Pursuant to the lease agreement dated ______________ (“lease”), subsequent occupant let from lessor ___________________________[address], _________________ [city] , _________[state], _________[ZIP], (the “premises”), and more specifically described in the lease, for a term which expires on ___________[date] (the “lease termination date”). Lessor and subsequent occupant agree that when all original occupant(s), namely ______________________________________________ no longer permanently reside(s) in the rental unit, the lessor may raise the rent of the subsequent occupant without regard to the limitations set forth in Section 37.3(a) of the Rent Ordinance. This notice also specifies that when the last original occupant of the rental unit no longer permanently resides in the rental unit, a new tenancy is created for the purposes of determining the rent under the Rent Ordinance.
