=============================================================================== PARENT/GUARDIAN CONTRACT As the parent or guardian of the child(ren) listed, I consent to their participation in the Franklin Township Pop Warner Football/Cheerleader activities and we agree to the following: 1. Hold FTPW and its organizers, officers, directors, board members, sponsors, supervisors, coaches, volunteers and players harmless for any claim arising from my or my child(ren)’s participation in any FTPW activity including any injury to myself, my child(ren) or any spectator accompanying me or my children at any FTPW activity. I further agree to limit any claims that I or my children may have to applicable insurance coverage. 2. Return uniforms and equipment in a condition as received, except for normal wear. 3. Provide an Original birth certificate for my child(ren). 4. Provide photo, proof of physical and final report card for my child(ren). 5. Provide proof of residence if required. 6. Demonstrate good sportsmanship values and conduct myself in a manner of good fellowship in support of the FTPW players and adult participants. I further agree to follow and abide by the Pop Warner Official Rules. I am aware that failure to do so will result in appropriate discipline in accordance with the Pop Warner Official Rules and that appropriate discipline may include suspension from the FTPW program depending upon the specific offense. 7. Cooperate with FTPW officials in safety efforts to physically and emotionally protect players. Actively support FTPW fundraising activities, i.e.: cheese cake sales, raffles, tag days and any other fundraising activities. Waive my right to a refund of all registration fees paid, if my child(ren) elect to leave the program after August 10th. 8. Participate in the Work Bond Program and waive my right to refund of the Work Bond Fee, if my child(ren) elect to leave the program after August 20th. I agree to comply with all of the bond requirements, including to work at least 4 hours to meet work bond work hour requirements. I understand that if I do not meet the work bond requirements, i.e. sell all raffles, sell required amount of one additional fundraiser to be determined, work the required work bond hours, return uniforms or fail to pay any debts owed to FTPW, I will forfeit my work bond payment. 9. I agree to allow my child(ren) to be photographed and/or videotaped individually or as part of a team of their participation in the FTPW program. I agree that the photographs or videos of my child(ren) may be used for FTPW program marketing purposes and sales to the general public without further approval from me and without compensation to me or my child(ren). 10. All returned checks will be subject to a $20.00 returned check charge. FTPW reserves the right to require payment by cash or money order from anyone that has issued a check to FTPW that has been returned not paid. I further agree to pay FTPW the amount of the returned check plus the $20.00 returned check charge within 10 days of receiving notice from FTPW or my bank that the check was returned un-paid.

=============================================================================== Parents, Guardians and Program Participants Conduct and Behavior Policy This document describes the policy of Franklin Township Pop Warner Football and Cheerleading Association, regarding the conduct of parents, guardians and program participants relative to fighting or improper physical contact, harassment, verbal abuse or any other improper physical or verbal conduct. • Franklin Township parents, guardians, spectators and adult program participants and administrators must demonstrate good sportsmanship values, good fellowship and conduct in support of the Franklin Township Program, and its policies and guidelines. Fighting or any improper physical or verbal abuses will not be tolerated. Any FTPW parent, guardian, spectator or program participant involved in a fight or an abusive verbal confrontation or improper conduct will be immediately removed from the playing, practice, or scrimmage fields and gymnasiums. Fighting carries a mandatory suspension of one year, subject to the rules of “due process” and the rendering of final ruling for the infraction by a Hearing panel. The Hearing Panel will consist of 3-5 adult members of the FTPW Trustee Board members, coaching staff, FTPW Board members or parents, depending on the severity of the infractions. The Hearing Panel will use the National Pop Warner rules, Administrative manual and or the Central Jersey Pop Warner Rules in deciding all case hearings. If found guilty of violations, the participant(s) will be suspended for 1 year. The 1 year suspension will include suspension from attending or participating in all FTPW activities, including FTPW practices, scrimmages, football games (home or away), or any other FTPW functions or facilities (rented or owned) during the suspension period. Any other violations could result in permanent dismissal from the program, including dismissal of your child or children. This policy also provides for any infraction by youth program participants who have violated FTPW rules and is suspended by his or her coach. Suspension may also include not being allowed to play in a football game, practice or cheer in any game or competition as a result of improper behavior.

=============================================================================== 2009 PARENTS CODE OF CONDUCT (This document supplements the attached Parent/Guardian Contract) Parents, family members, players, and cheerleaders hereby pledge to provide positive support, care and encouragement to participants in Central Jersey Pop Warner and to conduct themselves in a manner, which reflects positively on the organization and demonstrates good character by adhering to the Code of Conduct to follows: 1) Encourage good sportsmanship by demonstrating positive support for all players, cheerleaders, coaches, and officials at every game, practice of the Pop Warner event. 2) Place the emotional and physical well being of all persons above any personal desire to win. 3) Serve as a good example in winning and losing situations and conduct myself in a manner that represents the FTPW program, town and sport, well. 4) Demand a drug, alcohol and tobacco free sports environment, refrain from the use of alcohol, tobacco and performance enhancing substances at all Pop Warner events. 5) Treat other players, cheerleaders, coaches, fans, referees, and Pop Warner officials with respect regardless of race, sex, creed, or ability. 6) Will allow FTPW staff and officials to do their job. I will not enter the field of practice or play, or interfere in any way with the coaching staff. I agree to respect assignment decisions of the coaching staff and agree to resolve any disagreements in a private setting. I will not undermine, in work or deed, the authority of the FTPW staff or administration. 7) Help all children enjoy the Pop Warner football and cheerleading sports experience by assisting with field operations, helping with administrative matters, being a respectful fan, or volunteering in any capacity that I am capable of doing. 8) Participate in the FTPW fundraisers and agree to sell all raffles, one additional fundraiser to be determined and support any other fundraisers. 9) Fulfill my responsibilities to the Work Bond Program and return uniforms and equipment in a condition as received, except for normal wear. I understand that I may be billed for any unreturned or damaged equipment. 10) I understand that FTPW is the owner of the FTPW logo and signature and will refrain from selling, purchasing, displaying, or presenting in any manner merchandise (i.e. apparel), literature or other items without prior written authorization of the Board of Directors. 11) Submit accurate and truthful documentation. 12) Follow the procedure for parental concerns: The parent will contact their team Head Coach for assistance. The Head Coach may involve the Cheerleading Director, the Athletic Director, or other appropriate Director. The respective Director may present the matter to the Board of Directors for ultimate disposition. A violation of the Code of Conduct may result in a warning, probation, suspension, or expulsion from Franklin Twp. Pop Warner. The Board of Directors reserves the right to make changes to any rule, regulation, or code it deems necessary for the safe and proper operation of the organization.

FTPW Volunteer Job Descriptions
GAME DAY Gate Keeper 50/50 Sales Chains Concession Worker Game Gear Sales MISCELLANEOUS Tag Day Car Wash Picture Day COMMITTEES Journal Cheer Responsible for all details related to the publications of the Awards Journals (ex. Soliciting ads, organizing the layout and helping with the desktop publishing). Backbone group of the Cheerleading competition. Responsible for making posters, banners and decorating buses. Also in charge of submitting ideas to Cheer Director for cheerleading gifts. Responsible for daily operations and execution of all FTPW Fundraising activities. Also for collecting monies and submitting reports to Fundraising Chairperson. Organizes and runs these dances and all activities relating to the dance (50/50, door prizes, food) Makes sure chaperone coverage is provided for kids dance. Contact local newspapers and radio in regards to FTPW events, fundraisers and game scores. Prepare articles that will be published weekly. Liaison between newspapers and FTPW. Backbone of the concession stand. Helps Director of Parents with purchasing food supplies needed to run the concession stand. Will be available to support concession stand during home game if needed. Responsible for set up and clean up. Children hold canisters and solicit funds in front of designated establishments. Parental supervision is required. Adults and children wash and dry cars. Also, collect money from the drivers. 2 or 3 adults needed to place kids in order, check and collect envelopes. Keep unauthorized people away from photography area. Sits at the gate during home games to collect donations that are dropped into the donation bucket while people are entering. Sells 50/50 tickets at home games in both the home and away bleachers. (Sideline Helper) Move the chains along the field during the home games. Also knows as the “Best seat in the house.” Works in the concession stand as needed during home games. Assignments include food handling, serving, cashier, dispensing beverages, grill and more. Sells clothing ands merchandise at the table during the home game.
