                                      Benefits Overview


All full-time employees (and part-time employees working a minimum of 24-hours per week) are eligible
to receive benefits listed on this overview. Unless otherwise stated, benefits are effective the 1st of the
next month following employment. With certain benefits, employees may elect to cover eligible
dependents. For additional information regarding eligibility, please refer to the Summary Plan
Description for each respective benefit plan.

The following are highlights of the benefits and work/life programs we currently offer, designed to be
flexible, competitive and responsive to your needs, personal responsibilities and interests.

Medical Plan

Medical coverage for employees of Long Term Care Partners (LTCP) is provided through Anthem. We
offer two plans to choose from: Harvard Pilgrim HMO or Harvard Pilgrim PPO. Both plans provide
coverage for a wide range of hospital, surgical and other covered medical expenses. They also offer
prescription drug benefits. The majority of the premiums for this benefit are paid for by LTCP.

Dental Plan

LTCP offers a dental plan that covers preventative dental care, as well as major dental bills, including
orthodontics. Delta Dental offers both a network and a non-network option for your dental expenses.
The majority of the premiums for this benefit are paid for by LTCP.
401(k)

Eligible employees may participate in LTCP’s 401(k) plan immediately following commencement of
employment. The plan offers a diversified mix of investment options through Fidelity. Participants save
pre-tax dollars up to the limits established by the Internal Revenue Service, and LTCP matches
contributions dollar-for-dollar up to 6% of eligible pay, with 100% immediate vesting.

Life Insurance

Eligible employees receive basic life and Accidental Death and Dismemberment insurance coverage of 1
times annual base pay. LTCP pays the full cost of this coverage. In addition, employees may purchase
additional supplemental life insurance in the amount of 1x, 2x, or 3x annual base pay. This coverage is
underwritten and administered by MetLife.

Disability Insurance

LTCP provides Short- and Long-Term Disability coverage at no cost to eligible employees. The Long-
Term Disability insurance is underwritten and administered by MetLife coverage to start 1st of the month
following 90 days of employment.

Long-Term Care Insurance

Eligible employees and their eligible dependents have the opportunity to purchase Long-Term Care
Insurance through John Hancock.
Flexible Spending Accounts

Eligible employees may elect to set aside money on a pre-tax basis in a health care ($3,000) and/or
dependent care ($5,000) employee spending account. These accounts can be used to pay for eligible
unreimbursed health care or day care expenses.

Tuition Reimbursement Program

Eligible associates may receive up to $5,250 annually to pay for college-credited, pre-approved courses
after 6 months of employment.

Vacation

Full-time employees accrue 3 weeks vacation per year.

Sick/Personal/Family Care

Full-time employees are eligible to take up to 5 days per calendar year of Sick/Personal/Family Care,
pro-rated for new hires.

Holidays

Work/Personal Life Programs

All employees may utilize the Employee Assistance Program (EAP). The program provides employees
with work/life resources and referrals to assist employees and their families with information on a wide
range of child care, elder care, education and other work/personal life issues.
