					               Physician Information: New Medication Reconciliation Process
                          Starting August 29, 2010 (Post-Upgrade)

The Horizon Clinicals and Portal applications are being upgraded on August 28. The new
versions will be available on August 29. Once the upgrade is complete, the RxReconcile
application will no longer be available.

Admitting Medication Reconciliation:
       This process is no different than the current process, other than the way the report looks
       and that the nurse prints from Care Manager instead of RxReconcile.

       The nurse will enter/confirm home medications in Care Manager.
       The nurse will print the ‘admitting medication reconciliation report’.
       The nurse will place the admit med rec report on the chart under the physician orders tab.
       The physician will check boxes on paper of which home medications to continue or
        discontinue.
       The pharmacy is sent a copy of the admitting medication reconciliation.
       The admit med rec report will be placed under the physician orders tab.

Transfer Medication Reconciliation:
       This process is no different than the current process, other than the way the report looks
       and that the nurse prints from Care Manager instead of RxReconcile.

       The nurse prints the ‘transfer medication reconciliation’ from Care Manager.
       The nurse will place the transfer med rec report on the chart under the physician orders
        tab.
       Paper Process:
             o The physician will check boxes on paper of which home medications and active
                hospital medications to continue or discontinue.
             o The pharmacy is sent a copy of the admitting medication reconciliation.
       The nurse will place the transfer med rec report on the chart under the physician orders
        tab.
       CPOE Process:
             o **Physicians entering orders in CPOE/HEO continue with the current process.
             o Physicians using CPOE/HEO are not required to use the paper transfer med rec
                report.

Discharge Medication Reconciliation:

      The physician prints the ‘discharge medication reconciliation’ report from Portal
       (Flowsheet tab, Report Viewer sub-tab).
    The physician will check boxes on paper of which home and active hospital medications
       will be continued at discharge. Physician writes prescriptions. Bring prescription pads.
    The nurse enters the physicians discharge medication reconciliation orders in Care
       Manager under the home medications (same place as admission home medications). This
       will include confirming, adding and discontinuing medications as appropriate according
       to the physician discharge medication reconciliation.
    The nurse prints 2 copies of the report ‘UMF Patient Copy’ from Care Manager.
    Nurse compares the UMF with the discharge med rec for accuracy.
            o The patient signs one copy of the UMF Patient Copy, which the nurse keeps and
                puts on the chart to be scanned into the medical record.
            o One copy goes home with the patient
For any telephone med rec orders, the read back policy must be followed, therefore, home
meds and active meds on the med rec report will be read back to the physician.
