Form 1: Application form cover
   - Should write in Japanese only.
Form 2: Curriculum vitae for applicant, Japanese researcher
   - Certified by applicant’s director of his/her affiliation.
   - Signature should be handwritten, NOT typed
Form 3: Curriculum vitae for African researcher
   - Should write in English only (for the section 13-17).
   - African researchers should be under forty years of age.
   - Certified by nominee’s director of his/her affiliation.
   - Signature should be handwritten, NOT typed.
Form 4: Curriculum vitae for assistant instructor
   - Certified by the director of his/her affiliation.
   - Signature should be handwritten, NOT typed.
   - Describe assistant instructor’s relationship with Japanese researcher (for the section 14).
Form 5: Training and research plan/ Budget plan
   - Equipment cannot be purchased. (personal computer, printer, digital camera, furniture,
   scientific equipment, etc.)
   - Travel expenses and settling allowances should be calculated based on the rule of host
   institutions.
   - Should fully consider “evaluation criteria for selection” (section 9 of application
   guidelines).
Recommendation letter of host institution
   - Should write on letterhead paper.
   - Signature should be handwritten, NOT typed.
