              MAKING AN APPLICATION FOR COUNCIL CONSENT FOR
            DISPENSATION TO BUILDING REGULATION REQUIREMENTS

To make an Application for Council Report and Consent for a dispensation from
requirement/s of Building Regulations 2006,

      1       Name and address of the applicant.
              (Where this is not the owner, please provide owners written consent.)

      2       Applicant's contact information :- Phone number, Mobile phone number, Fax
              number and or E-mail address.

      3       Outline in detail of the reasons for seeking special consideration.
              You are encouraged to list every relevant reason why you should be granted
              consent. They must address the ministerial guidelines and other requirements.

      4       Applications may be accompanied with either a non-objection form or an
              objection form and reasons for objection from adjoining affected owners.

      5       Copy of properly prepared plans which clearly demonstrate the proposal
               in terms of its appearance and dimension. These plans must be endorsed
               by the adjoining owners if they have no objections. The plans should include
               the following information items dependant on the application requirements.

              1) Copy of Title and Plan of Subdivision (endorsed by Titles Office) and Covenant
              2) Developer's consent and endorsed plans if required by covenant.
              3) Site plan showing :-
                 a)    the position and distance from surrounding boundaries and all other
                       buildings on site,
                 b)    position and dimension of building on adjoining allotments,
                 c)    location of block in relation to nearest corner.

              4) Floor plan and elevations of new & existing buildings.

              5) Landscape plans & other plans or details requested by Municipal Building
                 Surveyor.

      6        A $222.30 fee is payable for each regulation requiring variation or consent

      7       Further information may be requested by the Municipal Building Surveyor after
                submission of the application to clarify any part of the proposal.
