MINNESOTA STATE COLLEGES AND UNIVERSITIES Instructions to College/University Preparation of Division 0 Documents Division Zero is the first section of the specifications for construction. For each advertised bid project, the College/University must direct the Architect/Engineer to the following documents to be included in Division Zero for inclusion in the Bidding Document’s Project Manual. Modifications (insertions) needed for each document are listed below. SECTION 00 01 10 TABLE OF CONTENTS Table of Contents for Division Zero to be modified by the Architect/Engineer. SECTION 00 11 130 1. Sealed Bids for: ADVERTISEMENT FOR BIDS Insert the Project Name, Building Name (if applicable), Name of College/University, County, City, Minnesota. Roof Replacement Fine Arts Building Fergus Falls Community College Otter Tail County Fergus Falls, Minnesota Insert the Name of the person in charge of receiving the bids, the exact location Name of Building and Room Number where bids are to be delivered, Name of College/University, City, Minnesota, Zip. John Doe, Director of Purchasing Wiecking Center, Conference Room 221 Minnesota State University Mankato Mankato, Minnesota 55602 EXAMPLE: 2. Received By: EXAMPLE: 4. Bid Opening Time and Date: Insert the Time and Date of the bid opening. EXAMPLE: Until 2:00 a.m., local time, July 19, 2006, at which time proposals will be open and publicly read aloud. Include an overall description of the scope and type of Work included in the Project. For projects over $1 million, include more information about the project specifics and elements. Project Scope: The Project consists of the remodeling of an existing area to create a new science laboratory. The Project will include approximately 1,500 gross square feet located on the third floor of Building C at the Minneapolis Community Page 2 of 7 5. Project Scope: EXAMPLE: (<$1M) Rev. 4/1/09 MINNESOTA STATE COLLEGES AND UNIVERSITIES Instructions to College/University Preparation of Division 0 Documents and Technical College. The Work will include, but is not limited to: minor demolition of masonry partitions, ceilings, and related mechanical and electrical systems; construction of new masonry partitions, doors, frames, hardware, suspended ceilings, specialities and finishes; mechanical systems including plumbing, heating, ventilating and air conditioning; and electrical systems including power, lighting and special systems. EXAMPLE: (> $1M) Project Scope: The Project consists of the remodeling of approximately 69,000 GSF of existing spaces at several locations on the campus. The existing gymnasium will be converted into two levels by adding a new intermediate floor to provide additional space. All building components and systems in the gymnasium vicinity will be upgraded. The Project includes renovation of some other spaces in the adjacent building complex after programs move into the Health Sciences area. The separate Heintz Center building located at the northwest corner of the campus will also be remodeled for a community dental clinic, a community primary care clinic, classrooms and support areas. The Project also includes infrastructure and site improvements adjacent to the remodeled gymnasium/Health Sciences building. This will include reconfiguration of the northwest campus entrance drive and parking areas, a main campus west entry and drop-off, improved traffic and pedestrian circulation, upgraded site utilities and site lighting, building service delivery entrance, plazas, walkways, an outdoor amphitheater and related landscaping improvements. The Work includes, but is not limited to: demolition, earthwork, concrete, masonry, structural and misc. steel, waterproofing, roofing, frames, doors, curtainwall, steel studs and drywall, suspended ceiling systems, floor and wall finishes and specialties. Mechanical systems include fire protection, plumbing, medical gases piping, heating, ventilation, air conditioning, temperature controls and special systems. Electrical systems include electrical power, lighting, life safety and special systems. Associated site demolition, grading, paving and related landscaping are also included. 6. Pre-Bid Meeting: For a Pre-Bid Meeting, insert Time, Day of Week, Date, and Location of the PreBid Meeting. EXAMPLE: A Pre-Bid Meeting will be held at 10:00 a.m. , Tuesday, July 10, 2006, in Conference Room 219A of Centennial Student Union, Minnesota State University, Mankato. Preparer of the Bidding Documents: Insert the company name of the Project Architect/Engineer who prepared the Bidding Documents. EXAMPLE: Bidding Documents as prepared by the Project Architect/Engineer, ABC Co., are on file at the office of the: 8. Bidding Documents on File: Insert the name of the Builders’ Exchanges where Bidding Documents will be on file. Always include St. Paul, Minneapolis and the nearest local Builders’ Exchanges. See “Builders’ Exchanges & Plan Rooms List” for addresses. Bidding Documents Distribution and Deposits: Insert the Name, Address, and Telephone number of the Project Architect/Engineer from whom prospective Bidders can obtain a copy of the Bidding Documents. EXAMPLE: ABC Co. 123 Any Street Minneapolis, MN ,Zip xxx-xxx-xxxx Insert the deposit amount, if any, required for each set of Bidding Documents. The Architect/Engineer shall determine the amount based on the printing costs. If prospective Bidders want Bidding Documents shipped to them, insert the shipping and handling Amount in addition to the document deposit. Insert the last Date to request documents. SECTION 00 21 13 INSTRUCTIONS TO BIDDERS On page 2, insert the Name of the person in charge of receiving the bids, Project Name, the exact location of Building Name and Room Number where bids are to be delivered, Name of College/University, City, Minnesota, Zip. This data should match the second item in the Advertisement for Bids section. Rev. 4/1/09 Page 4 of 7 MINNESOTA STATE COLLEGES AND UNIVERSITIES Instructions to College/University Preparation of Division 0 Documents EXAMPLE: The envelope shall be addressed and delivered to: John Doe, Director of Purchasing Centennial Student Union Remodeling Wiecking Center, Conference Room 221 Minnesota State University, Mankato Mankato, Minnesota 55601 SECTION 00 41 13 BID FORM 1. Bid Opening Time and Date: On the form top right, insert the the exact Time and Date of the Bid Opening. This must match the time and date in the Advertisement for Bids section. 2. Location of Documents: In paragraph (1), first part, insert the same information from the Advertisement for Bids section, second paragraph, as the individual receiving bids at the College/University and who has the Bidding Documents on file. 3. Project Name: In paragraph (1), second part, use the same Project Name as shown in the Advertisement For Bids section, first paragraph. 4. Deduct Alternates: In paragraph B., insert a Title of each Deduct Alternate for the Project or delete these paragraphs if the Project does not have any deducts. NOTE: Deduct Alternates must be accepted in numerical order only (i.e. Alternate No. One must be accepted before Alternate No. Two, Alternate No. Two before Alternate No. Three, etc.). See Design Standards for additional requirements. Add Alternates are not allowed. 5. Time of Completion: In paragraph (10), insert the Time for Completion of the Project using either a specific date (e.g. November 15, 2006) or a specific number of calendar days (e.g. ninety-five calendar days) after the date of the written Notice to Proceed. This date of completion may also be referenced in Section 00 73 80 and if necessary, should also be inserted there. 6. Unit Prices: In Paragraph (12), if Unit Prices are being requested, insert a brief description of the Work proposed on a unit cost basis and the unit of measure being used. NOTE: Unit Prices are requested for use in Change Orders and are not a factor in Page 5 of 7 Rev. 4/1/09 MINNESOTA STATE COLLEGES AND UNIVERSITIES Instructions to College/University Preparation of Division 0 Documents the award of the contract. If no Unit Price(s) will be requested, delete this paragraph. SECTION 00 43 13 BID BOND Insert the same Project Name and data as shown in the Advertisement for Bids section, first paragraph. SECTION 00 73 46 PREVAILING WAGE RATE REQUIREMENTS Insert a copy of the applicable Prevailing Wage Rate Determination Schedule, as published by Minnesota Department of Labor and Industry for the Project county of record. Information is available from the DOLI website for commercial construction at www.dli.mn.gov/LS/PrevWageComm.asp. . SECTION 00 73 80 SPECIAL CONDITIONS OF THE CONTRACT FOR CONSTRUCTION,TIME OF COMPLETION AND LIQUIDATED DAMAGES 1. Time of Completion: Insert the Time for Completion of the Project. This will match the text used in the Bid Form, paragraph (10). 2. Liquidated Damages Amount: Liquidated Damages may be used to pay for additional Project costs and/or services required until the Work is complete or for loss of use of the facility. Use $1,000 per day if you cannot quantify the loss. 3. Roofing Inspection Services Costs: Liquidated Damages may also be used to pay for additional roofing systems Inspection and Testing Services costs beyond the Contract Time projected for this work. When the project includes roofing, roof repairs or reroofing work, verify applicable roofing systems Specifications Section numbers and names with the Architect/Engineer and insert anticipated costs to be incurred by the Owner if the roofing and/or reroofing construction extends longer than the time allowed by the Contract Documents for this work. Determine and insert a Liquidated Damages amount to include, but not limited to: roofing systems Inspection Services, Testing Services and per diem reimbursable expenses for inspection and testing personnel. In the event that Substantial Completion of the Work has not occurred on or before February 27, 2007, then commencing on February 28, 2007, and continuing for each day or fraction thereof until Substantial Completion of the Work is established, the Contractor shall pay to the Owner, at the sole option of the Owner, the amount of One Thousand and No/100 Dollars ($1,000.00) per diem (“Damage Amount”). The Contractor and the Owner agree that in the event the Substantial Completion of the Work does not occur on or prior to February 27, 2007, the Owner will suffer damages in an amount which may, due to the special nature of the Project, the Owner’s business and the Owner’s reliance upon the Substantial Completion date, be impractical or extremely difficult to ascertain. The Owner and the Contractor agree that the Damage Amount is a reasonable estimate of the damages that the Owner will suffer in the event that Substantial Completion of the Work does not occur on or prior to the Completion Date. Also refer to roofing systems technical Specifications Sections including: Section 07 51 00: Built-Up Bituminous Roofing, Section 07 53 00: Elastomeric Membrane Roofing, and Section 07 62 00: Sheet Metal Flashing and Trim. For construction Work which exceeds the time period(s) provided in these Sections, the Contractor shall pay to the Owner, at the sole option of the Owner, the amount of Nine Hundred and No/100 Dollars ($900.00) per diem (“Damage Amount”), in addition to other liquidated damages amounts.
