                                     this application, along with any requested changes.

                                    To do business with Qwest as a CLEC or Reseller, a New Customer Questionnaire
                                    must be completed by you or a representative from your organization. Your Qwest
                                    Service Manager will assist you with this process.


      The previous version of this application will only be accepted for thirty (30) calendar days after the new version
      is posted to the web site. To access current forms, refer to the application forms web site address listed above.

 l       cable paths, etc.) is communicated to entities such as OneCall/Blue Stake, as appropriate.

l     B. The applicant is responsible for the submission of separate service order(s) to secure the
l        additional product(s) and/or service(s) required for the provisioning of this Collocation product.
l        This application does not ensure that those product(s) and/or service(s) will be provisioned under
l        the same timelines established for this Collocation product.

      submission.

    3. Note Section:
       Note 1: An augment or change request submitted to QWEST during the feasibility, quote, or
               construction phases of the Field Connection Point (FCP) job may impact the
               Ready for Service (RFS) date.
       Note 2: A Change Order submission will require a revised quote from QWEST. An Augment
               Order may require a Quote Preparation Fee (QPF) if contractually applicable. Please
               complete all applicable pages of this application when requesting an augment or cha
       Note 3: An augment is requested and performed on a completed FCP site or a FCP site
               request that has been accepted by the CLEC and for which 50% down payment has
               been received.
       Note 4: An augment or change to a FCP site can be a minor or major addition or reduction of
               the quantity of element(s) which are part of a FCP site.
       Note 5: A change order is any change requested on an active (accepted) FCP order prior
               to receipt of the 50% down payment. When a change order is accepted, the order
               interval restarts.

          Note 1:  Your request will be reviewed for technical and operational feasibility.
          Note 2:  Requirements and time frames for evaluating your request will follow those in the
                   FCP product process.
          Note 3:  A diagram of your proposed method of interconnection at the FCP location must be
                   attached to this application.

III.   DETAILED ORDERING INFORMATION
       A. GENERAL INFORMATION
          1. Indicate how the FCP facilities will be used, e.g.
             Dial Tone, Voice, Voice & Data.
          2. Identify the type of equipment that will be interconnected
              to the Qwest network, e.g. Cross-Box, DSLAM (Optional)
          3. Enter the size, gauge, and quantity of cable between the FCP and your equipment by Loop typ

    C. LOOP RESERVATION (Colorado only)
