					                            BRAN MANHOLE CHECKLIST

1.      Handhole at George Reynolds Library

        Inspected on 5/24/04

       Handhole had some debris in it which we removed. The innerduct in the
handhole was not plugged, we filled innerduct with sealant. Vertical racks are not
applicable. Splice case was in good condition (outer closure filled with encapsulant), the
BRAN cable was labeled, and warning labels are not applicable.


2.      Manhole 22nd and Spruce

        Inspected on 5/24/04

       Manhole had some water and no debris. The ducts were plugged in the manhole.
Vertical racks were secured to the walls. Steps and splice case are secured, (well above
the water), the BRAN cable is labeled, and BRAN warning sign is in place.

3.      Manhole (Public Safety Building)

        Inspected on 5/24/04

        Manhole had no debris, but ICG fiber is coiled but not racked, (only BRAN fiber
is coiled and racked). The ducts were plugged in the manhole. Vertical racks were
secured to the walls. Steps and splice case are secured, the BRAN cable is labeled, and
the BRAN warning sign is in place.

3. a.   Handhole in grass at Public Safety Building.

        Inspected 5/24/04

       Handhole had no debris. All of the ducts in the handhole are plugged. Vertical
racks and steps are not applicable. Splice case is in good condition. Handhole is dry.
BRAN cable is labeled and a warning sign is not applicable.

4.      Manhole near Railroad tracks on Pearl

        Inspected 5/24/04

        The manhole was free of debris. All of the ducts in the manhole are sealed.
Vertical racks and steps are secured to the walls. Cable and splice case are secure, (this
location has an extended cover on the splice enclosure—it does not have an outer cover—
the manhole is very dry). The BRAN cable is labeled and BRAN warning signs are in
place. I will check the original contract documents regarding the outer cover.
5.     Handhole 47th Street and Pearl Parkway

       Inspected 5/24/04

       The handhole was full of spider webs, which we swept out. The ducts were not
plugged upon our arrival, but we sealed the ducts with foam. Vertical racks and steps are
not applicable at this location. The cable and splice cases are secure, as well as being
labeled. A BRAN warning sign is not applicable at this location. This splice also has an
extended cover on it and is not encapsulated. The handhole is dry.

6.     ICG Manhole (East)

       Inspected 5/24/04

       The manhole was free of debris. The ducts are plugged. This manhole did not
have vertical racks and steps. The cable was coiled and the splice case is secure. The
BRAN cable was not labeled correctly, (label left on cutout section), we placed label on
BRAN 96 strand backbone. There is not a BRAN manhole warning sign in this location.

7.     ICG Manhole (West)

       Inspected 5/24/04

        This manhole was in poor condition. There was debris in the manhole upon our
arrival. The BRAN warning signs were ignored. The cable was not racked, (even though
there are racks installed), and labels were left on cutout sections of the BRAN cable. The
splice case was lying on the floor of the manhole. The ducts are sealed in the manhole
and the splice case is now secure. The label has been attached to the correct cable.
