1.0 ADMINISTRATIVE HEALTH SERVICES

1.1 Tobacco-Free Ways: Pima County’s Tobacco Prevention & Control Project – March

Prevention
   Eleven (11) schools began intensive implementation of Get Read About Tobacco or Project ALERT and twelve (12) schools
   began brief implementation of these programs.
   The S.W.A.T. coalition (Students Working Against Tobacco) worked on their “In Memory Of” cards for “Kick Butts Day.”

Cessation
  Twenty one (21) individuals enrolled in a tobacco cessation class.
  Five (5) individuals contacted TFW for information only.
  Two (2) classes were completed in March.
  Eight (8) referrals from health care providers and seven (7) referrals from the Arizona Smokers’ Helpline were received.
  Five (5) referrals were sent to the Arizona Smokers’ Helpline from Tobacco-Free Ways

Public Relations/Community Events
  Be Safe Saturday was held at TMC Hospital on Saturday, March 11, 2006. Approximately 2,500 people attended.
  The Tucson Indian Center held their Elder Health & Wellness Fair on Friday, March 31, 2006 at the Inn Suites. It was a good
  opportunity to network with the Native American community. Approximately 100 people attended.

 Enrollment and Eligibility Determinations:
 ADHS modified their formula used to identify the number of clients potentially eligible for WIC in Pima County. For 2005-06,
 ADHS now estimates there are 63,516 WIC eligible clients in Pima County, compared to the 2004-05 estimate of 32,673 women
 and children. In terms of FOOD Plus for Seniors, it is estimated 18,000 seniors in Pima County may be eligible. However, since
 these are not entitlement programs, contracts for the current fiscal year limit WIC enrollment to 17,450 women and children and
 FOOD Plus to 4,550 women, children and seniors. To stay within the contract limits, enrollment may be restricted in accordance
 with federal priority systems. WIC prioritizes enrollment in accordance with 7 levels of health risks. FOOD Plus prioritizes
 enrollment according to the client’s age and receipt of other supplemental food programs. Federal regulations require nutrition
 program eligibility be determined and benefits provided for new applicants within 10 days of initial request for assistance for
 high-risk clients, and within 20 days of request for all others. The number of eligibility determinations processed and the
 priorities open for each program follow.

 More FOOD Plus clients have requested assistance than can be provided through our current contract. As a result, in February
 2006, a waiting list for all new senior applicants was implemented. As of the end of this month, 117 seniors were on the waiting
 list for FOOD Plus.

 2.2 Nutrition Education
 Through group and individual encounters, clients receive education on basic health, nutrition, growth, development, and disease
 prevention strategies. For high-risk clients, individual case management is also provided. A summary of these efforts follows:

 2.3 Breastfeeding Education and Support Team
 This program provides individual and group breastfeeding support and instruction to limited income families. Community
 Health & Dietetic Services has over 15 staff that are certified breastfeeding counselors. Breastfeeding promotion and support are
 especially important for this population for at least the following reasons:
    Breastfeeding is more economical than formula feeding.
    Breastfeeding is less likely to cause illness than formula feeding because it eliminates problems from improper mixing,
    storing and handling of infant formula.
    Breastfeeding is associated with improved health for both the mother and baby, such as:
         Mothers have a lower incidence of breast cancer
         Infants have a lower incidence of obesity, asthma and allergies
         Mothers return to healthy weight through calories required to breastfeed
         Infants have a stronger immune system

 2.4 Project Heartlife
 This cardiovascular disease prevention program is federally funded by the Preventive Health and Health Services Block grant
 through a contract with ADHS. Studies have shown that participation in all types of activity decreases after the 3 rd grade,
 negatively impacting the health of youth and adults. Hence, Project Heartlife’s focus is to promote physical activity to reduce
 heart disease through the Promoting Lifetime Activity for Youth Curriculum. The goal of this program is to prevent the decline in
 physical activity among 4th – 8th grade students by teaching skills that promote self-directed, lifetime activity. This effort is
 undertaken to prevent children from becoming inactive teens and adults.

2.5 Well Woman Healthcheck
Contracted through ADHS, this program is funded by CDC's Breast and Cervical Cancer Early Detection Program. Funded
health screening services include free or low cost mammogram and pap tests to women age 50 and over who are uninsured or
underinsured.

Services are provided through sub-contracts with St. Elizabeth's of Hungary, El Pueblo Neighborhood Center, A New Creation
Women’s Center and Susan La Ponsie who provide pap smear tests, pelvic examinations, clinical breast examinations,
mammograms and breast self examination instruction. Pima County Health Department also offers Well Woman Healthcheck
services at the Health Department’s Theresa Lee and North Clinics.

PCHD provides education, support and outreach to eligible women and the surrounding community. WWHP outreach staff
educates physicians, neighborhood groups in addition to women on the importance of early cancer screening and resources
available.

As a result of an increase in the demand for Breast and Cervical Cancer Screening Services in Pima County, Well Woman
Healthcheck had to initiate a Waiting List for new applicants. This measure was taken to assure that funding is available to serve
the target population and stay within budgetary guidelines set by contractual agreement with ADHS.

2.6 Arizona Farmer’s Market Nutrition Program
Arizona has launched a program that encourages and supports the consumption of fresh fruits and vegetables among limited
income families. Recipients of Pima County WIC or FOOD Plus may be eligible to receive coupons for redemption at ADHS
approved farmer’s markets. For the new 2006 contract year, the value of AZMNP coupons doubled from $15/booklet to
$30/booklet. The table below summarizes the number of families enrolled each month. Months with zero families served
indicate either the contract was not yet enacted or the contract resources were exhausted.

2.7 Folate Program
This program’s goal is to reduce neural tube birth defects caused by inadequate pre-conceptual folate intake. Staff work in
community settings to identify and assess childbearing aged women, then (when appropriate) distribute multi-vitamins containing
folic acid. The program start date in Pima County was November 2002.

The Folate contract with ADHS expired June 30, 2004. Since that time, ADHS has not made funds available to pay for personnel
to operate the program. Pima County funded a supplemental budget request, effective July 2005. This funding supports
personnel and other operating expenses; hence, the program is again able to serve clients.
3.0 DISEASE CONTROL

3.1 AIDS Program

For April of 2006, the Surveillance team completed 27 investigations. Of the 27 cases investigated, 23 are newly reported, and 4
have been previously reported.

   The cumulative number of AIDS cases reported in Pima County since 1983 is 1,917.
   The cumulative number of HIV cases in Pima County is 1,852.

3.2 Bioterrorism Preparedness Program - March 2006

Program planning and readiness activities:
   Routine communication and interface with Arizona Department of Health Services (ADHS), community first responder
   agencies, schools, hospital and healthcare organizations.

   Staff organized and facilitated a one day conference attended by 40 Pima County school nurses on school health office
   preparedness. Topics included Introduction to Incident Command System, Introduction to Critical Incident Stress
   Management, Children in Disasters and Personal/ Family Preparedness.

   Pandemic flu and mass clinic planning continued, both internally and with community partners. Staff collaborated with
   bioterrorism preparedness representatives from Pinal, Santa Cruz and Cochise counties to discuss response to public health
   emergencies at a regional level.

   Staff continued work with MPH interns from the University of Arizona College of Public Health. The interns are assisting
   staff in the areas of public health preparedness planning, isolation and quarantine and food bio-security.

   Program management attended the Fatality Management for Pandemic Influenza Working Group Conference sponsored by
   the Joint Task Force for Mortuary Affairs in Ft Monroe, VA. The groups provided input for a white paper that will be
   delivered to Congress in order to assist in policy development for potential fatalities resulting from an influenza pandemic.

   Media spokesperson training was presented by the program’s communication coordinator to the Chief Medical Officer and
   her back-up physicians.

   Staff attended the annual ADHS Bioterrorism/Vector-borne/Zoonotic Disease Conference in Phoenix. Topics included
   planning for pandemic influenza, avian flu, West Nile Virus, Hantavirus and other emerging and endemic health concerns.

Communications and capacity building activities:
Final installation of the program’s video conferencing equipment occurred during March. The equipment, located in the
Emergency Operations Center, will allow for video training through the Arizona Telemedicine Program, as well as allow for
video communication between Pima County Health Department and external partners during actual emergencies.

3.3 Epidemiology

Disease and health related telephone calls:
Staff responded to at least 237 phone calls during the month. Forty eight (48) of 237 related to reportable diseases. Case intakes
completed as required. Health education provided to all callers. Twenty six (26) calls were related to vaccines and vaccination
recommendations including mumps and others. Twenty four (24) calls were either positive rabies reports on wild animals or
related to dog or cat bites needing rabies risk assessment.

New reports in April:
215 case reports were received and investigated this month. Please note that not all case reports resulted in confirmed cases. All
cases that were interviewed received appropriate health education messages related to their disease. Disease reports received

Outbreaks Investigated:
No new outbreaks were reported to Epidemiology during the month of April.

Communicable Disease Notes of Interest:

   Pertussis-The level of new reported cases appears to be leveling off and even decreasing. Epidemiology staff will assess
   level of this disease and may recommend declaring end of this outbreak during May 2006.

   Mumps- Pima County Health Department received a number of reports about possible mumps cases during April 2006. All
   reports were investigated. None were confirmed as mumps. The increase in reports of possible cases was most likely in
   relation to heightened awareness of the disease following national media coverage of a large outbreak in Iowa.

3.4 Sexually Transmitted Diseases

Disease Intervention:
   Syphilis: A total of 593 syphilis serologies were screened at our State Lab with 15 positives reported to us. Two cases of
   primary syphilis, one case of secondary syphilis and five late latent syphilis were diagnosed in April. All patients were
   properly treated and contact investigations were conducted. After an epidemiological investigation was conducted, it was
   determined that the remaining seven were previously treated. The number of syphilis cases reported in Pima County
   decreased by 29% from 41 YTD 2005 to 29 YTD 2006.

   Gonorrhea: Eleven cases of gonorrhea were diagnosed in the STD clinic during April, 57 cases were reported in Pima
   County during this period. All patients diagnosed in the STD clinic were properly treated for their infection, and contact
   investigations were conducted. A review of the morbidity cards submitted from medical facilities in the community indicates
   that all patients were treated with medications effective against resistant gonorrhea. The number of gonorrhea cases reported
   in Pima County increased by 27% from 147YTD 05, to 202 YTD 06.

   Chlamydia: Twelve cases of chlamydia were diagnosed in the STD Clinic; 211 cases were reported in Pima County. The
   STD clinic performed 135 tests on females with 7 (5%) positive and 302 males with 7 (2%) positive. The STD clinic
   diagnosed 109 cases of non gonococcal urethritis on males who tested negative for chlamydia. The number of chlamydia
   cases reported in Pima County increased by 9% from 915 YTD 2005 to 1003 YTD 2006.

1,601state supported chlamydia tests were conducted in Southern Arizona with 148 (9%) positive. The STD Program continues
to support Family Planning Clinics in the notification and management of patients diagnosed with STD's and provides treatment
and partner notification services to these patients when requested.

Special Projects
   Kino Teen Clinic screened 44 clients for gonorrhea with 2 positive.

   Staff participated in an active surveillance survey targeting El Rio Clinic, El Pueblo, UMC, Kino, TMC, Northwest, St.
   Mary’s hospitals, San Xavier Indian Health Services and Southern Arizona Red Cross Blood Plasma Centers.

   The STD Control Program screens selected individuals for STDs at the Pima County Jail. PCHD provides the chlamydia and
   gonorrhea collection kits, and the State Lab performs the tests and reports the positives to us for follow-up.

   The STD/Family Planning Clinic provides STD screening inside the Juvenile Detention Center, 150 clients were screened
   with 12(8%) positive chlamydia. The clinic will screen for STD’s every Monday and Wednesday. The laboratory support
   will be provided by the State Lab.

3.5 Vaccine Preventable Disease

The Immunization Program/Vaccine Preventable Disease Program strives to prevent and control vaccine preventable diseases in
several ways. It ensures that vaccine is available, monitors immunization levels, provides professional education, investigates
vaccine preventable diseases, and implements control measures.

PROGRAM ACTIVITIES:

Childhood Immunizations
   Tot shot Clinics: South Country Club WIC clinic served 49 children with 130 Immunizations during the month of April.
   (*Revised March Tot shot Clinics: served 74 children with 175 Immunizations)

   VPD staff continues to assist Tot Shots with clinic operations and provide recommended vaccines for adult contacts of infants
   by providing Tdap (Tetanus, diphtheria and acellular pertussis).

   800 Baby Packets (immunization informational package for new parents) were distributed this month. Staff continued to
   prepare packets to replace those distributed in anticipation of those needed for the rest of the year.

   Distribution of 100 Arizona School Immunization Records (ASIR) this month.

   VPD program participated in the 7th Annual City/ Pima Co. Health Fair. VPD Staff provided attendees with a wide array of
   important and useful information and services.
Adult Immunizations
   This Program continues to assist in providing vaccination to adult contacts of infants and high risk children. Vaccine was provided to
   El Pueblo Community Health Center and El Rio in an effort to reduced missed opportunities for contacts of infants.

Education/Training/Planning
   Continued participation in Flu Pandemic planning in collaboration with Bio-T, Public Health Nursing, and Epidemiology.

   VPD Manager attended Summer Clinic and Care Fair Planning Meetings.

   VPD Manager attended ASIIS Meeting in Phoenix, review & follow-up issues of last meeting minutes and demonstrations of
   various features.

   VPD Manager and staff attended TAPI Awards dinner and ceremony in Phoenix. Nominations for “Big Shots” Award were
   submitted. A staff member from PCHD East was awarded.

   VPD Manager and staff attended the 13th Annual Arizona Immunization Conference. The theme was “Love them., Protect
   them, and Immunize them.”

   Preparedness immunization training continues with Paramedics Tucson Fire Districts added. Smallpox vaccination techniques and Flu
   Pandemic Planning updates are also included in this training.

   Plans are in process for the upcoming flu vaccine season.

   Attendance to continuing education programs regarding Pandemic flu, State In-service on Immunization Data Report Process
   and Reporting and usage of the new Medsis disease control reporting system continues.

   CDC has selected PCHD to be showcased during National Infant Immunization Week along with Yuma County and Santa
   Cruz County. Plans continue for activities in Pima County which include activities such as Grand Rounds, showcasing a
   PHN immunization clinic.

   The VPD program continued to work with outside connections through SAIC and private partners.

   Planning activities continue with U of A Public Health student’s project. U of A students will be auditing 9th grades students
   regarding next year school requirements of Tdap for school entry. Arrangements are continuing with five school districts.
Contract Activities
   Perinatal Hepatitis B: Currently continuing to locate clients to provide information regarding Program. Program is currently
   following 37 post-natal clients. 11 Prenatal clients. Chart reviews were conducted to update, date and time of vaccination for
   infant’s birth dose of Hepatitis B vaccination and H-big. Thirteen Post-natal are waiting serology testing. Three prenatal
   clients are awaiting birth of infants this month.
   Influenza: Influenza Like Illness (ILI) reporting continues. ILI reported from four Child Cares and 16 Schools to equal 196
   cases. Flu labs reported five cases from two hospitals. These numbers do not include Sentinel Sites.
   SAIC Meeting was conducted; Communicable Disease presented a discussion on schools reporting and surveillance of
   diseases in school populations, have prompt to better understand and how to too best utilize the reports, which are received by
   Disease Control. In attendance VPD Staff, and Carondelet Parish, Concentra, Cochise County, ASDB, Co Med. Cup Program,
   PHS. U Of A Student.

   Sentinel site visits were conducted to report on seasonal planning with information on participation in the following year.
   CDI and Medical Director visited sentinel sites in an effort to recruit for next year and stress the importance of this reporting
   in planning for Flu Pandemic and local outbreaks.

   Varicella surveillance reported two schools, 23 cases and no Child cares submitted Varicella surveillance Forms. Nineteen
   Schools reported 56 cases collectively. (Three Doctors Office’s reported, three cases, two hospitals with one report, and one
   business reported one case.)
   Revised April Immunization Data Reports (IDR). Assessed were 222 Kindergarten, age groups: 0-2 years of age, 262; 2-5 yrs
   old and Head start, 337 students; also assessed were 110 7th graders; and 97 10th graders: total = 1,028. Eight charter schools
   and seven day care centers were audited in April. VPD continued IDR Validation audits in April. Validations (audits) will be
   scheduled for May. A class will be offered to anyone in need of reviewing State requirements of processing or filling IDR
   forms.
   The VPD continues to participate in the planning for Pima County Nurse Core and other Volunteer recruitment for activities
   in supporting immunization clinics and daily activities. First recruiting meeting held with ten nurses in attendance.

Vaccine Preventable Diseases:

   Respiratory Suncytial Virus (RSV) positive lab reported from one hospital.
   Continuation of investigating Pertussis cases and reporting weekly statistics. Implementation of “Cover your cough, wash
   your hands” was recommended to all schools, Day Cares, and Pre-schools continues. Strategies for implementation of new
   Tdap vaccine to adolescents continue.
   VPD Manager was active in ongoing Smallpox training for first responders, Flu Pandemic planning, and Nurse Corps Volunteer process.

West Nile Virus:
Avian Surveillance
Through October 2006, Arizona Department of Health Services (ADHS) has begun
accepting birds for West Nile virus testing for the 2006 mosquito season. Persons who would like to submit a dead bird for West
Nile virus testing should call (520) 740-2760.

Pima County residents reported twenty-four (24) birds to Consumer Health and Food Safety (CHFS) for possible testing for the
presence of West Nile virus (WNV). Four (4) birds met the criteria set by Arizona Veterinary Diagnostic Laboratory (AVDL) for
testing.

Mosquito Surveillance

Pima County has resumed its mosquito surveillance program. Mosquito traps will be set in designated locations to assess
mosquito activity in Pima County. In support of the (ADHS) Mosquito Borne Virus Surveillance Program, mosquito specimens
are sent to ADHS Vector Borne and Zoonotic Disease Section (VBZD) for identification and arbovirus testing.

The Mosquito Borne Virus Surveillance Program tests adult female mosquitoes for mosquito related diseases: Western Equine
Encephalitis, St. Louis Encephalitis and West Nile virus. The testing was done weekly from March through October at selected
sites through out the county. Results are normally received 2-3 weeks after specimens are submitted to the Vector Borne and
Zoonotic Disease Section of ADHS.

Currently in 2006, no trap sites have been reported as testing positive for West Nile virus.

Community Relations Programs:

On April 6, 2006, Consumer Health and Food Safety (CHFS) staff presented a class/demonstration to The University of Arizona
Epidemiology Department at Sweetwater wetlands on Mosquito trapping and treatment.

On April 25, 2006, Kay Howard, Vector Control (CHFS) and Pima County Wastewater Outlying facilities staff held a meeting
regarding their joining the mosquito-trapping program.

            7.0 PUBLIC HEALTH NURSING

            PHN fulfills public health’s essential services by implementing strategies to address public health problems identified through
            community assessment. Interventions are designed to meet the needs of the community and are accomplished as part of a team
            with members from other public health disciplines and with community partners.

            Highlights:
               Health Start Program: 15 new clients, 25 Prenatal Visits, 41 Family Follow Visits.-
               Prenatal Block Grant Coordinator held informational question/answer session for community agencies/professionals
               interested in applying for County Prenatal Grants funding.
