this
CME Category 1 activity.
2. Disclosures:
Relevant disclosure information must be obtained and made available for
primary case presenters, moderators, regular participants, and planners.
Moderators must ask participants who exhibit bias toward specific products or
therapies to verbally disclose any relevant financial interests and ensure that
other options are discussed. Verbal disclosure must be documented.
3. Immediate Post-Activity Follow-up: Submit to the CME Coordinator as soon as possible
following the activity:
New disclosures and verbal disclosure documentation. [Note: Individual
disclosure forms are valid for one calendar year, so only one is needed per presenter.]
Attendance roster and compiled statistics (including telehealth sites).
Rationale: Case presentations and Grand Rounds series provide relevant and timely education
for practicing physicians.
