Who we are and what we do
Sue Ryder Care specialises in providing neurological and hospice care. Our hospices
and neurological care centres offer long-term and respite residential care, day care and
home care services. Sue Ryder Care exists to relieve human suffering, to empower
those most in need of our services and to help people and their families overcome the
challenges they face. Sue Ryder Care is about ‘Care that Liberates Lives’ specialising
in providing neurological and hospice care.

Last year Sue Ryder Care:

     cared for 4,222 people in our 17 care centres
     made over 415,000 home care visits in the UK
     welcomed 9,269 patients to our day care centres

The need for our services

Hospice care – Our hospice care supports people with cancer and other life shortening
conditions in supportive surroundings. We aim to meet the needs of the whole person,
and a range of therapies including physiotherapy, massage and aromatherapy are
offered. The needs of our patients’ families and friends are also provided for, with long-
term bereavement counselling.

Neurological care – In the UK five million people are affected, directly or indirectly, by
neurological conditions such as Multiple Sclerosis, Stroke, Cerebral Palsy, Parkinson’s
Disease, Motor Neurone Disease, Huntingdon’s Disease and Acquired/Traumatic Brain
Injury. Many of the people affected by these debilitating and life-shortening conditions
are in their 30s and 40s and may need our care for 10, 20 or even 30 years. We have
specialised knowledge of caring for people with neurological conditions and understand
that their needs are not just physical, but also emotional, social and spiritual. At Sue
Ryder Care, we cater for all of the person’s needs.

Homecare Services – More and more people are being cared for in the comfort of their
own homes, surrounded by a network of family, loved ones and friends. Sue Ryder
Care Homecare Services helps make this possible. Homecare adapts to meet the
changing needs of those it cares for, providing care for the whole person with access to
specialised services as required.

About the role
Located in glorious countryside, Sue Ryder Care – Leckhampton Court Hospice
provides specialist palliative care for people with cancer and other life limiting illnesses.
The hospice provides a holistic approach to the assessment of the palliative care needs
of each person and their family encompassing their physical, psychological, social and
spiritual needs. The hospice is based within a 14th century manor house and provides
Day Hospices inclusive of Day Therapy and Hospice at Home for local people in
Cheltenham. It is the only provider of specialist palliative care inpatient hospice facilities
within Gloucestershire. Wherever possible, the hospice aims to enable people to return
to their own home environment and enable them to maintain maximum independence
and self-esteem.

During 2007 we have extended the services we provide for the people of
Gloucestershire, these developments are supported by the local Primary Care Trust and
include the appointment of a Consultant in Palliative Medicine, and the development of
a Hospice at Home service. We have also been awarded a £400K capital grant by the
Department of Health as part of their Developing Services for Older People’s
programme; this will support the redevelopment of the public areas on the ground floor
of the main Hospice building including the development of a new reception area and
information point.

The Role
    Provide clinical leadership and direction to the Day Hospice Service at
     Leckhampton Court Hospice.
    Develop and efficiently manage nurse led clinics to enable patients to have a
     range of therapies as day patients including: ascitic taps, infusions and
     transfusions
    Work in collaboration with Palliative Care Services Manager and Head of Clinical
     Care to identify areas for development and assist in the formulation and
     implementation of strategies that underpin professional and organisation
     business developments
    Act as a resource to, advise and support health care professionals involved in the
     care and management of patients and families referred to the Day Hospice and
     the Hospice at Home Service.
    Provide specialist palliative care advice, support and interventions for those
     individuals referred to the Day Hospice at Leckhampton Court.

Continuous Service Improvement
In December 2005 Sue Ryder Care launched a strategy for Integrated Governance
which is being implemented at a local level and led by the Head of Clinical Care and
Clinical Governance Facilitator. In line with this, we believe that the standards by which
the Healthcare Commission and Environmental Health Agency inspect us are the
minimum we should provide for our patients, their family and friends. You will therefore

Job Summary

       Provide clinical leadership, specialist palliative care advice and direction to the
        Day Hospice at Leckhampton Court Hospice
       Manage efficiently and effectively all aspects of the clinical care provided by the
        Day Hospice at Leckhampton Court
       To maintain and develop enhanced clinical knowledge and skills to perform
        ascitic taps, infusions and transfusions.
       Work in collaboration with Palliative Care Services Manager, Consultant in
        Palliative Medicine and Head of Clinical Care to identify areas for development
        and assist in the formulation and implementation of strategies that underpin
        professional and organisation business developments
       Provide specialist palliative care advice, support and interventions for those
        individuals referred to the Day Hospice at Leckhampton Court

Clinical/Technical Responsibilities

       To be responsible for a patient caseload and provide expert specialist palliative
        care advice
       To synthesize complex information and consider a range of options based on the
        best available evidence/best practice to enable patients access a range of
        therapies as day patients including: ascitic taps; infusions and transfusions.
        Obtain written informed consent for procedures, adhering to Sue Ryder Care
        policies and procedures.
       Consult and liaise with the multi-disciplinary team, health and social care
        colleagues, service users, family/carers over their care, and provide specialist
        palliative care knowledge and advice to all relevant staff
       Assess potential patients in terms of appropriateness and priority of need and
        ensure that effective communication systems are in place for supporting their
        care and accessing the service
       Provide specialist palliative care advice, support and interventions for individuals
        referred to the Hospice at Home service

      Liaise and communicate with other health and social care professionals in Sue
       Ryder Care and outside agencies in the planning and delivery of patient care
      Act as specialist resource, to support the District Nurse in the assessment,
       planning, implementation and evaluation of individualised care using evidence
       based guidelines
      Establish effective working relationships with the Day Hospice, and external
       agencies and attend the weekly MDT meeting.

       care and management of patients and families referred to the Day Hospice at
       Leckhampton Court


Clinical Governance/Quality

      Ensure that all patients (or when they are not able, relatives) are given the
       opportunity to participate fully in the care planning process.
      Encourage innovative ideas to improve the service provision and take the service
       forward in line with Sue Ryder Care, PCT and government initiatives
      Promote evidence-based practice, developing clinical guidelines and standards
       and auditing their effectiveness
      To be aware of, and comply with, policies, procedures and guidelines relating to
       nursing issues and organisational matters including Health and Safety
      Responsible for the co-ordination of Health and Safety training and risk
       assessments for nurses working in patient’s homes
      Monitor, supervise and support Hospice at Home team with their performance
       through the use of qualitative measures including clinical governance; clinical risk
       management, audit, clinical supervision, education and training

Managerial Responsibilities

      Foster good relationships between all nursing teams, departments, professions
       and personnel both internal to Sue Ryder Care and with external service
       providers
      Lead clinical developments with the multi-disciplinary team to enhance
       professional working across the Day Hospice at Leckhampton Court Hospice
      Represent the Hospice locally, regionally and nationally as required and deputise
       for the Head of Clinical Care
      Develop and promote IT provision, which integrates patient records, and
       statistical data and focuses on effective communication within the Hospice
       network and with General Practitioner surgeries and hospitals
      Ensure that any clinical incidents, near misses and complaints are reported
       appropriately in accordance with Sue Ryder Care policy and procedure
      Ensure that any compliments and examples of good practice are recognised and
       shared appropriately
      To develop, manage and evaluate a nurse led clinic to enable patients to have
       ascitic taps, infusions and transfusions as day patients
