PURPOSE OF THE POSITION

The Community Health Representative (CHR) is responsible for participating in the delivery of
high standard Community Health, treatment and surveillance programs in order to provide
quality heath prevention and treatment in the community. As a member of the Health Care
Team, he/she is also responsible for liaising between patients, families, the community and
health care providers to ensure patients and families understand their conditions and treatment,
and are receiving appropriate care. The incumbent will also educate and monitor the community
for general health, and in epidemics and emergencies.

SCOPE

The Community Health Representative directly impacts individuals and the community as a
whole by working with health care providers and the community to educate and provide
information and support about healthy individuals, families and communities based on a holistic
approach to health and health care. He/she will perform a wide variety of consulting and support
services to individuals and groups that encourage prevention, and intervention when necessary.
He/she will provide up-to-date information and resources to promote healthy lifestyles through
education, immunization and clinics. The incumbent also monitors the home and community to
identify the required resources and interventions. He/she is required to research and produce
community relevant resources to ensure that the individuals in the community understand and
appreciate the information being provided.



RESPONSIBILITIES

encountered in carrying out the responsibilities.)

      1. Provide administrative support to the Health Board

           Main Activities

           complete and distribute all correspondence
           
           maintain current, accurate and secure files
           
           attend all Health Board Meetings
           
           record, distribute and file minutes of all meetings
           
           coordinate and develop monthly activity reports
           
           coordinate and develop monthly and quarterly reports for the GNWT Department of
           
             Health and Social Services
           assess facilities and equipment and identify needs
           
           coordinate and develop the Annual Report and Corporate Plan
           

      coordinate effective and efficient management of the office
      
      ensure incorporation of the Board
      

    2. Liaise with interested community, territorial and federal groups

      Main Activities

      attend out-of-town meetings as appropriate
      
      liaise with Chief and Council, and Band Manager on community Health issues
      
      liaise with NWT Health Care Association and GNWT Department of Health and
      
        Social Services
      provide the required monthly, quarterly and annual reports and documents to the
      
        GNWT Department of Health and Social Services
      negotiate amendments to the Transfer Agreement as required
      

    3. Perform other related duties as required

The incumbent must be knowledgeable of a holistic approach to individual and Community
Health legislation, theories, practices and procedures as well as Traditional Medicine and
medical terminology. He/she must have current Infant/Child CPR, advanced First Aid and a valid

interpersonal and analytical skills. The incumbent must be able to work effectively in a variety of
settings and in a cross-cultural situation and be knowledgeable of the services available in the
community.

The knowledge, skills and abilities required for this position would normally be acquired through
completion of a recognized Community Health Representative Program.

Physical Demands
(The nature of physical effort leading to physical fatigue)
There are some physical demands associated with this position. The incumbent must spend the
majority of his/her time using a computer which can cause both mental and physical fatigue.

Environmental Conditions
(The nature of adverse environmental conditions affecting the incumbent)
The incumbent works in a very busy open area where there are constant distractions and
disruptions. The incumbent also works in an environment and will therefore be exposed to
people in difficult and dangerous situations and may be required to react in a safe and efficient
manner. The incumbent must spend the majority of his/her work day operating a computer.

The incumbent may find that they are exposed to sights and sounds associated with patients who
are ill and injured. The incumbent must spend the majority of his/her work day operating a
computer which requires a great deal of concentration, speed and accuracy. Operating a
computer can also cause eye, hand and muscle strain.

Mental Demands
(Conditions that may lead to mental or emotional fatigue)
Stress is caused by the volume of work and the need to meet tight and often conflicting
deadlines. Stress is also caused by the routine and repetitive nature of the work, by constant
interruptions and by the requirement to deal with people who are ill or suffering from an injury
and may therefore be agitated, angry or in emergency situations.
