                (These prices are effective April 30, 2009 and are subject to change without notice)
The goods and services shown below are those we can provide to our customers. You may choose only the items you desire.
However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other
requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the
statement we provide describing the funeral goods and services you selected.
                                                       ITEMIZED SERVICES
Basic Services of Funeral Director and Staff                                                                             $890.
 This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee
 is already included in our charges for direct cremation, anatomical donation, immediate burial, and forwarding or receiving
 remains.) Our basic services include: coordinating the arrangements conference with the family or Funeral Director,
 coordination of services and plans, 24 hour availability of personnel, general clerical administration, preparation and
 placement for local newspaper obituaries, coordination with the cemetery, crematory, airlines and/or other third parties
 involved and recording any authorizations and permits as necessary.
Preparation of the Remains
Embalming                                                                                                             $490.
  Except in certain cases, embalming is not required by state law. Embalming may be necessary, if you select certain funeral
  arrangements, such as funeral with viewing. If you do not want embalming, you usually have the right to choose an
  arrangement that does not require you to pay for it, such as direct cremation or immediate burial. Dressing, Cosmetics and
  Casketing is included in our Embalming Fee.
Additional preparation care from Autopsy/Organ Donation cases                                                          $125.
Other Preparation of Remains
  Cleansing, setting facial features, dressing of unembalmed remains for private ID and/or final disposition           $125.
  Dressing, Cosmetizing and/or Casketing of Remains Transferred-in                                                    $125.
  Refrigeration (When embalming is not requested or when authorization to embalm has not been obtained                 $225.

  Use of Facilities and staff for gathering/viewing per night 9:00 PM.–10:00 AM                                         $650.

SPECIAL CHARGES
Forwarding Of Remains to Another Funeral Home                                Within U.S.A. $1195.            Outside U.S.A. $1495.
This charge includes local transfer of remains from place of death, basic services of funeral director and staff, embalming, air
tray or shipping container (not a casket), local courier vehicle, local transport to Tampa Int’l Airport. This charge does not
include any use of staff and facilities for visitation/viewing or service prior to forwarding of remains, casket, other merchandise,
shipping charges, medical examiner’s permit fee, international consulate fees or translation charges, overnight courier charges
and other cash advances.
Receiving Remains From Another Funeral Home                                                                               $1535.
This charge includes transport from Tampa Int’l Airport to funeral home, basic services of funeral director and staff, Hearse and
service vehicle to local cemetery. This charge does not include the use of facilities and staff for any visitation/viewing or
service, public or private, flower car casket or other merchandise, family transportation and cash advances.
Immediate Burial                                                                                                          $1830.
This charge includes basic services of funeral director and staff, local transfer of remains from place of death, use of
refrigeration facility, local courier vehicle, hearse & service vehicle to local cemetery. It does not include embalming, funeral
director and staff for visitation/viewing or funeral ceremony, merchandise, cash advances or cemetery fees.
Anatomical Donation                                                                                                         $1695.
This charge includes basic service of staff, embalming, local transfer of remains from place of death, local courier vehicle, and
transportation to the Anatomical Board at the University of Florida, Gainesville, Florida. This charge does not include any
merchandise, use of facilities and staff for visitation/viewing or service; local medical examiner’s permits fee, university fee (if
applicable) and any cash advances.
DIRECT CREMATION for remains up to 400 lbs (if over 400 lbs add $200.)
This charge for a direct cremation includes local transfer of remains from place of death, use of refrigeration facility, basic
services of funeral director and staff, local courier vehicle, local transportation to crematory and the cremation fee. It does not
include embalming, use of facilities and staff for visitation/viewing or service; local medical examiners permit fee, merchandise
and any cash advances. If you want to arrange a direct cremation, you can use an alternative container. Alternative containers
encase the remains and can be made of materials like cardboard (with or without an outside covering or inside lining) or wood
materials. The containers we provide are made of cardboard or composite wood materials.
