      Job Description:

      Plans, directs, and coordinates activities of the operating room Department.
      Maintains optimum standard of nursing care and practice through preoperative
      And postoperative assessment planning, implementation, and evaluation of
      patient care.


      Job Summary:

      Provides professional nursing care in assigned unit. Performs frequent patient
      evaluations, including monitoring and tracking vital signs, performing
      procedures such as IV placement, and administering medications. Develops the
      day-to-day nursing care plans both in hospital, and for care after discharge by
      families.

      Specific Duties and Responsibilities:
      1. Scrub-nurse
              - Identifies and prepares the needed instruments/ supplies/ equipment
                  For the surgery
              -   Takes and record vital signs
              -   Administer intravenous fluids
              -   Administer oxygen if needed
              -   Identifies the patient properly, knowing his indications for surgery and
                  Assesses patient’s status along with the circulating nurse.
              -   Prepares the back table (inc. opening sterile packs/instruments/supplies)
                  ahead
              -   Does the Scrubbing, gowning and gloving aseptically.
              -   Secure results of laboratory test and other request
              -   Prepares the sterile mayo table accordingly and arranges the
                  instruments
                  Properly .
              -   Does the accounting of all sponges (inc. visceral packs, peanuts,
                  cherries, etc), instruments and needles carefully before, during and after
                  the surgery together with the circulating nurse
              -   Rechecks the functionality of each instruments, prepares the cautery
                  pencil and suction pool.
              -   Notifies the circulating nurse regarding the time to which the operation
                  has started and ended, and the time the specimen was cut out.
    -   Anticipates and serves instruments needed by the surgeon.
    -   Does the cleaning of each instrument after each use and prepares it for
        the next one.
    -   Assist the surgeon or his assistant in applying wound dressing.
    -   Takes charge of the specimen after surgery and endorses it to the
        circulating nurse.
    -   Takes care of the used instruments by rinsing it under running water,
        brushing and soaking these in a disinfectant solution.
    -   Segregates and discards the used needles/ sutures and blades in a puncture proof
        container.
    -   Allowing the instruments to be air dried and does the repackaging of
        these items for autoclave purposes.
    -   Develop and manage nursing care plan for the patient.
    -   Participates in general and medical staff meeting as required
    -   Participates in hospital and nursing activities and formal education
        programs.
    -   Applies principles of aseptic technique in performing surgical scrub,
        gowning, gloving, setting up instruments and draping of patient as
        needed in the procedure

2. Circulating Nurse and Recovery Room

     Circulating Nurse
        - Maintains optimum standards of nursing care and practice through
           preoperative and postoperative assessment planning,
           implementation, and evaluation of patient care.
        - Provides professional nursing care to patients undergoing surgery.
           Prepares and maintains operating room for all scheduled and
           emergency procedures.
        - Maintains open communication lines to provide for total care needs
           of the patient.
        - Maintains concise and complete surgical records and reports.
        - Receives patient from floor and/ or E.R NOD
        - Identifies the patient properly, knowing his indication/s for surgery
           and assesses patient’s status along with the scrub nurse.
        - Receives endorsement regarding doctor’s orders and other
            Pertinent data.
        -   Checks patient’s chart regarding requirements prior to
            Surgery such as consent for operation and anesthesia
            Including Cardio-pulmonary clearance postoperative and
            Pre-operative checklist.
        -   Accompanies patient from room to OR table.
        -   Attaches all necessary apparatus needed for surgery.
        -   Takes initial vital signs of the patient and identifies any unusualities.
        -   Writes on baseline information to the OR theatre’s whiteboard.
        -    Fills up information regarding patient to operative and anesthesia
            records as well as sundry and blue prescription pads.
        -   Assist in the induction of anesthesia.
        -   Notifies and records the time of the induction, to which the operation
            has started and ended as well as the time when the specimen was
            cut out.
        -   Does the catheterization and skin preparation observing aseptic
            technique
        -   Assist the scrub nurse in accounting the sponges, instruments and
            needles before, during and after the surgery.
        -   Does the charting and proper documentations.
        -   Anticipates the need of the surgical team.
        -   Administers due medications and accounting all medicines and
            supplies used.
        -   Assist in the aftercare.
        -   Monitors patient’s status at the recovery room as necessary.
        -   Catties out Doctors’ Order.
        -   Informs the floor NOD to prepare necessary equipments and
            materials needed by the patient.
        -   Transport out patient back to his room.
                   -   Endorses the patient to floor NOD at bedside and reviews the post
                       operative orders with the floor NOD.
                   -   Sends specimen/s and x-ray films to respective units.



                 Recovery Room
                  - Position patients comfortably on bed
                  - Restraint patients if necessary
                  - Carry out all the doctors order
                  - Monitor and record Vital Signs of patient
                  - Administers oxygen therapy and suctions secretions if needed
                  - Apply warm or cold compress if needed
                  - Gives medications to patient thru IV push, Intramuscular,
                     subcutaneous, intradermal and skin test as ordered by the
                     anesthesiologist or surgeon
                  - Refers any untoward reactions to anesthesiologist or surgeon
                  - Attends to the needs of patients
                  -   Gives psychological reassurance to patient and relatives
