Our health education and wellness programs bring important information and accessible health screenings into the community to help empower our members to understand, improve, and maintain their health. And through our participation in neighborhood programs and community outreach events, collaborations with individual health care providers and health systems, and our support of community groups that address social issues, Keystone Mercy makes a difference in the areas where our members live. Our Mission Is the Foundation Keystone Mercy and its mission grew out of the legacy of Catherine McAuley, the founder of the Sisters of Mercy. Sister McAuley’s Irish-based order first came to Philadelphia in 1861, carrying with them a mission to care for the poor and disadvantaged. Again, thank you for your help.” - Mother of Special Needs Unit Member* We help people get care At Keystone Mercy, we are dedicated to helping our members get care, and to making sure it is the right care for them, helping them to get better and stay well. Getting care, however, is not enough. We strive to make sure our members get the best quality of care possible through our extensive network of over 12,000 primary care providers and specialists, the largest provider network in the Southeast HealthChoices region. Members have 24-hour phone access to our Member Services department for assistance. The Plan coordinates care for our members through the oversight of our qualified teams of doctors, nurses, and social workers, and through close partnership with our network providers. Our coordination of care is reviewed through rigorous processes of external accreditation with organizations such as the National Committee for Quality Assurance (NCQA) and the Utilization Review Accreditation Committee (URAC). Keystone Mercy has earned the following recognitions: • NCQA’s highest accreditation rating of EXCELLENT, based upon customer service and health care quality (re-accredited in 2007) URAC accreditation for case management Ranked #31 among all Medicaid health plans in the country by U.S. News & World Report/NCQA’s “America’s Best Health Plans” in 2007 • • Intensive Care Management To respond to the medical and social needs of our members who most need our help, we developed a holistic approach to care management that allows us to maximize the care for these members. Our care management program provides proactive and intensive health care coordination services for more than 12,000 members who are identified as having multiple chronic conditions and co-morbidities. Proactive case management saves on costs, reduces utilization, and improves quality In 2007, Keystone Mercy received the Thomson Healthcare “Innovator Award” for its proactive approach to case management, using a population management strategy that targets high-risk members with multiple chronic conditions and co-morbidities. These members make up a small portion of membership, yet utilize a disproportionately high volume of resources. The Plan found that two percent of its total member population was responsible for 14 percent of its costs. The program leverages technological solutions for data access and case management, financial incentives to primary care practices for program participation, and partnerships with two vendors for outreach to selected members. Through Keystone Mercy’s care management program, participants were assessed for barriers to care delivery and coached and guided to receive needed care. Participants experienced fewer inpatient admissions and higher medication compliance, resulting in an overall reduction in the cost of their care. Most importantly, the program positively affected members’ quality of life, as evidenced by their many letters of appreciation. Keystone Mercy’s prenatal care program, WeeCare, is designed to improve birth outcomes and reduce the incidence of pregnancy-related complications through early prenatal education and intervention. A member’s Care Manager acts as advisor, coach, and advocate throughout the pregnancy and into the post-partum period, and plays a hands-on role, as necessary, in coordinating appointments, home care, transportation and social services. Through the WeeCare program, our preterm birth rate has decreased from 9.7 to 7.6 percent. Pediatric Immunizations Our pediatric immunization rate is an area of continued focus for Keystone Mercy due to the high percentage of pediatric members on our Plan. Vaccinations have overwhelming public health benefits and the ability to improve long term health outcomes and reduce health care costs through disease prevention. Over the past four years, the Plan has implemented a focused outreach program to encourage parents to take children for the appropriate immunizations. The Urgent Care Practice at Kensington Hospital and the Urgent Care Facility at Lansdowne were both opened in 2007 to address the increasing over-utilization of hospital emergency rooms. The urgent care centers provide patients with prompt, quality care after normal office hours and on Saturday afternoons. The Urgent Care Practice at Kensington Hospital, in the heart of the Latino community in North Philadelphia, is one of the first in the Philadelphia region. Special Needs Unit This unit was created to help members with a range of special needs navigate the health care system and get connected with the care they need. Our team coordinates and collaborates with members’ primary care providers or specialists, and with necessary community-based agencies. The team also empowers these members by educating them on their conditions and by addressing the psychosocial issues that may be impeding positive health outcomes. Member Retention Advocacy In May 2007, Keystone Mercy formed a team dedicated to helping members who are struggling with retaining their Medical Assistance benefits. This team is unique because it not only helps the members it serves, but it also provides work opportunities to a group of former Temporary Assistance for Needy Families (TANF) recipients who were hired and trained to staff the new unit. Since its inception, this caring and compassionate team has helped over 24,000 members navigate the eligibility and recertification processes. 9 competence “This program…has helped save my daughter’s life. We help people stay well Keystone Mercy Health Plan helps people stay well by providing health education programs that teach our members and communities how to prevent and manage illness. Healthy Hoops ® Healthy Hoops® is an innovative health education program that uses basketball as a platform to educate children with asthma and their families about ways to manage the disease. The program has reached over 2,500 children and family members in the Southeast HealthChoices area, generating positive health outcomes since 2003. In addition to asthma, the program now targets childhood obesity and cardiovascular disease. Healthy Hoops hosted six major events in 2007. As a result of Healthy Hoops educational classes and interventions, program participants increased their use of and compliance with appropriate medications for asthma. Program outcomes also show a decrease of 12 percent in overall emergency room utilization for Healthy Hoops participants, as well as a decrease in inpatient stays of 23 percent. Daniel J. Empowerment Tour for a Healthier Life The Health Ministry Program for Women, started in 2000, is a health education initiative that uses a faith-based environment to provide wellness education to minority women and inspire them to take charge of their health. “Keystone Mercy has created a model for plans nationwide to follow on how to bridge language and cultural gaps that so easily get in the way of quality health care.” “The Healthy Hoops program is an innovative solution that tackles the very real issue of health care disparities among racial and ethnic minorities.” “It has motivated me to take care of my body by eating right and making better lifestyle changes.” - Health Ministry participant* In early 2007, the Health Ministry - NCQA President Margaret E. O’Kane Program for Women partnered with six Philadelphia-area churches to present the “40 Day Journey,” which challenged participants to make healthy lifestyle changes. Pre- and post-Journey physical assessments were given to track individual health progress. In the fall, the Health Ministry Program hosted the “Empowerment Tour for a Healthier Life,” which included five health conferences held at churches throughout Philadelphia, each focusing on a specific health disparity affecting the community: breast cancer, diabetes and weight management, high blood pressure and stroke, heart disease, and depression and mental health. Over 2,500 screenings were provided to participants during the Tour. The program was also expanded to include men’s health issues, with an additional conference promoting prostate and colon cancer prevention for men. 11 “The playground that Keystone Mercy not only donated but also built has been a true gift that continues to give everyday. The Village Charter School students now have a safe place to play and they get the opportunity to engage in healthy exercise daily. Finally, the students experienced and recognized that the act of human kindness and giving to people you don’t know can in fact ‘change a child’s world.’” - Dale D. Kelly, Principal, Village Charter School hospitality Associates contributing time to Rebuilding Together Philadelphia We help build healthy communities At Keystone Mercy, we believe that because of the many social and economic concerns experienced by our members, our commitment must go beyond our day-to-day work of providing health care coverage and services. As part of our mission, we strive to not only care for our members’ physical needs, but also to care for the communities in which they live. That is why the Company sponsors and supports a wide range of organizations that address social, economic and health issues in the community. We also look for other ways that our Company can further support the community and our members’ needs. Here are a few examples of some of the activities that our Company and our associates participated in during 2007 that illustrate how we put our mission into action in the community. Anne Morrissey with Village Charter School students Building Safe Playgrounds As a way to give back to the community we serve, the Company started a new initiative in 2007: building safe playgrounds at area schools. We recognized the need for play spaces that provide settings for safe, physical activity, while decreasing preventable injuries and contributing to the healthy development of the children in the community. Physical activity has also been shown to increase academic performance, reduce youth violence, and help to break the cycle of poverty. In one day each, Keystone Mercy’s leadership team and associates, along with community members, built the playground structures from the ground up, painted murals, and cleaned the surrounding areas of two local schools, Philadelphia’s Luis Muñoz-Marin Elementary School and Village Charter School in Chester Township, PA. These projects contributed to the safety and healthy development of close to 1,400 children. Rebuilding Together Philadelphia For nine years, the Company has partnered with Rebuilding Together Philadelphia, a volunteer organization that makes essential repairs for low-income homeowners in the city’s most fragile neighborhoods. In 2007, the Keystone Mercy team, consisting of associates and their family members, worked on two houses in North Philadelphia that required clean-up and small renovations to provide a warm, safe, and dry environment for the residents. Additionally, our associates sponsored one of two Rebuilding Together Philadelphia Dragon Boats in the International Dragon Boat Festival, held on the Schuylkill River in October. The Rebuilding Together paddlers raised thousands of dollars, enough to sponsor five houses for rehabilitation. Associates Learn to Save Lives In 2007, as part of our sponsorship of the GO RED initiative, our associates were given the opportunity to attend free CPR trainings hosted by the American Heart Association. We hosted wellness pavilions at several of these events to promote health education and disease prevention. The program provides an opportunity for college students from our communities to gain work experience and to learn about the health care field. Many of our interns come to us from schools in our area or from local community-based organizations, and some have even been recipients of our Member Scholarship Program. Keystone Mercy is also a participating employer in WorkReady Philadelphia, a city program that places students in area companies to give them real-life work experience. For Keystone Mercy, the internship program has two main goals: to provide opportunities for growth, development and future employment to local youth, which benefits our community, and to train the next generation of health care professionals, which benefits the health care field and our future members. dignity “Thank you from the bottom of my heart. Whether through donations to our Mission Partnership Program or through volunteer work, our associates make our mission come alive far beyond their work duties. The following are highlights of Company and associate giving in 2007: • 20% of associates volunteered almost 2,000 hours to serve in the community. The Company hosted and participated in over 100 health promotion events in the community, and supported over 160 community organizations (including non-profits, faith-based, and health and human services organizations). Our associates raised hundreds of thousands of dollars for various health and social causes. 2007 Member Scholarship winners • • Going Beyond Health Care: Mission Partnership Mission Partnership is our unique, associate-funded giving program that seeks to serve our members’ needs beyond traditional health care services. The program started in 1988 when associates got together to donate a scholarship to one member. 2007 marked the 19th anniversary of the program, and associates donated over $200,000. “Everyone has a purpose in life…with the right encouragement, support, and education, anyone can reach their highest capability in life.” - Keystone Mercy Member Scholarship Recipient The annual Mission Partnership Program supports the Member Scholarship Program and the Gifts of Mercy Program, both detailed below. Member Scholarship Program The Member Scholarship Program recognizes Keystone Mercy members who have committed themselves to continuing their education beyond high school. The program, funded by associates, provides assistance to members of all ages, both recent high school graduates and non-traditional students, to help them pay for their education from an accredited college or trade school. In 2007, 34 members received $2,000 scholarships from the program. Since its inception in 1988, the Member Scholarship Program has awarded nearly 650 scholarships to members, totaling nearly $1.3 million. Transforming Lives Through Our Gifts of Mercy Over the past 15 years, our associates have provided holiday gifts and shared the season’s joy with over 600 families through the Gifts of Mercy Program. In 2007, teams of associate volunteers transformed the holidays of nearly 150 children who would have had no Christmas without us. $18,000 in funds raised during the Mission Partnership campaign were spent to bring a happy holiday season to member families in need, fulfilling holiday dreams (ranging from Elmo dolls to bicycles) and meeting everyday needs. Some families had no food, or their utilities had been turned off. Some had no coats, gloves or hats. Some had no dishes, sheets or towels. Our Gifts of Mercy teams, led by our associates, shopped, wrapped and delivered gifts for these member families on their own time. 17 . . . in their own words “The expertise and commitment shown by the Keystone Mercy staff is commendable. The HEOAC works to identify members’ health education needs, and helps us plan, implement and evaluate health education and outreach activities. The members of the HEOAC then serve as our ambassadors in the community, helping to promote Keystone Mercy’s health education outreach programs.
