Improving the Patient & Carer Experience
NSW Health Service Performance Improvement Branch has developed a method of collecting and
analysing stories about patient and carer experiences. This method of appreciative enquiry can be
used regularly by all staff. The method deliberately goes beyond questions of satisfaction, and
explores patient and carer perceptions of their care and what they actually experienced. Visit:

The most significant positive and negative dimensions of care should be identified to assist in
discerning the key positive and negative aspects of their journey. Most journeys include high points
and low points, so patients and carers are encouraged to consider the aspects of the journey that
were most important or had the greatest impact on them, and use these to identify the most positive
and negative aspects of the journey as a whole.
You can use this information from the stories to identify areas for improvement as well as
acknowledging the areas you are doing well. This can be done using this spreadsheet which tallies
the positive and negative aspects in terms of never, sometimes or always in relation to 8 dimensions.
The graph generated from the spreadsheet will enable you to determine the positive and negative
dimensions of care and identify the area you will need to focus to improve the patient & carer
experience.
How to use this Data Spreadsheet
1. Firstly, enter the following data below
Enter your AHS name
Enter the facility name
Enter The Journey name
2. Click here to access data page to populate graph
1) Each line represents an individual story from your selected journey.
2) To populate the graph on the summary page go to data page and enter free
text into columns A,E,F and H. To enter remaining data please click on cell
to generate the drop down box and select appropriate information.
3) Once all data has been entered the graph on the summary page will be
completed for use to present in the monthly reports and/or meeting and presentations
3. A Summary of your data can be printed from here
4. The data from this spreadsheet will automatically generate the graph for the
Patient & Carer Experience monthly report.
5. A pivot table can be generated over time if all data captured is stored on a single spreadsheet.
6. You should now return your data to the Area co-ordinator who will then forward a final report to the
Department of Health.
Please do not make changes to the architecture of this file, if you have thoughts on how it can
be improved please contact the Patient and Carer Experience team at the Department of
Health. The team can then coordinate the changes so everyone can benefit.
