     Eligibility – Annual Program
     Employees currently enrolled in the KM UHC Plan and new hires enrolled in the Plan
     prior to July 1st. If employee has a dependent spouse/DP enrolled, both members
     must complete the HRA & 5-week online Coaching Support Program.

     Requirements
     Obtain your biometrics – Know Your Numbers (January – March)

     Complete the online University of Michigan HRA via www.myuhc.com (March – May)

     Complete 5-week NCQA Certified evidenced-based Program as determined by the
     member's UHC Personal Health Profile/HRA results (June – August)

     Drawings
     Complete the HRA, employee will be entered into drawing to win 1 of 4 Nintendo
     Wii Sport Machines. (June)

     Complete the 5-week online coaching program, employee entered into drawing for
     a BRAND NEW FORD TRUCK!!! (October)

             2.   If you already registered, log into your account with your user
