CITY OF MCKINNEY, TEXAS FILM PERMIT APPLICATION Date of Application: __________________________________ Signature of Authorized Representative: ________________________________ Any production company, business or individual (“Applicant”) wishing to conduct a film shoot shall obtain a Film Permit. For purposes of this Permit, a “film shoot” means to perform or undertake any activity in a public place within the corporate limits of City, which constitutes, in whole or in part, a component part of or necessary preparation for the process of recording moving images onto a tangible medium, including but not limited to motion picture photographic film, videotape, videodisc, or digitized storage of video data. A “film shoot” does not include “home movies” or “home videos” not intended for commercial rebroadcast or viewing. Please provide the following information pertaining to the Film Shoot. After completion of the items listed herein, return this form to the City of McKinney for processing and distribution. This form must be returned not less than fifteen (15) days prior to filming. Exceptions to this deadline may be made with good cause shown. Film Company is responsible for acquiring any additional permits or permission required if any portion of Film Shoot lies outside of the jurisdictional limits of McKinney. If requested, Film Company shall provide evidence of additional required permits or permission. The Film Permit fee is Twenty-Five Dollars ($25.00), payable upon submission of this Permit Application. TRAFFIC CONTROL: Please describe the area of usage, streets/alleys involved, requested modifications of standard usage, type of controls needed, police/security, cones, barricades, flags, etc. Please clearly describe the location and duration of any requested street closings. A site plan of the area of usage must be included in this Permit Application. Notes: 1. All traffic laws shall be adhered to by Film Company; the only exception is where traffic flow is otherwise clearly designated and controlled. Film Company understands that street closures are a matter of public health and safety and are completely within the discretion of the McKinney Police Department. Street closings may require additional permitting. IV. HAZARDOUS ACTIVITIES. Please describe any activities that are planned for the Film Shoot which may pose a danger to public health and safety (e.g., fires, car chases, explosions). ________________________________________________________________ ________________________________________________________________ ________________________________________________________________ ________________________________________________________________ ________________________________________________________________ A permit may be denied if the police department, fire department or special events manager determines that the Film Shoot would pose a serious threat to public health and safety. V. INDEMNIFICATION: The Film Company shall indemnify and hold harmless the City, its officers, agents and employees, from and against any and all claims, losses, damages, causes of action, suits and liability of any kind, including all expenses of litigation, court costs and reasonable attorney’s fees, for injury or death of any person or for damage to any property that arise out of or in connection with the activities performed by Film Company within the City, whether such injuries, death or damages are caused by the negligence or intentional acts of the Film Company or the negligence of City, or the joint negligence of City and Film Company. IV. VIOLATION OF PERMIT REQUIREMENT OR CITY ORDINANCE: The City reserves the right to close down any Film Shoot that violates any City ordinance, code or deviates from the terms and conditions of this Permit.
