The Jefferson County Fair Park is located in the city of Jefferson, just 6 miles south of the junction of I-94 and Hwy 26. Take I-94 to Hwy 26 (exit 267); go
 south on Hwy 26 approximately 6 miles to the first stop light. This is the junction of Puerner St (Hwy N) and Hwy 26. Turn right onto Puerner St. and go
                                                    approximately ¾ mile directly to the Fair Park gates.

Please read the following before completing entry forms.
Entries must be on the official entry form of the show. If additional entry forms are needed, either photocopy the form included herein, contact the show manager or secretary, or
visit the WAHA website (www.wisconsinarabian.com). All horses belonging to one owner must be entered on one entry form. No horse may be entered on more than one entry
form per show. Farm name may not be used unless the stable or farm is the bona fide owner appearing on the registration papers, or the stable or farm name as been
registered with USEF, and number is noted in the space provided. A copy of your USEF Farm Registration Certificate should accompany your entry. Every owner, trainer,
exhibitor, rider, driver, and handler must sign the entry form in the spaces provided. If the owner and trainer are the same, owner must sign in both spaces.
When Making Entries Be Sure to Include: No packets will be given if entries are not complete.
1. A copy of the registration paper showing proof of ownership and registration for each horse being entered. Any Farm or Entity owning and showing a horse must be a
       member of AHA, and that membership number must be on the entry form. For leases see AHA Handbook.
2. Negative Coggins test results from the current calendar year (2010) for all horses entered. The original results or an official health certificate with the date of the
       Coggins test must be presented at the Show Office for verification.
3. Copies of AHA membership cards of each owner, trainer, driver, rider, handler and agent or trainer or pay $35 per person AHA Single Event Membership fee. Walk Trot
       and Leadline riders are exempt.
4. Copies of USEF membership cards for each owner, lessee, rider, driver, handler and agent or trainer or pay $30 non-member fee. All junior exhibitors must have their
       own USEF membership number or pay $30. Walk Trot and Leadline riders are exempt.
5. Your amateur number where applicable. Non-USEF member amateur card $30. Junior exhibitors are considered amateurs and may enter a junior class and/or amateur
       class.
6. Dressage riders: Each competitor is responsible for supplying management with correct association numbers (USDF, USEF) for horse, owner, rider and trainer, before
       the day of the competition. The scores of competitors who fail to supply correct numbers may not be counted toward national awards.
7. A signed check made payable to WAHA August Show covering all fees. Any entries received without full payment of all fees will be considered as post-entries. No post
       dated checks.
                                                                                    GENERAL RULES
8. Physical Inspection: All animals are subject to a physical inspection at no charge by the Show Veterinarian at the discretion of the Show committee as a safeguard
       against communicable diseases. Any horse who exhibits questionable symptoms must be immediately removed from the show grounds. Examination or treatment not
       requested by show officials shall be on a private practice basis.
9. Liability: All entries are accepted with the understanding that the Wisconsin Arabian Horse Association, Region 10, and/or the Jefferson County Fair Park will not be held
       responsible for any loss, damage, or injury to any horse exhibited or any kind of article. All horses shall be under the control and direction of the Show Committee but
       solely at the risk of the exhibitor, who will be responsible for any loss, damage, or injury to any person, animal, or property occasioned by him, his agents or employees, or
       by any animal owned or exhibited by him, and shall indemnify the WAHA Board against any and all loss, including damages and liability thus occasioned, including but not
       limited to any and all legal costs, including attorney fees, which may be incurred as a result thereof. The submitting of an entry form to the Show Secretary shall constitute
       an acceptance by each person signing same of the provisions herein set forth.
10. Attendants/Grooms/Headers: No person will be permitted to enter the ring except participants in the class, judges, their assistants, and show officials. Attendants,
       grooms, and/or headers may enter the ring only when summoned by order of the judge. Headers will be permitted in all driving classes.
11. Halter Classes: Horses in breeding and in-hand classes will enter the ring in a pre-determined, randomly assigned order to be established by show management. Each
       horse must be present at the in-gate and ready to enter the ring when its number is called. Any horse not present and ready to enter the ring will not be admitted to the
       class. (Exception: Sport Horse In-Hand Entries with conflicts with main show ring)
12. Warm-Up Facilities & Competition Arenas: On Friday and Saturday the two indoor arenas will be available for warm up. On Sunday, one outdoor ring will be available
       for warm up. Arenas: Indoor 80’ x 150’; North ring 115’ x 225’; West ring 150’ x 250’ with sand footing.
13. Stallions: PB and HA stallions may be shown by ladies or junior exhibitors in any class open to purebred stallions. Half-Arabian/Anglo-Arabian stallions two years or over
       may compete in performance classes. Stallions are not permitted in walk-trot, showmanship, and leadline classes.
14. Class Size: The show committee reserves the right to split any class with duplicate awards presented.
15. Call for Classes: The two-minute rule will be strictly enforced. If time is needed for a tack change, the gate must be notified. All tack changes must be made at the ring
       entrance.
16. PA System: Due to possible unforeseen problems with any mechanical device, the PA system and class calls over that system are considered a courtesy. It is the
       exhibitor’s responsibility to have his horse at the gate at the prescribed time. While every effort will be made to maintain a working PA system, no protest will be upheld nor
       will any section be changed on the basis of a PA system problem.
17. Pets: Pets will be allowed on the grounds but must be on a leash. Pet owners are required to supply an official rabies vaccination certificate showing date of vaccination
       and serum used.
18. Electrical Connections: Must meet all current local, state, and national electrical codes. All cable must be three or four wire cable for connection to existing outlets. All
       connections must be properly grounded. Electrical cords must not run through hay, straw, or any other bedding. Cords will not be allowed in or crossing an aisle. The Show
       Manager will be advised of all violations and will notify the offender. If the violation is not remedied within four hours, or if it is the second violation, power will be cut off to
       the individual exhibitor.
19. Overnight Camping: Overnight camping facilities are available with electric and water hookup priced per night depending on the service required. Payment of the
       camping fee is made to Jefferson County Fair Park at the time of check in, and the Fair Park office will issue a camping permit that must be displayed in the window.
20. Stalls: Permanent stalls with stall fronts will be available at the show. Tack stalls may be limited at the discretion of show management. Each horse must be assigned a
       separate stall (except mares with suckling foals). Stalls will be available noon, Thursday, August 5, 2010. Early arrival permitted at an additional $20 per stall. Showing
       from a trailer is permitted. No smoking in the barns will be strictly enforced
21. Bedding and Hay: No bedding will be provided as part of the stall fee. Bedding will be available for sale at the show. No grain, pellets, or hay will be for sale.
22. Stall Assignments: Stalls are limited at the Jefferson County Fair Park. Therefore, stall assignments will be made on a first come, first served basis based on the
       following:
                a)       Members of the WAHA Show Committee.
                b)       Class sponsors paid/US government postmarked before entry closing date.
                c)       Stall orders paid in order of US government postmark date. Paid stall orders with the same postmark date will be assigned in inverse order to the number of
                         PAID stalls per order whenever possible. Show Management may exercise discretion in regard to stabling placement. Metered postmarks are not
                         acceptable.
23. Close of Entries: August show entries close July 17, 2010. Post-entries will close four classes before the one being entered.
24. No Shows: There will be a $15 penalty assessed for “no shows.” If you have entered a class and are not going to show in that class, you must notify the Show Office and
        gate before that class enters the ring or pay a $15 penalty.
25. Open Checks: No “open” checks will be accepted for pre-entry fees or stall reservations. All pre-entries and stall reservations must be accompanied by fully completed,
        negotiable checks dated on or before the close of entries date. Entries received without such checks will be returned.
26. NSF Check Policy: There is a $50 penalty for each check returned due to non-sufficient funds. In addition, either of the following may result in termination of personal
        and/or business check privileges in payment for any charges to WAHA shows or to WAHA for any other charges (including but not limited to dues and/or fees for WAHA

      programs): 1. Failure to fully and promptly pay for entries, fees, stabling, and related items (including but not limited to such things as hay, grain, camping, electrical
      hookups, services, dues, programs, or any other show expense) or for any other WAHA expenses (such as dues and/or fees for WAHA programs; and/or 2. Presenting
      two NSF checks to WAHA in any one calendar year.
27.   Hoof Length, Shoes and Pads: Management reserves the right to request measurement of toe length, shoe dimensions, and remove shoes and/or pads for hoof
      inspection of any and all horses in any class. Shoes may be pulled and inspected at random.
28.   Printing Errors: The Show Committee is not responsible for errors in printing. Corrections of any error will be posted in the Show Office.
29.   Refunds: Stall fees and office fees will not be refunded under any circumstance. Entry fees for entries cancelled before the official closing date for pre-entries will be
      returned. Refunds of entry fees will be made after the closing date for pre-entries upon presentation of a Veterinarian’s Certificate to the Show Secretary by the end of the
      last of the class of the show. No refunds will be made thereafter.
30.   Life, senior active and junior active members shall be eligible to participate in all classes at Regular Competitions, Eventing Competitions at the Preliminary
      Level or above and Combined Driving Competitions at the Advanced Level, Dressage, Reining and Vaulting Competitions and Endurance Rides. A
      nonmember may participate as a handler, rider, driver, owner, lessee, agent, coach or trainer at Regular Competitions, Eventing Competitions, Dressage
      Competitions, Reining Competitions and Combined Driving Competitions upon payment of a $30 nonmember registration fee. Participants in the following
      classes are exempted from the Requirements of this rule: 1) leadline; 2) exhibitions; 3) games and races; 4) classes for 4-H members; 5) walk trot and
      academy classes (academy classes are classes limited to horses used regularly in a lesson program); 6) USDF introductory level tests, pas de deux and
      quadrille classes; 7) NRHA Endorsed Reining Competitions. 8) Opportunity classes, 9) citizens of other nations who have proof, in English, of current
      membership in good standing of their own National Federation, 10) USEA beginner novice division; and 11) assistant handlers in Dressage Sport Horse
      Breeding classes. EC 7/21/08 Effective 12/1/08
31.   Minors who do not have a valid driver’s license which allows them to operate a motorized vehicle in the state in which they reside will not be permitted to operate a
      motorized vehicle of any kind including, but not limited to, golf carts, motorcycles, scooters, or farm utility vehicles on the competition grounds of licensed competitions.
      Minors who have a valid temporary license may operate the above-described motorized vehicles as long as an adult with a valid driver’s license accompanies them.
      Violations of this rule will be cause for sanctions against the parent(s), guardian(s) and/or trainer(s) who are responsible for the child committing the offense. Penalties
      may include exclusion of the child, parent(s), guardian(s), and/or trainer(s) from the competition grounds for the remainder of the competition and charges being filed
      against any of the above individuals in accordance with Chapter 6. Wheelchairs and other mobility assistance devices for individuals with disabilities are exempt from this
      rule. (USEF GR 901.29)
                                                                          DOVER MEDAL/USDF RULES:

      The USDF/Dover Saddlery Adult Amateur Medal Program is designed specifically to recognize and encourage adult amateurs riding at Second Level. 1. Dover Medal
      Class Winner .The eligible rider with the highest score of 58% or better in the designated Second Level Test Three Class or applicable test of choice USDF/Dover Adult
      Medal class at a participating competition will receive a handcrafted USDF/Dover pewter medal and neck ribbon. Winners will receive a $20 Dover Saddlery Gift
      Certificate for each of the first three USDF/Dover Adult Medals they win during the 2009 competition season. 2. The USDF/Dover National Merit will be awarded to riders
      winning three USDF/Dover Medals in the same competition year. 3. Year End Awards The USDF/Dover Medal National Champion and Reserve Champion are
      determined by the average of the top three winning ride scores during the competition year. The Champion receives a $1000 Dover Saddlery gift certificate and a Jaguar
      dressage saddle. The Reserve Champion receives a $500 Dover Saddlery gift certificate and a Jaguar dressage bridle. Rider Eligibility
      1. Rider must be a current USDF group, participating, or intercollegiate/interscholastic member at the time of the competition.
      2. Rider must ride Second Level Test Three.
      3. Rider must be an adult amateur in accordance with Article GR808 of the USEF rules and be registered with USEF as an adult amateur as of the competition date.

                                                              WAHA MEMBER JACKPOT RULES
      What’s Happening??
      In 2009, the WAHA Board replaced the former WAHA Futurity/Maturity program with a WAHA Member Jackpot program including the same classes that
      had been in the Futurity/Maturity program. For 2010, the same Halter classes remain, but the WAHA Board combined the performance age group
      classes to increase the size of classes and increase the amount of money paid back to each placing. The categories are Western Pleasure, Hunter
      Pleasure, and Country Pleasure with four classes in each: a Purebred Open class and a Purebred Amateur class, a Half/Anglo Arabian Open class and
      a Half/Anglo Arabian Amateur Class. So amateurs still have two chances to show for big paybacks.

      To meet the “WAHA MEMBER” requirement, the horse must be owned by a full voting WAHA member. But the rider/driver/handler does not have to
      be a WAHA member. Not a member yet? No problem! You can join at the show. Or send in the form on page 22 with your entries.

      This great new program has several cost-saving advantages for exhibitors. There are no early nominations, no “catch up” fees, less ongoing paperwork
      and confusion. Just pay the jackpot entry fee of $40 whenever you enter, and enjoy! There is no post-entry fee for the WAHA Member Jackpot
      classes.

      How does the payback work??

      Each Jackpot class costs just $40 per horse to enter, with no additional cost for post entry. Each class pays back money through 4th place. “Allocated
      Money” is all entry fees minus trophy and ribbon expenses for that class. One (1) horse: 100% of allocated money Two (2) horses: 60%, 40% Three (3)
      horses: 50%, 30%, 20% Four (4) horses: 40%, 30%, 20%, 10% Payback checks will be mailed out to the recorded owner of a horse as soon as
      possible after the show ends.

                                                          Option Classes, WAHA August Show

                                           Option Class purchase fee is $40 and includes one entry for that class
An Option Class may be requested by submitting the Option Class Request Form below, or call or fax your request. Select two of the Option Class slots for
your class. After the Option Classes are posted, anyone may enter the classes at the entry fee of $15. Post entry fee does not apply. Management
reserves the right to select final placement in the class schedule for requested option classes. Arabian Breeders Sweepstakes points cannot be earned from
any option class. Qualifications points for Regionals and Nationals can be earned. Championship classes requested as Option Classes must have one or
more qualifying classes in order to be approved. You may purchase a qualifying class and a championship by requesting two Option Classes. Championship
Option Classes will not payback.

Consult the prize list before selecting an option. Example: A class already in the prize lists may not be selected as an Option Class.

Mid-America Arabian Breeders Futurity Rules
Auction Futurity for Purebreds
The Stallion Service Auction will be held annually. Members will donate the service of their stallion to be auctioned off to the highest bidder. The resulting foal
is eligible to be shown in the Auction Futurity Class as a yearling. 50% of the total auction proceeds are paid back to the Top Five winners in both the yearling
colt/gelding and yearling filly classes.

Only 25% of the final auction bid is due on the day of the Auction. The balance of the auction service is due on or before February 1 of the year the mare is to
be bred. There is a 10% discount on the winning auction bid if it is paid in full on the day of the auction.

Auction nominated mares must be bred in the year following the auction. However, if the mare fails to settle or if the breeding does not result in a live foal, you
have a live foal guarantee. The mare or a substitute mare may be bred the following year and be nominated for the Auction Futurity. First year extensions
approved by the Board shall be free of charge. Any extensions granted beyond the first year will cost $250 per extension per year. Extensions require formal
action and approval by the MAAB Board of Directors. A written request must be sent in by the mare owner to the Futurity Secretary.

* Stallion Service Auction fees can only be refunded in the event that the nominated stallion dies or suffers injury or infertility.
* All futurity entries must be shown by an amateur who has a USEF Amateur Card.
* The successful bidder must nominate the mare bred with the stallion service purchased at the auction no later than December 31 of the year the mare is
bred. Futurity forms are available from MAAB stallion owners, the Futurity Secretary, and at www.maab.org.

Since this is a futurity program, all financial commitments must be met in full at the stipulated time or the mare owner loses eligibility and the
resulting foal will not be allowed to compete in the Futurity.

Paybacks are as follows: 1st - 35%, 2nd - 25%, 3rd, 17.5%, 4th - 12.5%, 5th - 10%

Select Futurity for Purebreds
A mare owner may nominate a mare to the MAAB Select Futurity if she is bred to a stallion who was nominated to MAAB on or after July 1 of the year previous
to said breeding. For example, if a stallion is nominated on July 1 of 2009, any purebred Arabian mare bred to him in 2010 is eligible to be nominated for the
Select Futurity.

The nomination requires a signed nomination form and a $50 fee payable to MAAB no later than December 31 of the year the mare was bred. The $50 fee is
non-refundable. The resulting foal will be eligible for the Select Futurity in its yearling year, provided it has been registered and issued a MAAB Certificate.

The Mid-America Select Futurity pays back to the Top Ten yearlings in the colt/gelding class and to the Top Ten yearlings in the filly class. Prize money is
determined by the stallion service auction proceeds and mare nominations. By rule, 25% of amounts to $20,000 raised at the auction go to the Select Futurity.
After $20,000 has been raised at the auction, 15% of the additional amount raised will go to the Select Futurity. The actual size of the futurity payback can be
determined only after February 1 of each year. The yearling Select Futurity classes are held annually.

These classes produce outstanding competition and can generate large payouts. For these reasons, it is important mare owners remember that they must
have their mare nomination postmarked no later than December 31 of the year the mare is bred. Only amateurs that have USEF Amateur cards are
allowed to show futurity entries.

Paybacks are as follows: 1st - 20%, 2nd - 17.5%, 3rd - 15%, 4th - 12.5%,5th - 10%, 6th-10th - 5% each.

There is also a Jackpot class for two-year-old colts/geldings and two-year-old fillies that have been previously nominated to the Auction and/or Select Futurity.
Placings are made to 5th place in these classes.

Performance Futurity for Purebreds
Performance Futurity classes are held in the Hunter Pleasure, Western Pleasure, English Pleasure, Country English Pleasure and Driving divisions. Mares
bred to a stallion nominated on or after July 1 of the previous year may be nominated for the performance futurity. The fee for a nomination made by
December 31 of the breeding year is $100. The fee for foals of currently nominated sires during their weanling year is $150. Thereafter, horses sired by
stallions on the approved MAAB list may be nominated for a fee of $350.

Only Amateurs with USEF Amateur cards may show horses in the performance Futurity. Any horse winning a specific class three times will be recognized and
thereafter becomes ineligible for that particular class.

A horse nominated to the Performance Futurity program remains eligible for life regardless of sale or owner transfer. In order to show, the nominated horse
must have been issued a MAAB Certificate under the current owner. There is a $20 fee for transfer of ownership.

No cross entry is permitted between English Pleasure and Country English Pleasure classes. Pleasure Driving is an open class in this futurity, meaning that if
the horse is ridden in any other division, it is permitted to show in the driving class without restriction of cross entry.

Payback consists of all performance nomination fees, 25% of auction proceeds, up to $20,000 (35% thereafter) and the class entry fees. Payouts are as
follows: 1st - 50%, 2nd 25%, 3rd - 10%, 4th 7.5%, 5th - 7.5%

Class will be judged according to division specifications as per USEF/AHA rules.

Mid-America Half-Arabian Futurity Program

The Half-Arabian program will duplicate the existing program for purebred Arabians, with the exception that the Auction and Select Yearling classes and the
Two-Year-Old Jackpot classes will be shown by conformation type, not gender.

* Eligible horses must be sired by Mid-America nominated purebred Arabian stallions.
* Auction foals will result from the winning bids at the annual Stallion Service Auction for Half-Arabian breedings.
* Select foals will result from breeding to currently nominated MAAB stallions whose services were offered in the Half-Arabian Stallion Service Auction.
* Performance horses will result from horses sired by stallions on the current approved sires list.

Auction and Select Futurity Classes for Half-Arabians
Auction and Select Yearling Classes for Half-Arabians will be shown by conformation type, not gender. Colts and fillies will compete against each other in
either Saddle type or Stock type conformation classes. The assignment to either class will be designated by the mare’s foundation breed type: If the mare is a
registered Saddlebred, NSH, Friesian, Tennessee Walker, Dutch Harness Horse, or any other gaited breed, the foal will be placed in the Saddle type division.
If the mare is a Quarter Horse, Paint Horse, Morgan, Appaloosa, or any other Stock breed, the foal will be placed in the Stock Type division. If the mare is
unregistered, the MAAB Board of Directors, upon submission of conformation photos, will determine the conformation type. The Board’s decision will be final.
These Saddle Type and Stock Type division designations apply only to the Auction, Select, and Two-Year-Old Jackpot classes. They do not apply to the
Performance classes.

Stallion Service Auction - Stallion Owner Participation
Owners of purebred Arabian stallions nominated to the Mid-America Arabian Breeders Futurity may opt to participate in either the purebred or Half-Arabian
Auction or both. The same rules apply as in the past: Stallion owners guarantee $500 or half the advertised stud fee for a purebred breeding; half the
advertised stud fee for a Half-Arabian breeding. Whichever service is sold at auction determines which foals may be nominated to which futurity; I.e., the
service sold for purebreds entitles only purebred foals to be nominated; the service sold for Half-Arabians entitles only Half-Arabian foals to be nominated.
If a stallion owner does not designate a different service fee for Half-Arabians in the advertised stud fees, then FOR THE PURPOSES OF THE MAAB
PROGRAM ONLY, the fee for Half-Arabians is assumed to be half the advertised fee. The stallion owner then guarantees half of that amount for the Half-
Arabian Stallion Service Auction.

Performance Futurity for Half -Arabians
Nomination is for the lifetime of the horse.

Membership Requirements for USDF-recognized/USEF-licensed Competitions: Owners                            by USDF for
and riders, including foreign riders and owners who are not residents of the US, wishing to                   2010
participate in a USDF-recognized/USEF licensed competitions as a rider or owner/lessee, must
have either a USDF group membership (GM), intercollegiate/interscholastic membership (IM),
participating membership (PM), or have a USDF non-member (NM) identification number and pay the USDF NM fee.
(Note: USDF business members (BM) can own horses but cannot compete as riders with their BM.)
Horses competing at USDF-recognized/USEF-licensed shows must have either a USDF horse identification (HID)
number or a USDF lifetime horse registration (LHR). Proper credentials or verification certificates must be presented
to the competition secretary.
Riders and owners wishing to participate in a USDF recognized competition, and not able to present the competition
secretary with verification of a USDF membership or NM identification and USDF LHR or USDF HID, must complete a
USDF Affidavit Form and pay the $5 affidavit-filing fee. A copy of a completed affidavit form from another competition
may also be used for verification purposes for up to 60 days. (Exception: Affidavits cannot be used at Regional
Championships.
Exceptions:
• Horses competing only in Individual Breed Classes (IBC) at DSHB competitions, horses competing only in breed
restricted dressage or DSHB classes at Regular Competitions (e.g. all Arabian, all Friesian, or all Morgan classes), or
in USDF Introductory Level tests, Pas de Deux, Quadrille, or FEI ParaEquestrian classes, are exempt from this
requirement. Also exempt are horses ridden in leadline, exhibitions, games and races, classes for 4-H members,
walk-trot and academy classes. For foreign-owned horses competing in a CDI, USDF accepts copies of the horse
identification pages from an FEI passport in lieu of a USDF HID or Lifetime Horse Registration (LHR) number.
• Riders and owners/lessees competing only in classes which are exempt from the USDF HID requirements are also
exempt from the USDF NM identification number and NM fee requirements. Also exempt are Handlers of horses
competing in DSHB In-Hand or Group classes.
                                                  Memberships Defined
Participating Membership (PM): A membership issued to an individual directly by USDF. The membership year is
December 1 through November 30. PMs are available for 1-year ($62), 5-years ($250), or life ($1,000). A USDF
Youth Participating Membership (YPM) is available for members under 21 (as of December 1 of the current
membership year) for $35. A PM allows the member to compete at USDF recognized/USEF-licensed competitions
and to participate as a rider and/or owner for all Adequan®/USDF Year-End Awards, Great American Insurance
Group/USDF Breeders’ Championship Series Finals, and the Great American Insurance Group/USDF Regional
Dressage Championship program.
Group Membership (GM): A membership issued to an individual by USDF when the member name and dues are
submitted to USDF through an affiliate Group Member Organization (GMO). The GM year begins December 1 and
ends November 30. A GM allows the member to compete at USDF-recognized/USEF-licensed competitions and
participate in the USDF Rider Awards program.
Intercollegiate/Interscholastic Membership (IM): A membership issued to an individual by USDF when the member
name and dues are submitted to USDF through an affiliate Intercollegiate/Interscholastic Member Organization (IMO).
The membership year is December 1 through November 30. An IM allows the member to compete at USDF-
recognized/USEF-licensed competitions, and participate in the USDF Rider Awards program.
Business Membership (BM): A membership that runs December 1 through November 30 and is issued to a
business or organization directly by USDF. The USDF BM ($175) offers a wider variety of advertising benefits. BMs
satisfy requirements of horse ownership only.
Non-Member (NM) Identification Number: For each USDF non-member owner/lessee and USDF non-member rider
there will be a USDF non-member fee of $20 and a USDF non-member form per competition that must be collected
by the show secretary and submitted to USDF along with the USDF Report of Fees document and the $5 affidavit fee
if the person cannot provide a copy of their USDF NM card. Owners/lessees or riders exempt from the non-member
number requirements are also exempt from paying the USDF non-member fee. If all the
owners of the horse are non-members, only one of the owners must pay the non-member fee. If the horse has at least
one owner who is a current USDF member there is no non-member fee assessed. Competition management will be
invoiced for all applicable USDF non-member fees not received with the USDF post competition paperwork, along
with a report listing all USDF non-members. Competition management is then required to submit the non-member
fees within 30 days to the USDF. An individual with a USDF NM identification number is not eligible for any USDF
membership benefits. Once a USDF NM number has been assigned, a USDF NM card can be obtained from the
USDF Web site.
Horse Identification (HID) Number: A one-time tracking number for horses ($20). Scores earned by a horse with an
HID will be recorded, but will NOT be counted towards Adequan®/USDF Year-End Awards. A horse with an HID is
not eligible to compete in Great American Insurance Group/USDF Regional qualifying or championship classes or
Great American Insurance Group/USDF Breeders’ Championship Series Finals. An HID can be upgraded to a
Lifetime Horse Registration (LHR) for a fee of $65. If a horse has a USDF LHR it does not need an HID. An HID does
not need to be renewed.
Lifetime Horse Registration (LHR): A horse with a LHR ($85) is eligible for Adequan®/USDF Year-End Awards, and
eligible to compete at Great American Insurance Group/USDF Breeders’ Championship Series Finals and Great
American Insurance Group/USDF Regional qualifying and championship classes, assuming requirements for the
owner and rider are met. If a horse has a LHR it does not need an HID. A LHR does not need to be renewed.

Applications for participating membership (PM), business membership (BM), non-member (NM) identification number,
horse identification (HID) number, and lifetime horse registration (LHR) may be found in the following locations:
• On the USDF Web site: www.usdf.org. You can submit the application online, print, fax or mail it.
• In the show office at USDF-recognized/USEF-licensed competitions classified by USEF as Dressage Competitions,
and Regular Competitions which are open to all breeds.

The effective date of membership is the date the form and correct fees are received in the USDF office or, if
applying at a competition, the date received by the competition secretary.

AHA ENTRY AGREEMENT – Regional and Local Show
I have read the rules concerning competitions as printed in the Arabian Horse Association (AHA) Handbook and Directory and agree to be bound by and subject to those Rules.
    AHA ASSUMPTION OF RISK, RELEASE AND INDEMNIFICATION
    This document waives very important legal rights. Read it carefully before signing.
In consideration for AHA permitting me to participate in this Competition, and by signing the entry blank, I agree as follows:
    I AGREE that I choose to participate voluntarily in this Competition, as a rider, driver, handler, lessee, owner, agent, coach, trainer, junior exhibitor, or as a parent or guardian of a junior exhibitor. I AM FULLY
AWARE AND ACKNOWLEDGE THAT HORSE SPORTS AND PARTICIPATION IN THIS COMPETITION INVOLVE SERIOUS RISK OF HARM INCLUDING, BUT NOT LIMITED TO, RISKS OF ACCIDENT,
SERIOUS BODILY INJURY, INCLUDING DEATH, BROKEN BONES, HEAD INJURIES, TRAUMA, PAIN, AND SUFFERING, AND PROPERTY DAMAGE. I ASSUME ALL RISKS OF HARM TO ME, MY
HORSE OR MY PROPERTY.
    I AGREE for myself, my heirs, executors, administrators, successors and assigns to release AHA, the Competition, the facilities leased by the Competition and the owner(s) of the facilities, and all of their
respective officers, officials, directors, employees, agents, personnel, volunteers, affiliated organizations and insurers (collectively, the "Released Parties") from any and all claims for damage, loss, or injury to
myself, other persons, horses or other property belonging to me to the fullest extent permitted by law that arises out of or relates in any way to the Competition and my participation in the Competition
INCLUDING, BUT NOT LIMITED TO, DAMAGES, LOSS, OR INJURY RESULTING FROM ANY ACTS, FAILURE TO ACT, NEGLIGENCE OR NEGLECT OF OTHER ENTRANTS, THE RELEASED PARTIES,
THEIR CONTRACTORS OR INVITEES, as well as for theft, vandalism, fire, other casualty damage, or damage arising out of any defects in the premises.
    I AGREE to indemnify and hold harmless (that is pay all losses, damages, attorneys fees and costs of) the Released Parties from and against any and all claims, demands, penalties, actions, losses, costs,
damages, injuries, liabilities and obligations (including attorneys fees) of whatsoever kind and nature, which may be asserted against or incurred by any of them as a result of (1) my participation in the Competition
or (2) any act, failure to act, or neglect (a) by me, my agents, employees, riders, handlers, trainers, coaches, drivers, contractors or invitees, or (b) by any animal owned or exhibited by me or in my custody or
control.
    I AGREE and represent that I am qualified and eligible to enter and/or participate in the Competition, and every horse I am entering is qualified and eligible as entered.
    I AGREE to accept AS FINAL any decision of AHA, the Show Commission or Show Officials concerning my qualification or the qualification of my horse to enter the Competition or any results of the
Competition, except to the extent that the Rules of AHA, the Competition, Equine Canada or U.S. Equestrian Federation permit a protest or hearing of such decisions. Should a hearing be requested, I agree to
accept AS FINAL the decision of the particular hearing body. I agree to release, hold harmless and not to sue AHA, the Competition Sponsor, their officers, directors, employees, volunteers or members
concerning any decision of AHA, the Competition, its Show Commission, Show Officials or any hearing body that relates to my qualifications or my horse(s)’ qualifications to enter the Competition or any results of
the Competition.
    I AGREE that AHA has the sole right to control, sell, supervise or give away (or assign to others the right to do so) the exclusive rights to broadcast, televise, reproduce, transmit and disseminate all or part of
this event, and I agree that AHA may use or assign, in any way AHA sees fit, photographs, films, videos, audios, cablecasts, or other likenesses of me and my horse taken during the course of the Competition for
the promotion, coverage or benefit of the Competition or AHA. Those likenesses shall not be used to advertise a product and they may not be used in such a way which implies endorsement of any company,
product, product category or service. I hereby expressly and irrevocably waive and release any rights in connection with such use, including any claim to compensation, invasion of privacy, right of publicity, or to
misappropriation.
    By signing below as a parent or guardian of a junior exhibitor, I consent to the child’s participation and agree to all of the above provisions, and further agree to assume all of the obligations of this AHA
Assumption of Risk, Release and Indemnification personally and on behalf of the child.
This AHA Assumption of Risk, Release and Indemnification is governed by the Laws of the State of Colorado and is intended to be interpreted as broadly as possible. I agree that exclusive jurisdiction and
venue (place) for any legal action against AHA, its officers, directors, employees, volunteers or agents shall be in the local district courts or the federal court of the State of Colorado. If any part of this agreement
is determined to be unenforceable, all other parts shall remain effect.
Federation Entry Agreement
By entering a Federation-licensed Competition and signing this entry blank as the Owner, Lessee, Trainer, Manager, Agent, Coach, Driver, Rider, Handler, Vaulter or Longeur and on behalf of myself and my
principals, representatives, employees and agents, I agree that I am subject to the Bylaws and Rules of The United States Equestrian Federation, Inc. (the “Federation”) and the local rules of the competition. I
agree to be bound by the Bylaws and Rules of the Federation and the competition. I will accept as final the decision of the Hearing Committee on any question arising under the Rules, and agree to release and
hold harmless the competition, the Federation, their officials, directors and employees for any action taken under the Rules. I represent that I am eligible to enter and/or participate under the rules, and every horse
I am entering is eligible as entered. I also agree that as a condition of and in consideration of acceptance of entry, the Federation and/or the competition may use or assign photographs, videos, audios, cable-
casts, or other likenesses of me and my horse taken during the course of the competition for the promotion, coverage or benefit of the competition, sport, or the Federation. Those likenesses shall not be used to
advertise a product and they may not be used in such a way as to jeopardize amateur status. I hereby expressly and irrevocably waive and release any rights in connection with such use, including any claim to
compensation, invasion of privacy, right of publicity, or to misappropriation. The construction and application of Federation rules are governed by the State of New York, and any action instituted against the
Federation must be filed in New York State. See GR908.4.
Federation Release, Assumption of Risk, Waiver, and Indemnification

    I AGREE in consideration for my participation in this Competition Wisconsin Arabian Horse Association and Mid-America Arabian Breeders to the following:
    I AGREE that I choose to participate voluntarily in the Competition with my horse, as a rider, driver, handler, vaulter, longeur, lessee, owner, agent, coach, trainer, or as parent or guardian of a junior exhibitor. I
am fully aware and acknowledge that horse sports and the Competition involve inherent dangerous risks of accident, loss, and serious bodily injury including broken bones, head injuries, trauma, pain, suffering, or
death (“Harm”).
    I AGREE to release the Federation and the Competition from all claims for money damages or otherwise for any Harm to me or my horse and for any Harm caused by me or my horse to others, even if the
Harm resulted, directly or indirectly, from the negligence of the Federation or the Competition.
    I AGREE to expressly assume all risks of Harm to me or my horse, including Harm resulting from the negligence of the Federation or the Competition.
    I AGREE to indemnify (that is, to pay any losses, damages, or costs incurred by) the Federation and the Competition and to hold them harmless with respect to claims for Harm to me or my horse, and for
claims made by others for any Harm caused by me or my horse at the Competition. I have read the Federation Rules about protective equipment, including GR801 and EV113, and I understand that I am entitled
to wear protective equipment without penalty, and I acknowledge that the Federation strongly encourages me to do so while WARNING that no protective equipment can guard against all injuries.
    If I am a parent or guardian of a junior exhibitor, I consent to the child’s participation and AGREE to all of the above provisions and AGREE to assume all of the obligations of this Release on the child’s behalf.
    I AGREE that “the Federation” and “Competition” as used above includes all of their officials, officers, directors, employees, agents, personnel, volunteers and affiliated organizations.
    I represent that I have the requisite training, coaching and abilities to safely compete in this competition.
    I AGREE that if I am injured at this competition, the medical personnel treating my injuries may provide information on my injury and treatment to the Federation on the official USEF accident/injury report form.
BY SIGNING BELOW, I AGREE to be bound by all applicable Federation Rules and all terms and provisions of this entry blank.

Membership: (includes club affiliation; does not include competition privileges or Excess Personal
Liability Insurance; see Competition Cards). Select One:
 Adult - $40 – Includes AHA membership for one year; 1 vote
 Adult 3 Year - $115 – Includes AHA membership for three years; 1 vote
 Youth (17 or under as of 12/01/09) – $25 – Includes AHYA membership for one year; 1 vote
 Parent/Child Discount - $55 – Includes adult and youth AHA membership for one year; 2 votes
    Each Additional Child Discount - $20 – Includes youth AHA membership for one year; 1 vote
 Family - $40 – Includes adult AHA membership for one year; no AHA youth membership; 1 vote
 Married Couple - $70 – Includes two adult AHA memberships for one year; 2 votes
 Associate - $30 – Adult; no AHA membership; no vote; not eligible for Competition Card
Competition Card: (required to compete in or officiate in AHA recognized events; includes $1 million
Excess Personal Liability Insurance for US residents. Not required for AHA Incentive Riding Programs)
 Adult – $35
 Adult 3 Year – $105 (one adult)
 Youth (17 or under as of 12/01/09) – $25
 Family – $35 (one adult)
 Married Couple – $70 (two adults)

I agree to abide by the Constitution and Bylaws of the Wisconsin Arabian Horse Association (WAHA) and
to uphold them. I fully understand that my application will be kept on file and is subject to review by
WAHA and that any false statements or information contained in my application will be just cause for
immediate termination of my membership. New memberships are not effective until approved by the
WAHA Board of Directors.
