					         REQUEST FOR EXTENSION OF THE PERMITTED IN-WATER WORK WINDOW and/or
     BIOLOGICAL ASSESSMENT/BIOLOGICAL OPINION AND CONCURRENCE LETTER REVISIONS


                          FORM PROCESS AND DIRECTIONS

The purpose of this joint form is to facilitate the process by which the biological assessment,
biological opinion, concurrence letter and/or permitted in-water timing can be revised when
necessary. This also clarifies the process that the Project Manager/Maintenance Manager must
complete prior to and after the permit(s) are revised. The completed form also will document that
all the appropriate agencies have agreed to the proposed change or revision.            When the
appropriate sections of this form are completed, the form will be utilized to amend the in-water
date conditions stated in the permits. The completed form (including all reference material) will be
attached to the current permit package and must remain on the project site. Notice to proceed will
not occur until this completed application is returned to the Project Manger’s /Maintenance
Manager’s office.

                       Directions for in-water window extensions Section 1

Any change or revision (activity or timing) to the permitted in-water work date requires a permit
amendment.

The Project Manager/Maintenance Manager will initiate the amendment process by completing all
fields in Section 1. Project Manager/Maintenance Manager should document discussions and
individuals contacted (ODFW, ODOT and regulatory biologists) and attach to the form. The
completed form (with attachments) will then be sent to the ODOT Permit Coordinator for
processing (Ken Franklin). The Permit Coordinator will circulate the form and attachments to the
relevant personnel (ODOT Biologist, Region Environmental Coordinator, ODFW, USACE, DSL,
NMFS/USFWS). When all agencies (relevant to the project) concur with the revision, (see Section
3), the Permit Coordinator will send the form and all relevant reference material(s) to the Project
Manager’s/Maintenance Manager’s office, and appropriate agencies, Region Environmental
Coordinator and Biologist.

Directions for amendment(s) to the biological assessment, opinion or concurrence letter Section 2

Any change or revision to the activities defined in the biological assessment, biological opinion or
the concurrence letter, and which changes the effects to the threatened or endangered species,
requires a permit amendment.

The Project Manager/Maintenance Manager will initiate the amendment process by completing all
fields in Section 1. Project Manager/Maintenance Manager should document discussions and
individuals contacted (ODFW, ODOT and Regulatory Biologists) and attach to the form. The
completed form (with attachments) will then be sent to the ODOT Biologist. The ODOT Biologist
will complete Section 2 and Section 3. If necessary, the ODOT Biologist will circulate the completed
form to the relevant personnel, including the Permit Coordinator. The Permit Coordinator will
submit the form to the DSL and USACE, if appropriate. When all agencies (relevant to the project)
concur with the revision, (see Section 3), the ODOT Biologist will send the form and all relevant
reference material(s) to the Project Manger’s/Maintenance Manager’s office, and appropriate
agencies, Region Environmental Coordinator and Permit Coordinator.
